fix(fhir): code allergy and injury conditions with active SNOMED CT concepts

Allergy used 106190000, inactive since 2013; it is now 473011001 Allergic
condition. Injury keeps 417163006 with the display SNOMED CT publishes,
Traumatic or non-traumatic injury; the old display was rejected.
