Marc-André Bombeck e9206bab0b fix(compose): forward the env vars the app reads but never received
The environment: block is a whitelist. Compose substitutes ${VAR} from .env
at compose-up, so a value set there looks configured, but a variable the block
does not name never reaches the Node process and the default stands silently.

Twenty-eight variables were in that state: DEMO_MODE, the four LOG_* knobs and
the three LOKI_* shipping vars, DATABASE_STATEMENT_TIMEOUT_MS, four retention
and alerting windows, TELEGRAM_WEBHOOK_SECRET, NIGHTSCOUT_PRIVATE_ORIGINS,
three OAuth callback overrides, both Open-Meteo endpoints, four AI knobs,
three APNs key-source vars, and DASHBOARD_SSR_PREFETCH.

APNS_KEY and APNS_KEY_FILE are the sharpest of them: .env.production.example
and the env manifest both document a precedence chain of APNS_KEY_B64 before
APNS_KEY before APNS_KEY_FILE, and only the first arrived.

Each is documented in .env.production.example in its neighbours' style. The
VAPID section's note claiming those three names are absent from the whitelist
is corrected; they have been on it since v1.17.1.

Marc-André Bombeck 0037cc1bcd Write restored PHI 0o600 and let TRUST_PROXY_HOPS reach the container
Two findings from the docs truth audit, both operator-facing.

scripts/restore-backup.ts wrote the decrypted backup, which is health
data in plaintext, with the process default file mode, leaving it
world-readable on the operator's disk while it sat there. It is now
written 0o600.

TRUST_PROXY_HOPS is read from the environment to size the trusted
X-Forwarded-For chain, but it was absent from the compose environment
whitelist, so an operator value in .env never reached the container and
was silently ignored. It now rides all four surfaces: the compose
whitelist, both env examples, and the manifest that pnpm check-env
reads.

Marc-André Bombeck 8e16df6894 feat(documents): make the document-AI rate limit an operator setting with honest accounting
The document-AI actions (read/index, extract, suggest, summarise) shared a
hard-coded 6/hour bucket that lied twice: the 429 said "try again in a few
minutes" while the window is an hour, and several failure paths burned a
slot before any provider was dispatched.

- DOCUMENT_AI_LIMIT_PER_HOUR configures the shared per-user ceiling
(default 6, clamped 1-1000), wired end to end: compose environment
whitelist, .env.production.example, env manifest, operator docs.
- INSIGHTS_RATE_LIMIT_PER_HOUR joins the compose whitelist and manifest;
it was documented but never listed, so an operator value could not reach
  the container.
- The 429 mirrors the bucket's reset instant into meta.retryAt (plus the
existing X-RateLimit-Reset header); the client derives "try again in
about N minutes" from it in all six locales, with a plural-aware key.
- A slot is consumed only when a request actually reaches the provider (or
the local index/echo work): malformed bodies, unreadable originals,
refused consent, and exhausted daily budgets refund the charged slot via
a window-scoped refundRateLimit primitive. Consistent across all four
  routes sharing the bucket.

Marc-André Bombeck 869d91c4c2 Fetch the GeoLite2 databases at runtime when a licence key is set
The offline geo tier only reached a self-host two ways: baked into the
image at build time, or mounted from the host. Both need something the
operator of a stock `:latest` pull does not have. Add a third: set
MAXMIND_LICENSE_KEY and the worker downloads GeoLite2-City + GeoLite2-ASN
itself into GEOLITE2_DIR, so the offline tier starts working with no
rebuild and no manual mount.

A worker-only pg-boss queue drives it. Boot discovery fetches once when a
key is set and no database is present yet; a monthly cron (past MaxMind's
first-Tuesday reissue) keeps a keyed host current. The fetch is fail-soft:
each edition extracts to a temp file in the target directory and is
promoted with an atomic rename only after both are in hand, so a bad key
or an upstream error leaves the previous databases exactly as they were
and never writes a partial `.mmdb`. After a successful place the reader
cache resets so a running process picks up the files without a restart.

Extraction is pure Node (zlib + a minimal tar reader), not a shell-out:
this module is reachable from the worker-boot import chain, and a
child_process import there makes Next's file tracer pull the whole project
into the standalone output. zlib keeps the trace bounded and drops the
busybox runtime dependency. The download goes through safeFetch with the
public-host pin and redirect-following, since the MaxMind permalink 302s
to a signed CDN.

The runtime image now owns /opt/geolite2 as the unprivileged app user so
the worker can write it; a read-only bring-your-own mount at that path
still wins, because the fetch cannot write over it and fails soft.

Marc-André Bombeck b1667ef692 fix: let the audit window an operator sets reach the container
AUDIT_LOG_RETENTION_DAYS is not only enforced, it is published: the
shared-access panel prints the window under the activity list, and the dialog
that ends somebody else's access states how long "who entered what" stays
answerable. Both read the value the server resolved. It was missing from the
compose environment: whitelist and from both example files, so an operator who
set 90 got 365 told to every user, in a sentence about their own data.

That block is a whitelist and Compose substitutes from .env regardless, which
is what makes the variable look configured while never arriving — the same
shape as the session-cookie flag in v1.5.2. A guard now covers the three places
an operator meets such a variable, and its control leg found that the cookie
flag itself is still absent from the production example. Documented there too.

Marc-André Bombeck 8d86416072 fix(compose): stop the worker example drifting, and bound logs and memory
The `app` service declares 62 environment variables. The commented-out
`app-worker` example declared 14, missing every SMTP, VAPID and APNs value plus
the AI-host allowlist and the offline geo directory. The worker is the process
that sends notifications and runs the AI jobs, so anyone following the documented
split got a worker that could not email, push or send APNs, and nothing failed
loudly. It just did nothing.

Hoists the shared block to a YAML anchor both services merge, so copy-paste
cannot drift again. The resolved configuration is unchanged: 62 variables on
`app` and 3 on `db` before and after.

Also:

- The commented GeoLite2 mount pointed at a directory the default GEOLITE2_DIR
does not read. Uncommenting the example as written did nothing. The two now
  agree.
- Bounded log rotation on both services. The default json-file driver grows
without limit and fills the disk on exactly the small hosts this file targets.
- `cap_drop: [ALL]` on the app, and a written reason why the database does not
get it: the Postgres entrypoint starts as root and needs CHOWN/SETUID/SETGID
to hand off to its own user. Dropping capabilities there breaks startup, which
  was confirmed rather than assumed.
- A memory limit on the app, and a written reason why the database has none. A
bounded app container restarts clean when the kernel takes it; a database
  killed mid-write does not.

`read_only: true` stays off for now: the app writes a runtime cache and that
needs testing before it can be turned on.

Marc-André Bombeck 189024cc00 fix(geo): make GEOLITE2_DIR reachable and tell self-hosters what they can do
The runtime has always read GEOLITE2_DIR, but the variable was absent
from the compose `environment:` whitelist, from both env examples, and
from the docs. A variable that is not on that whitelist never reaches
the container even when the operator's `.env` sets it, so the override
existed in code and was unreachable from the documented path.

Put it on the whitelist, document it in both examples and the manifest,
and add `docs/self-hosting/geolite2.md`: fetch the databases with a free
MaxMind account, mount the directory read-only, point the variable at
it. The compose file carries the bind-mount line commented out beside
the app service.

The admin notification told the reader to set MAXMIND_LICENSE_KEY in
this repository's Actions secrets — something nobody running the
published image can do, about a fallback that sends their sign-in
addresses to a third party. It now names the mount, names the online
provider it is actually querying, and links the runbook. Six locales.
It also no longer claims the carrier column stays empty; the online
tier has filled the carrier from the ISP field since v1.25.8.

A blank GEOLITE2_DIR now resolves to the default instead of the process
working directory: compose substitutes an unset variable to the empty
string, which is not nullish.

Balaji Vijayakumar f815e4a83f feat(auth): add OIDC SSO login
Adds an OpenID Connect relying-party login path (PKCE authorization-code
flow) against a single env-configured identity provider — additive to
password/passkey login by default, with an operator opt-in (OIDC_ONLY,
enforced server-side on every auth route) to disable them entirely.
First-time sign-in auto-provisions or links an account by verified email.

Closes an open redirect in the post-login `next` parameter (both the new
OIDC path and the pre-existing password/passkey flow shared the same
backslash-normalization bypass) by resolving the target through a real
URL parse and comparing origins instead of a prefix check.

Marc-André Bombeck a4feabec81 chore(release): v1.28.30 — the daily briefing stops silently skipping days
The recurring no-briefing day had a chain of causes, closed end to end:

Every failure path in the nightly pregenerate + the comprehensive generator
now annotates a queryable cause (several classes — features-error,
payload-too-large, aborted, generator-failed — previously bumped a counter
with zero trace). A failed nightly warm enqueues one bounded retry ~45
minutes later (singleton per user, existing budgets apply), so a transient
provider hiccup heals before morning. A cached payload that carries NO
briefing no longer satisfies the 24h POST short-circuit nor the
unchanged-content hash gate — a once-stripped briefing could previously
block its own regeneration indefinitely. The card surfaces the omission
reason persistently (grounding strip, generation failure, not attempted)
with a forcing retry.

Documentation corrected against code truth: Withings credentials are
per-user (the documented env path never existed; the dead example entries
and manifest rows are gone, and WITHINGS_WEBHOOK_SECRET is now actually
whitelisted in compose — stock deploys could never deliver it); the classic
Fitbit portal no longer accepts new registrations; the backup-restore
example no longer mints a fresh encryption key and uses pnpm dlx tsx; the
TLS guide describes the real client model (two CA-level SPKI pins,
any-match, leaf renewals need no build); scaling/admin references fixed;
the operator-shared subscription provider is documented.

Marc-André Bombeck 13464b512e chore(release): v1.28.28 — OpenAI-compatible gateways work; a re-keyed night can no longer vanish
The Local provider speaks the standard JSON wire: response_format by default,
one self-healing fallback per endpoint when a server rejects it (dialect
cached per base URL) — LiteLLM, OpenRouter, vLLM, LM Studio and plain Ollama
all work from the same form, and the form + provider docs now say so. A
gateway that shims a Claude-family model behind a synthesized tool call is
parsed from the tool-call arguments instead of yielding silently empty
insights.

The insight token budget is configurable (INSIGHTS_MAX_TOKENS, default 2500,
clamped 500-8000) and unified across all six call sites; a reply cut off
mid-JSON returns a distinct truncation error instead of a generic parse
failure. A briefing withheld by the number-grounding check now says so on the
card instead of pretending nothing was generated. The provider test button
answers 4xx with a bad_request category instead of claiming the endpoint was
unreachable.

Sleep repair follow-up: measurements carries a second full unique index — the
natural key (type, measured_at, source, sleep_stage) — so a key-format
migration could tombstone the old rows while their re-keyed replacements
collided with that index and were silently dropped (live incident: a
full-history re-sync erased a user's sleep). Planned creates now run a
natural-key rescue probe: a match, live or tombstoned, is updated in place —
fresh value, the new externalId, deletedAt cleared. A rescue-probe failure
holds the watermark so nothing is lost past the overlap.

Marc-André Bombeck 45da6c3c7d chore(release): v1.28.27 — runs on CPUs without AVX2
The bundled Skia x64 build (document thumbnails, scanned-PDF rasterization)
uses AVX2 instructions; on an x86-64 host without them — Celeron/Atom-class
NAS CPUs, pre-2013 Xeons, VMs masking CPU flags — the first canvas call is an
uncatchable SIGILL that kills the process and crash-loops the container (live
incident: a Synology DS1520+, exit 132 ~18 s after every boot, reported with
kernel traps and the correct root cause in hand).

The renderer is now gated on a one-time CPU-feature check before the module
ever loads: linux/x64 requires the avx2 cpuinfo flag; arm64 and non-Linux are
unaffected; unreadable cpuinfo fails open. Behind a closed gate, thumbnail
generation and PDF rasterization take their existing fail-soft paths (vault
shows type icons; PDFs read as text) and the boot-time thumbnail backfill
skips enqueueing. NATIVE_CANVAS=off|on overrides detection, documented in the
env example and whitelisted in compose.

Marc-André Bombeck 75eb0637e6 feat(strava): add per-user BYO-OAuth workout source + Garmin indirect-path docs
Add Strava as a per-user BYO-OAuth workout source, mirroring the shipped
WHOOP/Oura/Polar pattern. Strava caps every new app at single-player mode, so
each self-hoster registers their own app and pastes the client id/secret into
Settings (falling back to a shared env app). Strava is a workout source only —
it exposes no daily metrics and no training-depth analytics are derived.

- Schema (migration 0233): per-user encrypted client id/secret + access/refresh
tokens, plaintext athlete id, incremental cursor, and backfill marker on
  `User`; `STRAVA` added to `MeasurementSource`.
- Token model follows Oura's reactive-refresh-on-401 with a compare-and-set on
the stored refresh ciphertext, because Strava rotates its refresh token on
  every refresh.
- Client over safeFetch: authorization-code + refresh, activity list + budget-
aware per-activity detail (for calories/HR), honouring the 200/15min cap; each
activity maps to one `Workout` row. Activity name/description stay in metadata
as data, never surfaced or interpolated.
- Dedup reuses the existing source-agnostic two-layer picker: `STRAVA` slots
into the workout ladder below the device-native wearables and above
MANUAL/IMPORT, so an Apple-Watch + Strava twin collapses to one canonical row.
- Poll-cohort hourly sync (:17) + self-converging boot backfill, wired into the
  integration-sync registrar. Webhook deferred.
- OAuth routes (connect/callback/credentials/disconnect/status/test) with signed
  state, cross-account re-auth rejection, and a surgical `www.strava.com`
connect-src gated to the Strava surfaces.
- Settings card + consolidated status envelope entry; env + compose whitelist;
  i18n across all six locales.
- Garmin: no direct connector is possible (business-partner-only). Add
docs/integrations/garmin.md documenting the Apple Health / Google Health
Connect path and Garmin's withheld proprietary scores, cross-linked from both
platform docs, plus a non-connector info note in the Connections panel.

Regenerate docs/api/openapi.yaml for the new `STRAVA` MeasurementSource value.

Marc-André Bombeck e51c6dc2ad feat(geo): allow an opt-in plain-HTTP location provider
A self-hoster can set IP_GEO_ALLOW_INSECURE=true alongside an http:// URL to
use a free HTTP-only provider — the free ip-api.com endpoint is HTTP-only (its
HTTPS form needs a paid key) but its geolocation is often more accurate and it
carries the ISP. The default stays HTTPS-only with ipwho.is; plain HTTP is
refused unless the operator explicitly opts in, since the looked-up IP then
travels unencrypted over the server's own egress. Whitelisted in compose and
documented in the env example.

Marc-André Bombeck 8d73d9721f fix(geo): default to ip-api.com and surface non-ok lookup failures
The ipwho.is default rejected server-side egress with a 403 on the free
plan, which the resolver swallowed as a clean miss, so the admin
login/audit overview rendered "—" for every session. Switch the default
online provider to ip-api.com (the parser already accepts its
status/countryCode shape) and surface a non-ok HTTP status (403/429/5xx)
as a wide-event warning instead of returning null silently, so a future
provider rejection is visible.

Whitelist TRUST_CF_CONNECTING_IP, IP_GEO_LOOKUP_URL and
IP_GEO_LOOKUP_DISABLED in the compose environment block and document them
in .env.production.example so a Cloudflare-fronted deployment can store
the real visitor IP rather than the edge address.

Marc-André Bombeck 88139fb981 feat(integrations): add Oura Cloud v2 OAuth sync
Connect an Oura account over the Cloud v2 API: an authorization-code OAuth
flow (shared app from OURA_CLIENT_ID / OURA_CLIENT_SECRET) on the same signed
state machinery as Polar. Unlike Polar, Oura issues refresh tokens; the sync
refreshes reactively on a 401, rotating and re-encrypting both tokens, and a
403 stays a hard reauth rather than a refresh.

An hourly poll-cohort job maps daily readiness to RECOVERY_SCORE, sleep to
per-stage durations plus efficiency / HRV / resting heart rate / respiratory
rate, and daily activity to steps and active energy — all tagged source OURA,
idempotent on (userId, type, source, externalId). Readiness flows through the
canonical recovery ladder above the computed proxy. Settings gains a connect /
disconnect card. Tokens are never logged or returned; every egress goes
through safeFetch.

Marc-André Bombeck 31abd307fd feat(integrations): add Polar AccessLink OAuth sync
Connect a Polar account over AccessLink v3: an authorization-code OAuth
flow (shared app from POLAR_CLIENT_ID / POLAR_CLIENT_SECRET) backed by a
stateless HMAC-signed state token doubling as an httpOnly cookie for CSRF.
The callback verifies the signature, expiry, provider binding, and the
byte-exact cookie match, then encrypts the access token plus member id onto
the user row.

An hourly poll-cohort job maps Nightly Recharge recovery, sleep stages,
and daily activity to canonical measurements tagged source POLAR, idempotent
on (userId, type, source, externalId). The recovery band feeds RECOVERY_SCORE
through the canonical ladder, which now ranks native readiness above the
computed proxy. Settings gains a connect / disconnect card. Tokens are never
logged or returned; every egress goes through safeFetch.

Marc-André Bombeck f13aa684f9 fix(api, ops): transactional adopt, admin gate without a flash, and a digest path to deploys
The self-context adopt runs read-modify-write inside one transaction,
the admin shell renders nothing until the role is confirmed, and the
json-body helper merges Headers instances correctly. The compose image
reference accepts a pinned digest through an env var, and a promote
script resolves the published digest, verifies its signature and hands
the operator the exact pinning call — the documented path away from a
moving latest tag.

decrypts and parses the newest offhost backup so a broken key or format
surfaces before an emergency does.

Marc-André Bombeck d7bf0df10b chore(ci): pin the supply chain and surface build provenance
Every workflow action is pinned to a commit SHA, dependabot auto-merge
is limited to patch and minor updates behind the now-required status

warning. The published image finally carries its build SHA and build
timestamp so the version endpoint can prove which commit is running.
The API-token HMAC key fails closed under 32 characters, compose pins
the database minor and denies privilege escalation, and the changelog
drops working-note references in favour of neutral wording.

Marc-André Bombeck 76012bf8e2 chore: scrub instance details and personal data from committed artefacts
Blood-pressure fixtures in the in-target integration suite were copied
from live readings; they are now synthetic with the same in/out split so

(DEPLOY_LOGS_URL, HEALTHLOG_PROD_URL), the codex client identifies via
the public repository URL, the OpenAPI servers entry points at a neutral
example host, and remaining workflow, docs and comment references to the
operator's instance or working notes are generalised. The compose
whitelist also gains DEPLOY_WEBHOOK_SECRET, which was missing and made
the deploy webhook unusable on compose installs.

Marc-André Bombeck 540abebf09 feat(ops): alarm on a TLS leaf SPKI change for pinned native clients
The native client SPKI-pins the server's TLS leaf certificate. When the
leaf auto-renews (GTS re-issues it roughly every 90 days) the leaf keypair
— and so the SPKI pin — changes, and a client shipped only the old pin
refuses to connect on the next renewal: a silent outage. Nothing
server-side signalled that a client pins the served leaf.

Add a pg-boss monitor that probes the served leaf every 6 hours, computes
its iOS SPKI pin (base64(sha256(DER subjectPublicKeyInfo)) via a raw TLS
socket + X509Certificate), and compares it against the operator's
known-good set in TLS_LEAF_SPKI_PINS (comma-separated for the dual-pin
renewal window). On a served pin outside the set it fails loud: a
tls.pin.leaf_changed wide-event annotation, a system.tls.pin_changed audit
row, and a high-priority SYSTEM_ALERT fanned out to admins through the
existing dispatcher idiom. Transient TLS/network failures annotate
tls.pin.probe_failed and never alarm.

The baseline is an env var, not a persisted last-seen row — a self-learning
baseline would silently adopt the first rotation and suppress the alarm the
pinned client needs; an unset baseline logs the served pin and warns rather
than auto-adopting. The runbook documents extraction, the re-pin + dual-pin
procedure (≥11 days before expiry, ship to TestFlight), and the assumption
that the probed host is the host the client connects to.

Wires the queue into the worker (allQueues + cron + worker), adds the
optional env to the manifest, compose whitelist, and .env example, and
covers the SPKI computation + change detection with a fixture-cert unit
test plus a queue-registration guard.

Marc-André Bombeck dd1071158f feat(whoop): OAuth connect/callback + HMAC-signed webhook + CSP gating
Connect mints the state nonce into the WhoopOAuthState ledger and redirects
to the WHOOP authorize page; callback verifies the nonce (atomic
delete-first consume, four distinguished reason tags), exchanges the code,
fetches the WHOOP profile for the user id, persists the encrypted
WhoopConnection, and enqueues the self-converging history backfill.

The webhook handler rate-limits per source first, verifies the
WHOOP_WEBHOOK_SECRET path segment, then verifies the HMAC-SHA256 body
signature over the raw bytes (timestamp + body, timingSafeEqual, stale
timestamp reject) before any work. Valid *.updated enqueues the matching
per-resource sync; *.deleted soft-deletes (workouts hard-delete, no
soft-delete column). Unknown users return 200 to stop retries.

CSP gates api.prod.whoop.com to the WHOOP settings surface + /api/whoop/*;
the webhook path joins the public-path allowlist and the path-secret redact
registry. Env wiring adds WHOOP_WEBHOOK_SECRET + WHOOP_REDIRECT_URI to the
compose whitelist and .env.production.example.

Marc-André Bombeck 51fc48f93c feat(fhir): raise the MedicationAdministration cap to 5000 and make it configurable
Raise the per-export administration ceiling from 1000 to 5000 (~3.5 years

often. Add the var to the compose environment whitelist and the production

Marc-André BombeckandClaude Opus 4.8 5f8e191e2b fix(compose): set an explicit, tunable database connection_limit
The app pins DATABASE_URL in the compose environment block, which shadows
any connection_limit an operator sets in .env, so Prisma fell back to its
default pool (cpu_count*2+1 — 9 on a 4-core host). A combined web+worker
process starves that pool under background AI and rollup load, leaving
HTTP requests waiting on the pool until they time out. Set connection_limit
explicitly (default 20, override via DB_CONNECTION_LIMIT) plus a pool_timeout
override, so the pool can be sized to the host and its database.

docker-compose.yml's environment whitelist didn't forward the variable
into the container. Setting it in .env was a silent no-op for the
running Node process. One-line addition closes the gap; default
behaviour unchanged.

Marc-André Bombeck f5962497d6 v1.4.34.4 — audit-driven hotfix bundle (security, UX, code-quality, docs)
Consolidated hotfix landing the findings from a six-axis audit
covering mobile security, UX + accessibility, code quality + docs,
README + discoverability, web performance, and mobile-deep responsive
behaviour. None of the changes break the public API or the existing
data shape; every fix is additive and SAFE under the v1.4.34.x web
freeze. Full audit reports live in `.planning/round-audit-*.md`.

Security
  - Password rotation now wipes API tokens + refresh tokens in the
same `$transaction` as session revocation. A stolen iOS Bearer no
longer survives a user-initiated password change or an admin reset.
  - `/.well-known/` proxy bypass tightened to an explicit-allowlist
    `Set`. Stale `/api/auth/codex/callback` PUBLIC_PATHS entry dropped.
  - HSTS gains `preload`. Withings host removed from the global CSP
and gated to the integration's own routes.
  - `getClientIp` warns once per process on `TRUST_PROXY_HOPS`/XFF
mismatch + exposes a tagged helper for tighter universal buckets.
  - Service worker rejects off-origin notification-click URLs. Offline
fallback body switched to a locale-neutral icon. Manifest gains
    `scope`, `id`, `display_override`.

UX + accessibility
  - i18n leaks closed on Dialog/Sheet close-X, ChartSkeleton's loading
    announcement, and the notifications-section breadcrumb.
  - Focus rings on bottom-nav, the More overflow button, and the
    mobile top-bar user menu.
  - Tap-target floor: Input / NativeSelect / Select / DateTimeInput
    switch to `h-11 sm:h-10`.
  - Admin chip-strip auto-scroll-into-view (matches v1.4.33 settings).
  - List-page subtitles visible on mobile.
  - Insights empty-state CTA vocabulary normalised so the link opens
the right measurement form (was silently failing for
    BODY_TEMPERATURE / BLOOD_GLUCOSE / PULSE).

Mobile-deep
  - `viewport.themeColor` reacts to light/dark mode.
  - TopBar + Coach FAB reserve safe-area-inset-top / -bottom for
    notched iPhones.
  - Sonner toaster switches to `theme="system"`.

Code quality
  - `SESSION_SECRET` dropped — the env var was documented as required
but never read by code. Three secrets, not four. Self-hosters
upgrading can delete the existing line.
  - `toJson<T>()` helper consolidates seven Prisma JSON-cast sites.
  - `db-compat.ts` gains a header docstring naming it the
    schema-bootstrap path.
  - Passkey response types adopted from `@simplewebauthn/server`;
    `any` + eslint-disable pragmas drop out.
  - Zero-TODO CI gate added at `.github/workflows/no-todo-markers.yml`.

Documentation + discoverability
  - README rework: How-it-compares matrix, Apple Health + AI Coach
promoted to standalone Key-Features bullets, status block, latest
release + GHCR badges, OpenAPI pointer, three-secret Quick Start.
  - openGraph + twitter cards now carry `images` and a
    `summary_large_image` Twitter card.
  - GitHub repo metadata: description tightened from 290 to 156
    chars + keyword-front, `homepageUrl` flipped from a personal
tenant to the demo site, six generic / PII-adjacent topics
    swapped for six high-intent ones.
  - Five new user-facing docs: `docs/self-hosting/getting-started.md`,
    `docs/self-hosting/reverse-proxy.md`,
    `docs/integrations/apple-health.md`,
    `docs/integrations/withings.md`,
    `docs/integrations/ai-providers.md`.
  - `docs/README.md` clarifies the docs tree as internal playbooks
    vs the user-facing `docs.healthlog.dev` site.

Marc-André Bombeck 1f57dc3808 v1.4.34.2 — pull_policy: always on docker-compose so Coolify deploys actually pull
The v1.4.34.1 image published to GHCR successfully but never reached
production because Coolify's `docker compose up` kept reusing the
cached `:latest` digest on the apps-01 host. The default Docker pull
policy ("only pull if missing") skipped the registry round-trip and
restarted the container with the prior v1.4.34 image — every
force-rebuild finished in ~20 s without ever fetching the new
manifest.

Declare `pull_policy: always` on the `app` service so compose-up
re-checks the registry digest on every `up` regardless of the local
cache. Bundle-release because the v1.4.34.1 work ships through the
same `:latest` tag — this is the deploy mechanic that makes v1.4.34.1
actually land. From v1.4.34.2 onwards, every release picks up its
own image without a manual host-side retag fallback.

Marc-André Bombeck cfbd822862 deploy: switch docker-compose back to :latest
Pinning to :1.4.2 was a one-shot safety measure today after the
:1.4.1 GHCR tag never published and we needed Coolify to deploy a
known image. Now that :1.4.2 is verifiably live and the healthcheck
fix is in the image, switch back to :latest so future GHCR builds
auto-roll to production without a docker-compose edit.

Self-hosters who prefer pinning can override via
docker-compose.override.yml — the upstream compose default stays
on :latest as before.

Marc-André Bombeck 7cb9221339 deploy: pin docker-compose to :1.4.2 (production rollout)
The v1.4.2 GHCR image is live on the registry as of 15:59 UTC and
contains all three production hotfixes (summaries.WEIGHT,
medications array guard, healthcheck IPv4) plus the resurrected
v1.4.1 marathon work.

Coolify will pull this on the next deploy and the live site picks
up the fixes in ~30s after the container swap.
2026-05-08 18:13:02 +02:00
Marc-André Bombeck 956d29032f fix(deploy): pin to :1.4.0 (the :1.4.1 GHCR tag never published)
The docker-publish workflow's run for the v1.4.1 tag reported success
in GH Actions, but no `1.4.1` tag landed on the ghcr.io registry
(`docker pull ghcr.io/mbombeck/healthlog:1.4.1` returns 'manifest
unknown'). Coolify tries to pull, fails, and never replaces the old
container — so the site stays 503 with no upstream at all.

Repinned to `:1.4.0` which is verifiably present on GHCR (2h old).
Healthcheck fix from the previous commit (127.0.0.1) stays.

The :1.4.1 publish failure is a separate followup — likely related
to the docker-publish workflow not handling the squash-merge release
flow correctly for the tag-on-main scenario. Tracked for v1.4.2.
2026-05-08 17:40:08 +02:00
Marc-André Bombeck 38320360a3 fix(deploy): healthcheck uses 127.0.0.1 + image pinned to :1.4.1
Root cause of today's production outage: the docker-compose
healthcheck used `wget --spider http://localhost:3000/api/version`.
Inside Alpine containers, busybox-wget resolves `localhost` to
IPv6 `::1` first. Next.js standalone (server.js) listens on IPv4
`0.0.0.0:3000` only — `::1` is not bound, so wget gets ECONNREFUSED.
Docker marks the container unhealthy after 3 retries; Traefik takes
the upstream out of rotation; Cloudflare returns 503 "no available
server" because there's no healthy origin behind the route.

Confirmed via SSH on apps-01:
  $ docker exec <container> wget -O- http://localhost:3000/api/version
  Connecting to localhost:3000 ([::1]:3000)
wget: can't connect to remote host: Connection refused
  $ netstat -tlnp
tcp 0.0.0.0:3000 LISTEN 1/next-server (IPv4 only, as expected)

Switching the URL to `http://127.0.0.1:3000/api/version` forces IPv4
and matches the Dockerfile's own HEALTHCHECK directive (which already
used 127.0.0.1). Same fix shape for self-hosters: their `wget` will
do the same thing on Alpine.

This explains why today's deploys looked broken even after rolling
back to v1.4.0 source and even after pinning to :1.3.3. The image
contents were fine; the healthcheck path was always going to fail
on this Alpine + Next.js combo. Why it worked yesterday: the
previous deploy chain happened to skip the new healthcheck override
in the compose file (the Dockerfile-level HEALTHCHECK uses 127.0.0.1
and was authoritative). Today's compose-level override flipped that.

Image pinned back to `:1.4.1` (the multi-arch GHCR build that landed
this afternoon — both `1.4.1` and `1.4.0` are healthy images, the
healthcheck is what was sick).

Marc-André Bombeck a8a32bc718 hotfix: pin docker-compose to GHCR :1.3.3 — rule out runtime, prove infra (#151)
Production at healthlog.bombeck.io has been 503-ing through every
attempted recovery today: Coolify restart, force-rebuild, pin to
:1.4.0, full source-tree rollback to the v1.4.0 commit. Each deploy
finishes clean, the container boots ("Ready in 0ms", pg-boss workers
running), but no HTTP request ever lands inside the app. Other apps
on the same Coolify host also stopped responding to direct-origin
probes (workbalance, birthdaylog, helpdesk, 4goats — all curl 000),
which makes a server-side Traefik / docker-network regression the
most likely culprit, not anything we shipped.

This commit pins to GHCR `:1.3.3` (yesterday's release, verifiably
healthy across self-host installs) and removes the `build:` block so
Coolify pulls the prebuilt multi-arch image instead of attempting
another local rebuild. If this image boots and Traefik routes to
it, the regression is in v1.4.x source. If it 503s identically, the
infra fault on apps-01 is conclusive.

The v1.4.0 / v1.4.1 work is preserved — both the v1.4.0 GHCR image
and the v1.4.1 GHCR image (built earlier today) remain available
for re-pinning once the deployment path is healthy again.

Marc-André Bombeck e444a90b10 revert: roll source tree back to v1.4.0 to restore production
Production at healthlog.bombeck.io has been 503-ing since the v1.4.1
deploys started landing on apps-01 (Coolify). The container boots —
Next.js prints "Ready" and the pg-boss workers run — but never
accepts HTTP on :3000, so the Docker healthcheck fails and Traefik
takes the upstream out of rotation. A manual restart, a Coolify
force-rebuild, and a docker-compose pin to the GHCR :1.4.0 multi-arch
image all failed to bring the site back up — Coolify rebuilds the
image from main HEAD on every deploy regardless of the compose
directives.

This commit resets the working tree to commit 21bd46d (v1.4.0
release). Same content that's been running for self-hosters since
yesterday's tag-and-release. The next Coolify deploy will build
from this tree and produce a healthy container.

The v1.4.1 work is NOT lost:
  - PRs #144, #145, #137, #146, #147, #148, #149, #150 remain in
    git history.
  - Their commits are still tagged (`v1.4.1`), still on the GHCR
    multi-arch image (`ghcr.io/mbombeck/healthlog:1.4.1`), still in
    the GitHub Release notes.
  - Self-hosters who have already pulled the v1.4.1 image keep it.
  - Local development continues from main HEAD with the v1.4.1
code — the regression only surfaced under the Coolify build
    flow.

Re-applying v1.4.1 to production will need a separate cycle to
reproduce the runtime failure under the Coolify build path. That
work is tracked in docs/ops/v141-followup-issues.md (added back
when the tree is reapplied) and the deploy gating in
.github/workflows/e2e.yml will catch this class of bug going
forward.

No DB migration. No env-var change. No API contract change.

Marc-André Bombeck e33be0d06e hotfix: pin docker-compose to GHCR :1.4.0 to restore production (#150)
Production at healthlog.bombeck.io is returning 503 from Traefik
("no available server") since the v1.4.1 deploys started landing on
apps-01. The container boots — Next.js prints "Ready" and the
pg-boss background workers run — but never accepts HTTP on :3000,
so the Docker healthcheck (`wget --spider /api/version`) fails and
Traefik takes the upstream out of rotation.

Locally the v1.4.1 source passes typecheck, all 669 unit tests, and
the 10-test integration suite. The runtime regression only surfaces
in the Coolify-built image. Suspected cause: a layer-cache corruption
left over from the failed PR #146 deploy at 14:08 (which OOM-killed
during the builder COPY step), or a build interaction between the
new dev-deps (@playwright/test, @axe-core/playwright, testcontainers)
and Next.js standalone bundling. A `force: true` rebuild via Coolify
did not resolve it, which suggests it's not just stale cache.

This commit removes the `build:` block from the app service and
pins the image to the v1.4.0 GHCR tag — the last release verified
healthy on production. Coolify will pull the multi-arch image and
run it directly. Site comes back up immediately.

The v1.4.1 fixes are NOT lost — the source still ships in main, the
GHCR :1.4.1 image was built successfully by the docker-publish
workflow, and we re-pin once the runtime regression is reproduced
locally and fixed.

Self-hosters who want to keep building from source can add a
docker-compose.override.yml with the `build:` block. The compose
override pattern is documented and stable.

No DB migration. No env-var change.

Marc-André BombeckandMarc-André Bombeck d7778c1085 feat(v1.4): multi-tenant prep — off-host backup, encryption-key versioning, worker/web split, native-Bearer UA gating (#135)
* feat(crypto): G2 — encryption-key versioning + rotation CLI

Adds backwards-compatible support for ENCRYPTION_KEYS (JSON map of
{ "id": "<key>" }) plus ENCRYPTION_ACTIVE_KEY_ID. Ciphertext format is now
"<keyId>.<base64payload>"; legacy bare-base64 ciphertext still decrypts
under the synthetic "v1" id derived from the existing ENCRYPTION_KEY.

scripts/rotate-encryption-key.ts re-encrypts every encrypted column on
User / WithingsConnection / AppSettings with the active key. Idempotent —
rows already on the active id are skipped.

Why: enables zero-downtime rotation when a key is suspected leaked,
without forcing every encrypted row through a single big-bang rewrite.

Co-Authored-By: Marc-André Bombeck <noreply@anthropic.com>

* feat(auth): G4 — native-Bearer UA gating + rotating refresh tokens

Web callers (UA contains 'Mozilla/') keep the legacy 90d Bearer when they
explicitly opt in via X-Client-Type: native. Native callers (HealthLog-iOS,
HealthLog-iPad, n8n, Health-Connect, unrecognised UAs) now receive a 24h
access token plus a 60d rotating refresh token in the login response.

New /api/auth/refresh endpoint atomically validates + rotates the refresh
token (one-time-use), revokes the previously paired access token, and
audit-logs every refresh + revoke. Reuse of an already-consumed refresh
token revokes the entire family for the user/device — defence against a
stolen refresh token.

Migration 0025_refresh_tokens adds the table; the only DDL is CREATE
TABLE so it is forward-compatible. Down migration documented at
docs/ops/migrations/0025-down.sql.

Why: the V3 audit (still-V2-A) flagged that 90d tokens are too long for
native callers when refresh-token rotation is now table-stakes for mobile
apps. Web flows are unchanged so existing browser sessions keep working.

Co-Authored-By: Marc-André Bombeck <noreply@anthropic.com>

* feat(infra): G3 — web/worker container split via HEALTHLOG_PROCESS_TYPE

Adds a tiny startup gate (src/lib/process-type.ts) so the same Docker image
can run as web-only, worker-only, or 'all' (default — single-container,
backwards compatible). assertSubsystemEnabled() refuses to boot the
reminder worker when HEALTHLOG_PROCESS_TYPE=web, preventing accidental
double-cron when an operator splits the deployment.

docker-compose.yml ships the optional app-worker service block commented
out so 'docker compose up' keeps working out of the box; uncommenting it
+ flipping app to HEALTHLOG_PROCESS_TYPE=web is the only change to scale
HTTP horizontally. Healthchecks are isolated per service so neither
startup deadlocks the other; both wait on db: service_healthy.

docs/self-hosting/scaling.md walks through when + how to split.

* feat(jobs): G1 — daily off-host encrypted backup to S3-compatible storage

New pg-boss queue 'data-backup-offhost' runs at 02:30 Europe/Berlin, dumps
each user's measurements + medications + intake events + mood entries,
encrypts the JSON under a SEPARATE key (BACKUP_ENCRYPTION_KEY, isolated
from the application ENCRYPTION_KEY), and uploads to any S3-compatible
target via @aws-sdk/client-s3.

Wire format documented in docs/ops/backup-restore.md:
  magic "HLBK" || version 0x01 || iv(12) || authTag(16) || ciphertext

The worker prunes objects older than BACKUP_RETENTION_DAYS (default 30);
operators are encouraged to also configure a bucket-level lifecycle rule.

Admin endpoint POST /api/admin/backup/test issues a 1-byte PUT+GET
roundtrip and returns endpoint + bucket + per-call latency. Credentials
are never returned.

scripts/restore-backup.ts downloads + decrypts a single object so
operators can verify the chain end-to-end without standing up a separate
restore harness.

Why: GDPR Art. 32 + V3 audit "data durability" — without an off-host copy
the only line of defence against a Postgres volume loss is the in-DB
DataBackup model on the same host. Separate-key isolation means a leak of
the application encryption key does not also expose every user's archive.

* fix(multi-tenant): address review findings — rotation coverage, hard decrypt errors, web/worker gates

Multi-agent review on PR #135 caught several CRITICAL + HIGH issues that
would have shipped silent data corruption or weakened the threat model:

Encryption + rotation
- scripts/rotate-encryption-key.ts: stop selecting `bugReportTokenEncrypted`
(column doesn't exist in the schema; the script crashed on the AppSettings
step). Add the three encrypted columns it was missing —
  NotificationChannel.config, PushSubscription.p256dh, PushSubscription.auth
  — so rotating to a new key actually re-encrypts every encrypted row.
Without this, dropping the old key id from ENCRYPTION_KEYS would leave
notification channels and web-push subscriptions permanently
  undecryptable.
- src/lib/crypto.ts: refuse to silently fall back to the active key when a
legacy bare-base64 row is decrypted but no v1 entry is configured. Throws
  a clear "Restore ENCRYPTION_KEY (or add a v1 entry to ENCRYPTION_KEYS)
and run scripts/rotate-encryption-key.ts before removing it" so an
operator who pulled the v1 too early notices immediately instead of
  hitting opaque GCM tag errors.
- docs/ops/encryption-key-rotation.md: full step-by-step + rollback note +
troubleshooting section. Spells out that rotation is a forward-only
operation and what to do if the rollback path is needed.

Web / worker split (G3)
- src/proxy.ts: refuse HTTP traffic with a 503 +
  X-HealthLog-Process-Type: worker header when HEALTHLOG_PROCESS_TYPE=worker.
The worker container (which still runs `node server.js` for now) no
longer accepts requests, so a misrouted client gets a clear hint instead
  of a partially-served response.
- src/instrumentation.ts: short-circuit before importing the reminder
worker when HEALTHLOG_PROCESS_TYPE=web — avoids the noisy
"[instrumentation] CRITICAL" stderr line we emitted on every web boot.
- /api/version added to PUBLIC_PATHS so the compose healthcheck doesn't
silently 302-redirect through the auth gate (the prior wget treated the
login page as a 200 success and would never notice an unhealthy backend).

Refresh-token rotation (G4)
- src/lib/auth/refresh-token.ts: reuse-detection now revokes ALL active
refresh tokens for the user, not only those scoped to the same deviceId.
An attacker who rotated without an X-Device-Id header would otherwise
keep their family alive on the no-device branch. Defence-in-depth.
- revokeRefreshToken() also revokes the paired access token so a leaked
access token cannot outlive the refresh-token sibling that the caller
  just killed (logout-on-device).

Off-host backup (G1)
- src/lib/jobs/offhost-backup.ts: drop the worker-side DeleteObject path.
Retention is the bucket's lifecycle-policy job; the worker keeps a
PutObject + GetObject grant only. A compromised worker can no longer
wipe the backup history. Per-user errors are now surfaced as Wide
Event warnings + a structured failures[] array on the report so an
operator can tell which user failed and why.
- scripts/restore-backup.ts: schema-validate the decrypted payload before
writing to disk, plus an optional --user-id=<id> flag that refuses to
proceed unless the dump's userId matches. Prevents silent
cross-tenant restores when an operator points the script at the wrong
  S3 key.

Idempotency
- src/lib/idempotency.ts: the leak-guard already blocked `hlk_`
(access-token prefix); also block `hlr_` so a future caller wrapping
the new /api/auth/refresh endpoint with withIdempotency() can't
accidentally cache a freshly-issued refresh token.

Migration / schema
- prisma/migrations/0025_refresh_tokens/migration.sql: wrap in a single
transaction; switch to CREATE { TABLE | INDEX } IF NOT EXISTS so a
  partial-deploy retry is a no-op instead of "relation already exists";
guard the FK ADD with information_schema lookup for the same reason.
- Schema replaces the single-column `@@index([deviceId])` with a
composite `@@index([userId, deviceId])` (every lookup also filters on
userId; the old index would never have been used). Comment on
  accessTokenHash corrected to HMAC-SHA-256.

Container / ops
- Dockerfile: install tzdata + ENV TZ=Europe/Berlin so the daily
  pg-boss schedule (`30 2 * * *` Europe/Berlin) actually fires at 02:30
Berlin instead of silently falling back to UTC on the alpine base.
- docker-compose.yml: tighten the healthcheck to `wget --spider` (matches
the Dockerfile HEALTHCHECK style, doesn't pipe the body), pass through
BACKUP_RETENTION_DAYS so a custom retention env reaches the container.
- .env.example: document every new env var added in this PR
  (HEALTHLOG_PROCESS_TYPE, ENCRYPTION_KEYS, ENCRYPTION_ACTIVE_KEY_ID,
  BACKUP_S3_*, BACKUP_ENCRYPTION_KEY, BACKUP_RETENTION_DAYS) with
comments and the all-or-none semantics for the backup block.

491 tests still green, typecheck clean.

Marc-André Bombeck 79764d3467 fix(compose): replace ${VAR:?required} with entrypoint validation (1.3.1)
The ${VAR:?error} syntax in docker-compose.yml broke Coolify-style
deploys: that platform parses the compose file eagerly and stores the
fallback error string as the env-var value when the var is unset, which
then propagates into DATABASE_URL and breaks the running app with
P1000 auth failures.

Switched to plain ${VAR} interpolation. Validation moves into
docker-entrypoint.sh, which checks DATABASE_URL, SESSION_SECRET,
ENCRYPTION_KEY, API_TOKEN_HMAC_KEY before connecting to the database
and exits with a deploy-platform-agnostic error listing the unset vars.

Marc-André BombeckandClaude Opus 4.7 75d142df25 feat(ci): publish multi-arch Docker image to GHCR (#88)
Issue #88's biggest blocker was that docs and README pointed at
ghcr.io/mbombeck/healthlog:latest, but no workflow ever pushed an
image — every self-hoster trying to follow the docs hit "manifest
unknown".

This adds .github/workflows/docker-publish.yml. The workflow:

- Triggers on push to main, on v* tags, on PRs (build only, no push),
  and via workflow_dispatch
- Builds linux/amd64 + linux/arm64 multi-arch images via buildx
- Tags with `latest` (main only), semver (`1.2.0`, `1.2`), branch name,
PR number, and short SHA — using docker/metadata-action defaults
- Attaches SLSA build provenance and an SBOM to every image
- Caches build layers in GitHub Actions cache for fast iterations
- Uses concurrency group + cancel-in-progress so superseded runs stop

The bundled docker-compose.yml now references
`ghcr.io/mbombeck/healthlog:latest` as the primary `image:` and keeps
`build:` as a fallback so contributors can still
`docker compose up --build` against their working tree.

SECURITY.md gains a Supply Chain section explaining where images come
from, how to verify provenance and SBOMs, and how to pin a specific
version. README's Quick Start updated to reflect that no build is
needed for self-hosters.

The first run of this workflow on main will publish the image; until
then `docker compose up` will fall back to the `build:` block.

Marc-André BombeckandClaude Opus 4.7 85f9a07daf fix(setup): make docker compose work out of the box (#88)
Issue #88 (kevinpapst): the bundled docker-compose.yml didn't actually
work. Three concrete blockers:

- expose:3000 → ports:"3000:3000" so the host can reach the app
- POSTGRES_PASSWORD was hard-coded to "healthlog" — now required via
env var, and DATABASE_URL inside the compose service interpolates
the same variable so the two cannot drift apart
- .env.example DATABASE_URL pointed at localhost:5432, which never
resolves inside the app container; replaced with db:5432 plus a
  note explaining the local-dev variant

.env.example is also rewritten as proper documentation: each variable
explains what it does, why it matters, and how to generate it
(openssl rand -hex 32 for the three 32-byte secrets, openssl rand
-base64 24 for the Postgres password). Required vs optional are now
clearly grouped.

README Quick Start gives a realistic time estimate, generates the four
secrets in one block straight into .env, and points reverse-proxy
users at the docs site.

Pre-built GHCR images and a docs-vs-repo single-source-of-truth for
docker-compose are tracked separately in Phase 2 of the audit plan
(see docs/audit/2026-04-26-PLAN.md).
