test(coach): grade the dialog scenarios through the real pipeline

Ten scenarios, each in German and English, describe what a turn must do
with a question: which tool it calls and with which arguments, whether it
may read anything new, whether it asks back, declines a dose, offers
chips only for what the record holds, answers a chip without a provider,
offers to keep looking after a forced answer, and keeps an earlier
table's title out of the model's context. evaluateScenario grades a run
against them.

The deterministic suite drives POST /api/insights/chat with a scripted
provider; the tool loop, executor, table tool, chips, clarification
filter, method line, reply guards and stream run for real. Every check
also runs against a misbehaving model or a broken record and must fail
there.

route-followups.test.ts pins the front door for chips and answered
questions: validation, the rate limit and capability gate on a reuse
turn, the stored chip winning over anything the request adds, stale
chips and answers degrading to a plain message, and the continue line.

/**
* Shared fixtures for driving `POST /api/insights/chat` through the real
* turn pipeline with a scripted provider.
*
* Unlike the golden harness, the tool loop, the executor, the table tool,
* the chart heuristics, the chips, the clarification filter, the method
* line, the reply guards and the SSE stream all run for real. Only the
* edges are replaced: auth and the front-door gates, the database, the
* provider (a script of rounds), the data reads under the table tool, the
* snapshot and inventory builders, persistence and the observability sinks.
*
* A test file registers the module mocks itself (vi.mock is hoisted per
* file) and points each factory at the objects exported here; `mockModules`
* lists them so the two files that use this harness stay in step.

*
* The script plays a well-behaved model. What the suite proves is the

* answering from the stored table with no new read, the clarification
* filter keeping only metrics the record holds, the reuse chip answering
* without a provider call, the continue chip after a forced answer, the
* method line, the dose screen, and the fence that keeps an earlier

* Every check is also run against a misbehaving model or a broken world

/**
* Coach dialog scenarios: one question, the record it is asked against,
* and what the turn must do with it. Which tool it calls and with which
* arguments, whether it answers from a table the conversation already
* holds, whether it asks back, whether it declines, which chips it offers.
*
* Each scenario is graded twice, by the same `evaluateScenario`:
*
* - the deterministic suite (`__tests__/scenarios.test.ts`, in `pnpm
* test`) drives the real turn pipeline with a scripted provider, so it
* proves the server half: the chips, the clarification filter, the
* model-free reuse turn, the method line, the fence around earlier
* tables, the dose screen;
* - the live run (`run-case.ts` → `runScenarioLive`, nightly, only with
* `COACH_EVAL_API_KEY`) puts the same question to a real model and
* grades its tool calls, so it measures the model half.
*
* The shape follows a smoke-question suite: a prompt, the tool expected,
* the value the call must carry, whether it may read anything new, and
* the context an earlier turn left behind. Every scenario runs in German
* and in English.
*
* Pure: no database, no provider.
