feat(coach): give every turn its memory and let the Coach remember and propose plans

What a person told the Coach never reached a tool-mode turn: facts and plans
rode the snapshot's memory block, which tool mode does not send, and the
extraction ran only past twenty turns and failed to parse on every
non-Codex chain, because the status chain runs in JSON-object mode while the
prompt asked for a bare array.

- Memory block: WHAT YOU KNOW ABOUT THIS PERSON, built per turn behind the
  wire egress check (nothing is read on a refusal), fenced as data, at most
  1,500 characters, never trimmed before the inventory. Confirmed facts
  first, then the newest; active plans with a server-computed progress line;
  reminders the person asked for. Medication facts stay out while the
  medications module is off. Building it stamps last_used_at without moving
  updated_at. buildCoachToolRequest and buildCoachProviderPrompts take it as
  an optional memoryBlock; the snapshot memory keeps only the narrative and
  trend recall.
- remember_fact: preferences, goals and context are saved at once
  (source coach, source_message_id set); anything that names a condition,
  limitation or medication, by category or by wording, is only proposed.
  A fact must come from the person's current message; one note per answer.
- The person's answer reads the proposed fact from the encrypted trail on
  the assistant message, never from the request.
- propose_plan writes a proposed plan; the tap activates it with its review
  window counted from the tap. One per answer, three open at most.
- Progress: one sentence per active plan from the person's own day series
  and units, also offered to the briefing (two lines at most, behind the
  briefing capability). The plan review and the progress line read a step
  plan as daily totals instead of per-sample means.
- Background extraction asks for {"facts":[]} / {"plans":[]} and reads
  either shape, runs once a conversation has been quiet for 30 minutes, and
  stores health facts as proposals offered once at the next turn.
- Unanswered plan and fact proposals lapse after 14 days in the reminder
  sweep.
- Memory list API: POST saves one of the caller's own messages (the
  remember button), PATCH edits a fact's wording, GET reports source, origin
  and last use and never lists a waiting proposal.
- A structural guard keeps the egress check ahead of every read and the
  callers of both builders to the turn and the briefing.

* The memory block (`WHAT YOU KNOW ABOUT THIS PERSON`) carries facts, plans
* and reminders the person wrote; the briefing's plan lines carry plan
* targets. Both are built, not built-then-dropped: the egress or capability
* check runs first and nothing is read on a refusal. Behaviour is pinned in
* `memory/__tests__/context-block.test.ts` and `plan-progress.test.ts`; this
* guard pins the structure those tests rest on:
*
* 1. Inside each builder, the check comes before the first database read.
* 2. The builders are called only from the files that run after the turn's
* own egress check (the Coach turn) or behind the briefing capability (the
* briefing generation), and a Coach-turn caller hands over the chain so
* the wire check runs for exactly those providers.
* 3. No `annotate()` in the memory modules names a text-bearing value.

* Every check is also run against a source broken on purpose.

ONLY extract facts that are STABLE and will still be true next month: standing preferences, conditions the user STATES about themselves, durable goals, standing constraints, durable life context. Record descriptively in the user's own framing — NEVER diagnose, infer a medical condition the user did not state, or record a number/measurement (those live in the data). EXCLUDE: transient feelings ("tired today"), one-off events, anything time-bound, sensitive detail beyond what is needed to coach, and anything the user asked you to forget. A medication the user says they take is "medication"; a diagnosis or health condition they state is "condition"; a standing limitation (an injury, a diet they must keep) is "constraint". If nothing durable was said, return { "facts": [] }. Prefer FEW high-confidence facts over many speculative ones.`;

/**
* v1.41 — `WHAT YOU KNOW ABOUT THIS PERSON`: the memory every Coach turn
* starts with.
*
* Until v1.41 the Coach's facts and plans rode the SNAPSHOT's `memory` block,
* and a tool-mode turn (every provider that can call tools) sends no
* snapshot: what the person had told the Coach never reached it. They also
* went first when the snapshot was trimmed. This block goes into the first
* user message of every turn, tool mode and the no-tools path alike, ahead of
* the data inventory and never trimmed for it:
*
* - facts, at most {@link FACTS_INJECT_TOP_N}: confirmed by the person first
* (`user`), then saved by the Coach (`coach`), then the background's; the
* most recently changed first; then confidence. A fact waiting for the
* person's answer (`proposed`) is never in the block;
* - active plans, at most {@link PLANS_INJECT_TOP_N}, each with its
* server-computed progress sentence (`plan-progress.ts`);
* - near or overdue reminders the person asked for.
*
* Everything the person or the background wrote sits inside the self-report
* fence: data, never an instruction. The block is at most
* {@link MEMORY_BLOCK_MAX_CHARS} long; reminders, then plans, then facts are
* dropped from the end until it fits.
*
* Egress: the block is built only when the turn may send to its providers.
* With the chain in hand the wire check (`aiEgressRefusal`) runs for exactly
* those providers; without it, the record's `coach` capability. On a refusal
* nothing is read: the block is not built rather than built and dropped.
* Facts filed under `medication` stay out while the medications module is off
* for the record.
*
* Building the block stamps `last_used_at` on the facts it carries, without
* touching `updated_at` (an edit, not a use, moves a fact up the ranking).
*
* Server-only. Fact and plan text never reach `annotate()`.

* v1.41 — what the Coach remembers from a conversation, and how the person

*
* Three ways a fact gets into `coach_facts`:
*
* 1. `remember_fact`, the tool the Coach calls in a turn. A preference, a

* note with "Undo". Anything that touches health (filed as `condition`,
* `constraint` or `medication`, or worded like one, `lexicon.ts`) is only
* proposed: nothing is written until the person taps "Yes, remember it".
* A fact must come from the person's current message: at least half of its
* words, and every figure in it, must appear there. That keeps a quote, a
* document or the Coach's own idea out of memory. One note per answer.
* 2. The person's answer to a proposal (`memoryDecision` on the chat
* request). The fact is read from what the server stored on the assistant
