Steffen Becker d407febd4b feat: insert a chapter field, and set where a cell's content sits ...

Both round-tripped through save/load already but had no way in: the
header/footer bar gains a Chapter button next to page number/count, and the
table toolbar three buttons for the cell's vertical alignment.
