Marc-André Bombeck 30c96ff591 chore(deps): drop pdf-parse and the second pdfjs-dist it carried
Nothing imports pdf-parse any more, so its pdfjs-dist 5.4.296 and the
older @napi-rs/canvas that came with it leave the lockfile. The override
that kept that transitive copy past a renderer advisory has nothing left
to move and goes too. The standalone tracing stops naming pdf-parse; its
pdfjs-dist glob now matches the one copy the app pins. The Dockerfile and
canvas guard comments describe the second copy as history.

Marc-André Bombeck 7fda400a8c fix(i18n): serve English for an unreadable catalog, refuse to boot without it
The server reads messages/<locale>.json from disk on first use. A missing
or corrupt file threw, and since the matchers iterate every locale, one bad
non-English catalog turned every request that touched them into a 500.
That locale is now served the English catalog, and the failure is logged
once as i18n.catalog.fallback with the locale and the reason. English is
the fallback itself: a process whose messages/en.json is missing or does
not parse refuses to start, with the file and the reason in the message.

The image build checks the catalogs twice with
scripts/verify-message-catalogs.mjs: in the builder against the locale
list in the source, and in the runner stage against the copies the image
carries.

Marc-André Bombeck 6dfb7eada6 Merge branch 'release/v1.45.0' into feat/v145-runtime 2026-10-22 09:41:54 +02:00
Marc-André Bombeck 73362dc0ae chore(deps): take the weekly production and development updates
Production: the AWS SDK S3 client and lib-storage 3.1152.0, Prisma
(client, adapter-pg, CLI) 7.10.1, TanStack Query 5.104.3 and Virtual
3.14.21, dotenv 18.0.8, jspdf-autotable 5.0.9, lucide-react 1.59.0,
nodemailer 10.1.2, pg-boss 12.38.0, React and React DOM 19.3.1 (still
exact) and undici 8.12.

React 19.3.1 and undici 8.12.2 are security releases. Prisma 7.10.1
stops a nested interactive transaction from leaking its id into a
later top-level one.

pg-boss moves its schema from 45 to 46; the worker applies the
migration on start and the index builds continue in the background.
Its list and delete helpers now page and batch, which touches only the
queue retirement at boot, and that is idempotent. The retention test
pins the installed version, so it moves to 12.38.0 after re-checking
every source line it asserts.

Development: @axe-core/playwright 4.14.0, Playwright 1.64.1, Vitest and
its coverage provider 5.0.5, shadcn 4.22.0. The Stryker runner patch
still applies; it touches the runner's own name join, and 5.0.5 changes
nothing in how Vitest builds a test's full name. axe-core 4.14 turns on
label-content-name-mismatch for the WCAG 2.1 A tag the a11y specs run.

The S3 client is held on the same release as lib-storage rather than
the newest one its range allows, so the two share one SDK core.

Vite gets a floor at 8.1.6 for GHSA-587h-7wmj-h5pm and four sibling
advisories against its dev server and bundled launch-editor. It arrives
only under Vitest and never reaches the image.

The image's separate Prisma CLI install moves with the app tree to
7.10.1, and its dotenv to 18.0.8, which it had trailed by a patch.

Marc-André Bombeck 2ccac728f4 perf(i18n): read server catalogs from disk, one locale at a time
The eight catalogs were static JSON imports, so every server bundle
carried the whole eight-locale chunk as a source string and parsed all of
them: 32 parsed copies and two 9.7 MB chunk sources in an idle process.
The server now reads messages/<locale>.json the first time a translator,
the SSR cache or a matcher asks for that locale, and keeps one parsed copy
per process on globalThis. allMessages keeps its shape; its entries load on
first read. The image copies messages/ next to the server.

foldLabel moves to a leaf module, so the client medication map no longer
imports the catalog index.

Marc-André Bombeck f5d4dd2ec6 test: mutation testing on the core libraries with per-module floors
Stryker runs against encryption at rest, medication compliance, the rollups,
source ranking, unit conversion and backup validation, with the unit tests
that live next to them (vitest.mutation.config.mts). It is too slow for a
pull request and runs nightly and on demand (.github/workflows/mutation.yml).
scripts/mutation-floors.mjs reads the JSON report and fails when a module
scores below its floor in stryker.floors.json, so one well-tested module
cannot hide a weak one. The floors sit two points below the first measured
scores: crypto 46.9 %, compliance 66.8 %, rollups 41.7 %, source ranking
57.0 %, units 87.4 %, backup validation 25.5 %.

@stryker-mutator/vitest-runner 10.0.0 predates Vitest 5, which names a test
"describe > test" where the runner still built "describe test". Every test
inside a describe block was filtered out of the mutant runs, so nearly every
mutant survived (5.5 % across the set). A pnpm patch builds the name the way
Vitest 5 does; the Docker dependency stage copies the patches directory,
which the frozen install needs.

Marc-André Bombeck d8fcf814b2 fix(docker): a boot storm cannot flip the healthcheck to unhealthy
/api/health gives the database two seconds and answers 503 past that.
With three retries, a pool saturated for a minute after the start period
could mark the container unhealthy and take it out of the proxy's
routing. The compose healthcheck and the image's HEALTHCHECK now allow
five misses (two and a half minutes) and end wget after four seconds,
inside Docker's five, so a stalled request is one clean miss. A test
holds both to these bounds and to each other.

Marc-André Bombeck f6cf7c5e6b fix(docker): give the health check room for a slow migration
The entrypoint migrates before the server listens, and a migration that
waits out a held lock retries for up to about eight minutes, while the
health check marked the container unhealthy after two. The start period
is now ten minutes in the compose file and the image; the first answering
probe still ends it, so a normal boot reports healthy as before.

Marc-André Bombeck 9c89503019 fix(migrations): bound 0412's lock wait where it takes effect and retry a lock timeout on boot
migrate deploy runs a migration file statement by statement, so the
top-level SET LOCAL lock_timeout in 0412 did nothing: the migration
waited as long as the lock was held, with every write to measurements
queued behind it, and a file that failed half-way stayed half-applied.
0412 now takes its locks inside one DO block with a statement-scoped
timeout, which rolls back whole, after an idempotent CREATE OR REPLACE.

The entrypoint runs migrate deploy through scripts/migrate-deploy.mjs.
A 55P03 failure of a migration marked -- @retry-on-lock-timeout is
marked rolled back and retried after 15/30/60/120/240 s, read from
Prisma's report or, after P3009, from the stored row. Every other
failure stays fatal. A guard holds migrations to the shape that makes
the marker true, and 0411/0413 add their columns with IF NOT EXISTS so
a manual re-run is safe too.

Marc-André Bombeck b79bd8370d fix(heap): report again at 95 % and leave a report on an out-of-memory exit
The pressure report re-armed only after the heap fell under 70 % and
half an hour passed, so a heap that climbed to its limit soon after an
85 % report died with only that older report on record. A 95 % tier
now writes the same report at once, whatever the cooldown, and re-arms
under 85 %.

The image sets NODE_OPTIONS so Node writes its diagnostic report to
stderr when the heap runs out, with environment variables and network
interfaces excluded. The heap-diagnostics and scaling docs say how to
keep the flags when overriding NODE_OPTIONS.

Heap figures share one megabytes helper with the support report.

Marc-André Bombeck 369775e039 chore(deps): move dotenv to 18
The breaking changes in 18 do not reach this repository: nothing preloads
dotenv with -r, nothing reads a .env.vault, and both entry points in use
(the default export in prisma.config.ts and the side-effect dotenv/config
import in the one-shot scripts) keep their behaviour. dotenv/config stays
quiet by default.

What does change is the load message, which now goes to stderr. Prisma
reads prisma.config.ts on every migrate deploy at container start, so
both loads there pass quiet: true rather than adding two stderr lines to
each boot. The image's /opt/prisma-cli install, which the config resolves
dotenv from, moves to the same 18.0.6.

Marc-André Bombeck b63fa317f3 fix(docker): hoist the canvas binary of the pinned version
Refs #1124. The store holds two @napi-rs/canvas versions, the pinned one
and an older one that pdf-parse's own pdfjs-dist pulls in. The runner
stage took the musl binary with `find | head -1`, got the old one, and
copied it into the new loader's directory, where the loader looks first.
Rendering still worked, but `loadImage` calls `Image.decode`, which the
old binary lacks, so every preview thumbnail failed with "image.decode is
not a function" on both architectures.

The builder now records the resolved canvas version and the runner hoists
that version by name, as it already does for pdfjs-dist. The build stops
when the binary it copies belongs to another version.

Marc-André Bombeck 13db7935f6 fix(image): ship the operator scripts the runbooks run in the container
The password-reset runbook tells an operator to run
`docker compose exec app node scripts/reset-password.mjs`, and the
intake-repair runbook runs `healthlog-tsx scripts/repair-intake-anomalies.ts`
inside the container. The runner stage copied nothing from scripts/, so
both failed with "Cannot find module", the reset at the moment an admin
is locked out.

The runner stage now copies both scripts and the Argon2 parameter module
the reset imports, and proves at build time that each reaches its own
first check with every import resolved. A structural test holds every
in-container command in docs/ and the Dockerfile copies in step.

Key rotation is documented the way it works on the published image: the
Rotate now button under Admin > Encryption is the primary path. The
rotation script imports the application graph and runs from a source
checkout of the same release; the runbook, the admin hint in all seven
languages and the decrypt error now say so.

Marc-André Bombeck 36ff3e7cb9 chore(deps): move the Prisma client and the image CLI to 7.10
The bump raised the CLI to 7.10.0 but left @prisma/client at 7.8.0 and
the migration CLI the image installs under /opt/prisma-cli at 7.8.0, so
the generated client, the one that ships and the one that migrates would
have come from two releases. All three are 7.10.0 now.

Marc-André Bombeck c9f8aee476 build(docker): mirror the hono and valibot pins into /opt/prisma-cli
The image carries two installs. pnpm builds the app tree from
pnpm-workspace.yaml; npm builds /opt/prisma-cli and reads only its own
overrides field. The scan reported @hono/node-server@1.19.11
(GHSA-frvp-7c67-39w9, CVE-2026-39406) and valibot@1.2.0
(CVE-2026-59952) under that path while both packages were pinned and
clean in the app tree. Same shape as the deepmerge-ts finding, same
cause.

Rebuilt this exact install both ways to confirm rather than assume.
Without the two fields npm resolves @hono/node-server 1.19.11 and
valibot 1.2.0, which are the two versions the scan named. With them it
resolves 1.19.17 and 1.4.2, and prisma --version still reports 7.8.0.
Both arrive through prisma -> @prisma/dev.

The lockstep guard could not have caught this. Its workspace matcher
started a package name at [^"@], so every scoped key was invisible to
it: @hono/node-server was in the workspace file and absent from the
Dockerfile, the shared set never contained it, and the guard stayed
green. Scoped keys parse now, a test pins the reduction itself so the
blind spot cannot return quietly, and a second test asserts that the
three packages the image scan has actually flagged under /opt/prisma-cli
keep their npm-side mirror. Both fail when the pin is removed or moved
out of step.

Marc-André Bombeck 97b1603cb9 Pin mysql2 past GHSA-3f6p-5ww8-9rcr in both installs
The dependency audit went red on the release branch: mysql2 below 3.22.0
downgrades its auth plugin to mysql_clear_password and leaks the
credential. It reaches the tree only through prisma's MySQL driver, which
this app never opens, but the gate does not distinguish reachable from
present. Pinned to ^3.22.0 in the workspace overrides and mirrored with
npm's own override for the /opt/prisma-cli install, the pair the
lockstep guard holds together. The audit reads three moderates, below
the gate.

Marc-André Bombeck 9f07a46633 fix: declare pg as the runtime dependency it already was
The Dockerfile asserts that the standalone image can resolve pg, because
pg-boss loads it through native Node resolution. That assertion has been
passing on a coincidence: pg sat in devDependencies, and the version our own
entry pinned happened to match the one @prisma/adapter-pg pulled in, so both
names resolved to a single copy in the store and Next's file tracer packed it
whole.

Bumping the adapter to 7.10.0 moves its transitive pg to 8.23.0 while ours
stayed at 8.22.0. The tracer packs only the copy the adapter reaches, the
top-level link points at the other one, and the image fails to build on both
architectures:

    Error: Cannot find module '/app/node_modules/pg/lib/index.js'

Move pg into dependencies at ^8.23.0. The lockfile now carries one pg version
instead of two, so the two resolutions cannot drift apart again on the next
adapter release. Carries the adapter bump from #865, which cannot build alone.

Marc-André Bombeck bed7278e78 Pin the Prisma CLI install to exact versions
The /opt/prisma-cli npm install named prisma@7.8 and @prisma/engines@7.8
while its neighbours (tsx, dotenv) were exact — the one unpinned spot the
Scorecard pinned-dependencies probe flagged. Both ranges resolve to 7.8.0
today, so the exact pin changes nothing in the built image; it only stops
a future 7.8.x from arriving unreviewed.

Resolves Scorecard alert 63.

Marc-André Bombeck 4ce0e6c5ce Patch OS packages in the runtime image at build time
Trivy flags CVE-2026-14456 (openssl denial of service via unbounded
memory growth in the QUIC server) in the runner stage: the pinned
node:22-alpine digest ships libssl3/libcrypto3 3.5.7-r0 and the fix is
published as 3.5.8-r0 in the Alpine repo. An apk upgrade in the runner
stage picks up published OS security fixes without giving up the digest
pin, closing the window between a CVE landing in the secdb and the next
base-image rebuild.
2026-08-26 19:39:51 +02:00
Marc-André Bombeck 2e5ec7f49f Pin deepmerge-ts in the npm install the workspace override cannot reach
The image builds two dependency trees. `pnpm install` builds the app tree and
honours `pnpm-workspace.yaml`. A second, npm-owned install builds
`/opt/prisma-cli` for the migration CLI, and it reads none of that file.

Only the first was pinned. `prisma@7.8.0` resolves `@prisma/config@7.8.0`,
which resolves `deepmerge-ts@7.1.5`, so the Trivy image scan reported
CVE-2026-40345 (HIGH, stack exhaustion on recursive merges) against
`/opt/prisma-cli` on a commit whose dependency audit over the app tree was
green. The pin read as complete because the check that could see the gap runs
against the artefact, and the one that ran against the lockfile could not.

`overrides` is npm's own mechanism, so `npm pkg set` writes it into the
package.json `npm init -y` just created, before anything resolves. Rebuilding
this exact install both ways: without the field `deepmerge-ts@7.1.5` lands,
with it `8.0.1` lands and `prisma --version` still reports 7.8.0.

The guard beside it fails when the two files name one package at two ranges,
and asserts a non-zero match count first, so a matcher that stops matching
fails loudly instead of passing vacuously. It compares declared ranges only —
a new vulnerable transitive that neither file mentions remains something only
the image scan can see, and that job runs on main-push, not on pull requests.

Marc-André Bombeck 869d91c4c2 Fetch the GeoLite2 databases at runtime when a licence key is set
The offline geo tier only reached a self-host two ways: baked into the
image at build time, or mounted from the host. Both need something the
operator of a stock `:latest` pull does not have. Add a third: set
MAXMIND_LICENSE_KEY and the worker downloads GeoLite2-City + GeoLite2-ASN
itself into GEOLITE2_DIR, so the offline tier starts working with no
rebuild and no manual mount.

A worker-only pg-boss queue drives it. Boot discovery fetches once when a
key is set and no database is present yet; a monthly cron (past MaxMind's
first-Tuesday reissue) keeps a keyed host current. The fetch is fail-soft:
each edition extracts to a temp file in the target directory and is
promoted with an atomic rename only after both are in hand, so a bad key
or an upstream error leaves the previous databases exactly as they were
and never writes a partial `.mmdb`. After a successful place the reader
cache resets so a running process picks up the files without a restart.

Extraction is pure Node (zlib + a minimal tar reader), not a shell-out:
this module is reachable from the worker-boot import chain, and a
child_process import there makes Next's file tracer pull the whole project
into the standalone output. zlib keeps the trace bounded and drops the
busybox runtime dependency. The download goes through safeFetch with the
public-host pin and redirect-following, since the MaxMind permalink 302s
to a signed CDN.

The runtime image now owns /opt/geolite2 as the unprivileged app user so
the worker can write it; a read-only bring-your-own mount at that path
still wins, because the fetch cannot write over it and fails soft.

Marc-André Bombeck 315ca5e67c fix(deps): move the PDF renderer past a script-execution flaw
pdfjs-dist 6.1.200 and a transitive 5.6.205 both sit in the affected range
of a flaw that lets a crafted PDF run JavaScript inside the renderer. The
document vault feeds it every uploaded PDF, so pin the direct dependency to
6.2.108 and add a bounded override so the transitive copy lands there too.
The override stops short of pdf-parse's 5.4.296, which predates the flaw and
drives the local text-extraction path.

The runtime image ships every pdfjs-dist copy in the store, not just the one
that gets loaded, so an unused transitive copy still counted.

Hoisting in the image picked whichever copy the filesystem listed first, and
more than one copy reaches it, so the version named in package.json was not
reliably the one the renderer resolved. Carry the resolved version forward
from the build stage, select it by name, and fail the build when it is
missing rather than leaving another copy in its place.

Marc-André Bombeck a89e511359 fix(documents): run pdfjs + canvas un-bundled so scanned PDFs actually rasterize
The bundled (Turbopack) copy of pdfjs-dist failed its render path in the
standalone image with a bare Error, so a scanned PDF on a non-Anthropic
provider fell back to 'PDF scanning needs a Claude vision provider' — while the
identical un-bundled module rasterizes the same document fine (verified in the
container end to end on the real document). Externalize pdfjs-dist and
@napi-rs/canvas so the server loads the real modules, and hoist pdfjs-dist to
the top level in the image so the runtime import resolves. Also log the render
error message (not just its name) and drop the redundant 'not saved' note from
the document summary panel.

Marc-André Bombeck 0e7514f7b0 fix(documents): hoist @napi-rs/canvas so pdfjs can load it in the standalone image
PDF rasterization failed at runtime — pdfjs-dist does a bare
require('@napi-rs/canvas') from its own directory, and the standalone image had
no top-level @napi-rs/canvas for Node's upward resolution to find, so pdfjs
could not polyfill DOMMatrix and every scanned-PDF read fell back to
'PDF scanning needs a Claude vision provider'. Hoist the canvas package and its
musl binary to /app/node_modules/@napi-rs via symlink into the pnpm store (the
prior per-package .node copy only fixed canvas's own loader, not pdfjs's
require). Verified in the container: resolves from pdfjs and renders.

Marc-André Bombeck 324d4bb411 chore(release): v1.4.44 — REG-11 dashboard summary fix (iOS Home tile sparkline + value)
iOS-operator-blocking hotfix for REG-11 — Home dashboard tile renders
neither chart nor latest value for BP / Puls / Körperfett when the
most recent reading is older than 7 days. Root cause was in
`/api/dashboard/summary` SQL gates; `latestIn7d` + `sparkBuckets` both
required `measured_at >= sevenDaysAgo`, which returned empty for
sparse accounts, leaving the iOS tile with `latestValue: null` +
`sparkline: []` to render from. Detail-screen
(`/api/measurements/series?kind=…`) escaped because of its broader
window.

Server-side changes:
- `latestIn7d` → `latestEver` (drops 7-day filter; DISTINCT ON returns
latest reading EVER per type, bounded at N_metrics rows).
- `sparkBuckets` rewritten via ROW_NUMBER() OVER (PARTITION BY type
ORDER BY bucket_start DESC) so the trailing N daily buckets per
type render regardless of age, bounded at SPARK_DAYS × N_metrics.
- BP + Pulse metrics.push gated on `latest || allTimeCount > 0` like
BodyFat already is — accounts with zero readings ever no longer get
  empty placeholder tiles.
- 4 new regression tests in summary/__tests__/route.test.ts cover
sparse accounts, zero-data accounts, within-7d guards.

Other develop work bundled in this release:
- W4-QoL — copy nits.
- W5-WORKOUTS — user source priority threaded through write-time
  picker + batch route.
- W7-WITHINGS-RELIABILITY — response classifier + sync hardening.
- W8-OPS — ops phase docs.
- B11 — NEXT_PUBLIC_APP_VERSION build-arg routing so /api/version
  cannot drift from CI tag.
- env-check workflow — manifest / example lockstep enforcement.

because GitHub's UI couldn't auto-resolve develop's post-v1.4.42
contiguous work against the v1.4.42 squash on main; resulting tree is

Marc-André BombeckandMarc-André Bombeck d7778c1085 feat(v1.4): multi-tenant prep — off-host backup, encryption-key versioning, worker/web split, native-Bearer UA gating (#135)
* feat(crypto): G2 — encryption-key versioning + rotation CLI

Adds backwards-compatible support for ENCRYPTION_KEYS (JSON map of
{ "id": "<key>" }) plus ENCRYPTION_ACTIVE_KEY_ID. Ciphertext format is now
"<keyId>.<base64payload>"; legacy bare-base64 ciphertext still decrypts
under the synthetic "v1" id derived from the existing ENCRYPTION_KEY.

scripts/rotate-encryption-key.ts re-encrypts every encrypted column on
User / WithingsConnection / AppSettings with the active key. Idempotent —
rows already on the active id are skipped.

Why: enables zero-downtime rotation when a key is suspected leaked,
without forcing every encrypted row through a single big-bang rewrite.

* feat(auth): G4 — native-Bearer UA gating + rotating refresh tokens

Web callers (UA contains 'Mozilla/') keep the legacy 90d Bearer when they
explicitly opt in via X-Client-Type: native. Native callers (HealthLog-iOS,
HealthLog-iPad, n8n, Health-Connect, unrecognised UAs) now receive a 24h
access token plus a 60d rotating refresh token in the login response.

New /api/auth/refresh endpoint atomically validates + rotates the refresh
token (one-time-use), revokes the previously paired access token, and
audit-logs every refresh + revoke. Reuse of an already-consumed refresh
token revokes the entire family for the user/device — defence against a
stolen refresh token.

Migration 0025_refresh_tokens adds the table; the only DDL is CREATE
TABLE so it is forward-compatible. Down migration documented at
docs/ops/migrations/0025-down.sql.

Why: the V3 audit (still-V2-A) flagged that 90d tokens are too long for
native callers when refresh-token rotation is now table-stakes for mobile
apps. Web flows are unchanged so existing browser sessions keep working.

* feat(infra): G3 — web/worker container split via HEALTHLOG_PROCESS_TYPE

Adds a tiny startup gate (src/lib/process-type.ts) so the same Docker image
can run as web-only, worker-only, or 'all' (default — single-container,
backwards compatible). assertSubsystemEnabled() refuses to boot the
reminder worker when HEALTHLOG_PROCESS_TYPE=web, preventing accidental
double-cron when an operator splits the deployment.

docker-compose.yml ships the optional app-worker service block commented
out so 'docker compose up' keeps working out of the box; uncommenting it
+ flipping app to HEALTHLOG_PROCESS_TYPE=web is the only change to scale
HTTP horizontally. Healthchecks are isolated per service so neither
startup deadlocks the other; both wait on db: service_healthy.

docs/self-hosting/scaling.md walks through when + how to split.

* feat(jobs): G1 — daily off-host encrypted backup to S3-compatible storage

New pg-boss queue 'data-backup-offhost' runs at 02:30 Europe/Berlin, dumps
each user's measurements + medications + intake events + mood entries,
encrypts the JSON under a SEPARATE key (BACKUP_ENCRYPTION_KEY, isolated
from the application ENCRYPTION_KEY), and uploads to any S3-compatible
target via @aws-sdk/client-s3.

Wire format documented in docs/ops/backup-restore.md:
  magic "HLBK" || version 0x01 || iv(12) || authTag(16) || ciphertext

The worker prunes objects older than BACKUP_RETENTION_DAYS (default 30);
operators are encouraged to also configure a bucket-level lifecycle rule.

Admin endpoint POST /api/admin/backup/test issues a 1-byte PUT+GET
roundtrip and returns endpoint + bucket + per-call latency. Credentials
are never returned.

scripts/restore-backup.ts downloads + decrypts a single object so
operators can verify the chain end-to-end without standing up a separate
restore harness.

Why: GDPR Art. 32 + V3 audit "data durability" — without an off-host copy
the only line of defence against a Postgres volume loss is the in-DB
DataBackup model on the same host. Separate-key isolation means a leak of
the application encryption key does not also expose every user's archive.

* fix(multi-tenant): address review findings — rotation coverage, hard decrypt errors, web/worker gates

Multi-agent review on PR #135 caught several CRITICAL + HIGH issues that
would have shipped silent data corruption or weakened the threat model:

Encryption + rotation
- scripts/rotate-encryption-key.ts: stop selecting `bugReportTokenEncrypted`
(column doesn't exist in the schema; the script crashed on the AppSettings
step). Add the three encrypted columns it was missing —
  NotificationChannel.config, PushSubscription.p256dh, PushSubscription.auth
  — so rotating to a new key actually re-encrypts every encrypted row.
Without this, dropping the old key id from ENCRYPTION_KEYS would leave
notification channels and web-push subscriptions permanently
  undecryptable.
- src/lib/crypto.ts: refuse to silently fall back to the active key when a
legacy bare-base64 row is decrypted but no v1 entry is configured. Throws
  a clear "Restore ENCRYPTION_KEY (or add a v1 entry to ENCRYPTION_KEYS)
and run scripts/rotate-encryption-key.ts before removing it" so an
operator who pulled the v1 too early notices immediately instead of
  hitting opaque GCM tag errors.
- docs/ops/encryption-key-rotation.md: full step-by-step + rollback note +
troubleshooting section. Spells out that rotation is a forward-only
operation and what to do if the rollback path is needed.

Web / worker split (G3)
- src/proxy.ts: refuse HTTP traffic with a 503 +
  X-HealthLog-Process-Type: worker header when HEALTHLOG_PROCESS_TYPE=worker.
The worker container (which still runs `node server.js` for now) no
longer accepts requests, so a misrouted client gets a clear hint instead
  of a partially-served response.
- src/instrumentation.ts: short-circuit before importing the reminder
worker when HEALTHLOG_PROCESS_TYPE=web — avoids the noisy
"[instrumentation] CRITICAL" stderr line we emitted on every web boot.
- /api/version added to PUBLIC_PATHS so the compose healthcheck doesn't
silently 302-redirect through the auth gate (the prior wget treated the
login page as a 200 success and would never notice an unhealthy backend).

Refresh-token rotation (G4)
- src/lib/auth/refresh-token.ts: reuse-detection now revokes ALL active
refresh tokens for the user, not only those scoped to the same deviceId.
An attacker who rotated without an X-Device-Id header would otherwise
keep their family alive on the no-device branch. Defence-in-depth.
- revokeRefreshToken() also revokes the paired access token so a leaked
access token cannot outlive the refresh-token sibling that the caller
  just killed (logout-on-device).

Off-host backup (G1)
- src/lib/jobs/offhost-backup.ts: drop the worker-side DeleteObject path.
Retention is the bucket's lifecycle-policy job; the worker keeps a
PutObject + GetObject grant only. A compromised worker can no longer
wipe the backup history. Per-user errors are now surfaced as Wide
Event warnings + a structured failures[] array on the report so an
operator can tell which user failed and why.
- scripts/restore-backup.ts: schema-validate the decrypted payload before
writing to disk, plus an optional --user-id=<id> flag that refuses to
proceed unless the dump's userId matches. Prevents silent
cross-tenant restores when an operator points the script at the wrong
  S3 key.

Idempotency
- src/lib/idempotency.ts: the leak-guard already blocked `hlk_`
(access-token prefix); also block `hlr_` so a future caller wrapping
the new /api/auth/refresh endpoint with withIdempotency() can't
accidentally cache a freshly-issued refresh token.

Migration / schema
- prisma/migrations/0025_refresh_tokens/migration.sql: wrap in a single
transaction; switch to CREATE { TABLE | INDEX } IF NOT EXISTS so a
  partial-deploy retry is a no-op instead of "relation already exists";
guard the FK ADD with information_schema lookup for the same reason.
- Schema replaces the single-column `@@index([deviceId])` with a
composite `@@index([userId, deviceId])` (every lookup also filters on
userId; the old index would never have been used). Comment on
  accessTokenHash corrected to HMAC-SHA-256.

Container / ops
- Dockerfile: install tzdata + ENV TZ=Europe/Berlin so the daily
  pg-boss schedule (`30 2 * * *` Europe/Berlin) actually fires at 02:30
Berlin instead of silently falling back to UTC on the alpine base.
- docker-compose.yml: tighten the healthcheck to `wget --spider` (matches
the Dockerfile HEALTHCHECK style, doesn't pipe the body), pass through
BACKUP_RETENTION_DAYS so a custom retention env reaches the container.
- .env.example: document every new env var added in this PR
  (HEALTHLOG_PROCESS_TYPE, ENCRYPTION_KEYS, ENCRYPTION_ACTIVE_KEY_ID,
  BACKUP_S3_*, BACKUP_ENCRYPTION_KEY, BACKUP_RETENTION_DAYS) with
comments and the all-or-none semantics for the backup block.

491 tests still green, typecheck clean.

* chore: retrigger CI
