Rebuild the /coach surface as a centred-hero chat with a word-by-word fade, a
single thinking disclosure, and a per-message token footer; emit token usage on
the done frame and persist it via migration 0182; plumb an additive reasoning
frame with an elapsed-time fallback for non-reasoning providers.
