fix(ai): scrub fence markers until none can re-form
The shared data/instruction fence removed embedded markers in a single pass. Removing a marker joins the text either side of it, so a marker split around another one came back whole after the scrub: `<<<DOCUMENT_TEXT_E<<<DOCUMENT_TEXT_END>>>ND>>>` became `<<<DOCUMENT_TEXT_END>>>`, and the same split around any other marker worked too. Content could close its own fence wherever the helper is used: the Coach snapshot and self-report, MCP tool results, document reads and lab scans. The scrub now repeats until a pass changes nothing, which proves no marker is left. Content nested deeper than eight levels has every run of three or more angle brackets cut to two, after which no marker can exist or re-form. Ordinary text, angle brackets included, is unchanged.
