Name the refusal the last release notes left out

The fix that stopped a delegate overturning a recorded dose through the second
intake route landed on the trunk with no line in the notes. A change that
alters what somebody else may do to your record and says nothing about it is
the shape this project keeps rediscovering, and a release assembled from these
notes would have shipped it silently.

### Fixed
- Somebody with write access could still overturn a dose you had already
recorded. The rule that keeps that with the owner was on one request and the
medication card reaches the same entry through another, where an explicit
"skipped" was applied rather than refused, and the stock the dose had used
was credited back on the way. Both now refuse a decision that disagrees with
the one already there. Re-sending the same decision still works, because a
second tap, a phone catching up after no signal and a retry after a partial
sweep all look like that, and somebody standing over a pill box hits all
three.
- A control could be tapped in the moment before the app knew whose record was
on screen, leaving somebody stranded in a form the server would refuse. The
capture sheet and the dashboard's quick-entry sheets now withdraw the form
