Marc-André Bombeck db4d5928fd docs(ops): name what the v1.46 migrations drop and what a rollback to v1.45 does ...

The notes said the other migrations only add columns and one concurrent
index. 0487 drops four app_settings columns the previous image reads on
nearly every request, and 0488 builds its index on users without
CONCURRENTLY. The rollback step now names the columns to hand back, and the
notes say what v1.45 does with accounts pending deletion and held wipe
purges, and how to settle them before going back.
