docs(changelog): drop what the 1.38.18 entry said about its own checks

The entry claimed the setup screens leave room for longer translations
and pass the accessibility check, which describes how the work was
verified rather than anything a reader gets, and it mentioned the demo
instance, which is ours and not theirs. What remains is that the setup
is available in every language the app has.

- **A needs-based setup replaces the wizard.** A few questions (who the
record is for, the areas to watch, medication on a schedule, where
readings come from, an upcoming visit), one about units only when an
area needs it and the account does not hold it yet, then a confirm
screen that shows what the answers switch on, one first task with its
form right there (connect a source, add the first medication with a
reminder, or log one reading), and a done screen with the checklist
open. Every answer is saved as it is given, so leaving and returning
resumes at the same step; every question but the first can be skipped,
and "Skip for now" on the welcome screen leaves the app usable with
nothing switched on. "Set up again" and "Show the checklist again" live
under Settings → Account. The server works out the modules from the
answers once, the way v1.38.16 introduced; a module the person switched
on by hand, or one that already holds data, is never turned off by an
answer.

- **Answers given for someone you look after go to their record.** With
"someone I look after" the questions run in your own record, their
record is created on the confirm screen, and the modules go to that
record while yours stay as they were. Finishing without creating it
changes nothing on your record either. The checklist gets a row for that
record instead of reading those answers as yours. If you write against
the API: `POST /api/onboarding/complete` takes `managedRecordId`; a
record the caller does not manage is a 404, as on the managed-profile
routes.

- Module pages say which switch is off. A page whose module is off shows
an empty state that names the switch instead of a blank page, and the
two tour stops that had nothing to point at (integrations, the health
record) have their anchors.

### Changed
- **The dashboard order is seeded from the answers the first time,** and
only while the layout is unset, so an order the person has already
arranged is never overwritten. The demo instance can finish the setup
now; the disclaimer and tour writes are allowed there.

### Removed
- **`POST /api/onboarding/step`, the goal slugs, and the
`users.onboarding_step` and `users.onboarding_goals` columns**
(migration `0339_retire_wizard_columns`). The iOS app never called the
route or read the columns; `POST /api/onboarding/disclaimer`,

- **Take a backup before deploying, and expect no image rollback.** The
migration drops the two columns the v1.38.16 and v1.38.17 images read on
every sign-in, so an older image against a migrated database fails at
login, not only in the wizard. Rolling the image back means restoring
the database, or re-adding the columns with their defaults, which loses
