fix(documents): fold the inbound-document disclaimer into the page subtitle

Remove the standalone amber disclaimer card from the inbound-documents
review screen. The non-diagnostic essence now reads as the plain muted
subtitle line under the page title: upload a clinical document and review
the extracted details. Drop the now-unused disclaimer.title /
disclaimer.body keys across all six locales and the AlertTriangle /
CardDescription imports left orphaned by the removal.
