A Guardian signed in through single sign-on with a second factor enrolled could not hand a managed profile over once five minutes had passed since sign-in: the handover answered 401 auth.stepup.required, and the panel printed "Confirm your second factor, then try again" with no way to do so. The same held for every action behind requireFreshMfa: creating, editing and ending a managed profile, adding and removing a Guardian, disabling the second factor, rotating recovery codes, removing a security key or a passkey, changing the password, the data wipe, account deletion and the key rotation. The recent-proof routes had the dialog; these had text. - requireFreshMfa names the proofs that clear a stale stamp in meta.methods (the possession proofs the account holds, never a password, which stamps reproofAt only). - The re-proof flow treats that refusal like auth.reproof.required: open the dialog, post the proof to /api/auth/reproof, send the request once more. The flow moves into a React-free module so it can be driven in tests; the hook binds it to the dialog. - The managed-profile mutation hooks take a required proof guard, and every caller passes recentProof.run. The other surfaces wrap their requests in recentProof.run; disabling the second factor asks before its confirmation opens, so the code typed there is not spent twice. - An account with nothing to re-prove in place (single sign-on only, no factor) is told to sign in again when adding a factor, instead of being offered a password it does not have. - step-up-web-caller-guard freezes the gated route methods and fails when a web request to one of them runs outside the flow. An account whose only second factor is the identity provider's is still asked for an app factor on the managed-profile acts: the app cannot see the provider's factor. Enrolling right after a sign-in needs no further proof, and the confirmation stamps the session, so the act follows. Refs #959
docs/ — internal reference + operator playbooks
The user-facing documentation lives at https://docs.healthlog.dev — that's the canonical surface for self-hosting guides, integration walkthroughs, AI-provider setup, and the doctor-report workflow.
The files in this directory are the internal operator playbooks, audit notes, and machine-readable specs that ship alongside the source:
api/— the OpenAPI 3.1 spec for the native-client subset (iOS DTO codegen target). Seeapi/README.mdfor the preview commands. Also home to the MCP-server contract:api/mcp-capabilities.md(tools, resources, prompts, write model) andapi/mcp-skills.md(building a connector / skill).ops/— operator runbooks: deploy, backup / restore, encryption-key rotation, password reset and account recovery, env checks, TLS pinning, data repair. These are what an operator reaches for during an incident.self-hosting/— horizontal-scaling notes (HEALTHLOG_PROCESS_TYPE=web|worker|all) and the deploy-pipeline recipe. Routine install steps stay ondocs.healthlog.dev. Includesself-hosting/mcp.md— enabling the off-by-default MCP connector and pointing Claude.ai / ChatGPT / Claude Desktop at it, andself-hosting/sso.md— OIDC SSO login: IdP setup, the identity-pinning security model,OIDC_ONLYconsequences, and the break-glass runbook. Alsoself-hosting/geolite2.md— bringing your own offline GeoLite2 databases viaGEOLITE2_DIRinstead of sending login IPs to a third-party lookup. Andself-hosting/account-sharing.md— letting one account on the instance open another's health record: the three access levels, the eight sections a grant can be narrowed to, managed profiles for somebody with no login and the guardians who look after them, what revocation does and does not undo, and which trust properties of the instance are unchanged by any of it. Andself-hosting/support.md— reporting a problem: error ids, the support info every account can copy, the operator's redacted support report, and finding an error in the admin log.using/— notes on using the app that sit close to the code.using/search.mdcovers the command palette's record search: opening it, what it finds, how matching works, and its limits on very large and shared records.migration/— migration notes. Currently only v1.3 to v1.4; later releases are covered byCHANGELOG.mdand https://docs.healthlog.dev.integrations/— per-integration reference (Withings, Fitbit, Google Health, WHOOP, Apple Health, AI providers, data import).adr/— architecture decision records.security/— threat-model notes.diagrams/— architecture diagrams rendered through docs.healthlog.dev.releases/— release imagery.apple-store-connect-checklist.md— the iOS submission gate.codex-protocol-spec.md— Codex / ChatGPT OAuth protocol reverse-engineering notes.ui-guidelines.md— UI / a11y house style.
