Marc-André Bombeck 46f9dfa47f fix(integrations): make the new cards match the existing ones and link to their data ...

Give the Polar, Oura and Nightscout cards the same parked-state resume banner
and test-connection button the other integrations carry, distinct brand icons,
a connected-state link to where their data appears, and the correct status-key
invalidation so the pill refreshes after a disconnect. Fix the sleep empty-state
link that pointed at a non-existent settings slug.
