Marc-André Bombeck 0c9832eb8d fix(import): accept a blood-glucose reading that carries no context
A CSV import of a continuous-sensor export wrote nothing and rendered a
green success state. Three faults compounded, and the reporter took all
three apart before filing.

The parser refused every BLOOD_GLUCOSE row with a blank glucoseContext.
Behind it, the CHECK constraint from migration 0021 read both ways at
once: a glucose row MUST carry a context, every other type MUST NOT. The
second half is right and stays. The first was written when the only
glucose writer was the entry form, where a person picks fasting or
post-meal at the moment of the reading. Every machine writer that
arrived since has no such answer per sample, so the Nightscout pull, the
MCP log tool, the Telegram capture and both bulk importers were all
building rows the database then refused.

A missing context now stores as NULL, never as a fifth enum member.
Every per-context reader selects by equality against the four members,
so the reading stays out of the doctor report's fasting rows, the
matching FHIR observations and the per-context targets, while the chart,
the all-time summary and the clinical panel keep it in full. Hand entry
still asks for a context, because at that moment you know it.

The import cards now read the counts rather than the status code.
Nothing written is a failure, a mixed file is a warning, and the refused
rows are grouped by reason with a count instead of fifty identical
lines, with the per-line detail one disclosure away. Both cards render
through one outcome table, so a tick cannot reach a result that wrote
nothing.

Marc-André Bombeck 18691d4550 fix(medications): scope the intake idempotency key to its owner
`medication_intake_events.idempotency_key` carried a GLOBAL unique from
0001_init. The value is client-supplied, so two people running the same
client can mint the same key — and on a global unique the second one is
simply refused by the constraint. Their dose is not written and nothing
tells them so. The bulk drain made it worse: its replay probe read the
key with no user scope, resolved the first user's row, and reported the
second user's entry as a duplicate carrying a stranger's row id.

Migration 0273 replaces the global unique with `(user_id,
idempotency_key)`, the grain every other client-supplied identity on the
table already uses. Both unscoped reads in the bulk drain now key on the
composite; the other four reads were already narrowed by user.

On existing data this is a relaxation, so nothing needs reconciling: the
old constraint is strictly stronger than the new one, and no pair of rows
can violate an index the previous one already forbade. The migration
still carries a reconciliation step for a database whose global index was
dropped by hand at some point. It never deletes or rewrites a dose — it
clears the dedup HINT on the later row of a colliding pair and keeps
every intake event, timestamp, attribution and inventory stamp intact.
The new index is created before the old one is dropped, so a failure
aborts with the old protection still in place.

A structural guard freezes the read side: no `medicationIntakeEvent`
query may filter on the key without narrowing by `userId`.

Marc-André Bombeck f8f460bb66 refactor(measurements): drop the write-only aggregation columns (migration 0272)
Migration 0263 added three columns beside the Apple Health `stats:*`
authority ladder. Only `aggregation_provenance` is ever read back: the
external-measurement reconciler selects it and refuses a lower-authority write
against a higher-authority canonical row, and the cumulative drain reads it
too. That column stays.

`aggregation_contributor_count` and `aggregation_selected_source_hash` are
written by three call sites and read by none. No select, no raw SQL, no export,
no backup, no API contract, no repair path. The import-time selection keeps its
deterministic tiebreak, which compares an in-memory map key computed per flush
rather than a value read back from the table.

Both are plain nullable scalars with no index, unique, check or foreign key, so
the drop is a catalog-only operation that touches no measured value.

Marc-André Bombeck d0b45e36b3 feat(integrations): remove the moodLog integration
Mood has been a first-class module here for a long time, with entries,
structured tags, rated factors and notes. The bridge to the external moodLog
service had nothing left to add, and its Settings card was withdrawn in
a56e6fe61 on the promise of a dedicated removal. This delivers it.

Removed: the hourly `moodlog-sync` queue, cron and handler binding; the
inbound webhook and its shared-secret handling; the reverse push and its four
call sites; the pull-sync module; the per-user credential routes and the
instance-wide toggle; the entry on the `/api/integrations/status` envelope,
the `IntegrationKey` union, the cadence map and the display-name table; the
boot-time secret rotation pass; 22 message keys per locale.

Migration 0271 drops the five `users` credential columns and
`app_settings.mood_log_global`, clears the ledger row for the provider, and
deletes the retired pg-boss schedule so no cron keeps enqueuing work no
handler drains.

Mood data is untouched. `mood_entries` is not named anywhere in the migration
and cannot be reached from it: the dropped columns carry no index, constraint
or foreign key, and `mood_entries.user_id` references `users(id)`, not any of
them. Rows the bridge imported keep `source = 'MOODLOG'`, and every read path
- list filter, source badge, exports, backups - still resolves that value.

`src/lib/validations/moodlog.ts` held the native mood-entry schemas alongside
the bridge's own; it is now `validations/mood.ts` and carries only the former.

Marc-André Bombeck ce41d5bf17 fix(fitbit): key sleep segments on their start instant and sweep re-score leftovers
Fitbit re-scores a night after the fact, and the per-segment externalId carried
a positional index plus the stage label. Both moved on a re-score: renumbered
segments minted fresh keys, the previous scoring's rows stayed live because
nothing swept them, and the night total counted both copies from then on.

Key the segment on the stable session anchor plus its own start instant
(`<logId><segment-start>`), and add the replace-by-window sweep every
other segment-writing provider already has. The two ship together with the
natural-key rescue that has been sitting in the Fitbit upsert unused: the sweep
tombstones the old-keyed rows first, so the rescue re-keys those very rows in
place instead of inserting a parallel copy or losing the insert to
`skipDuplicates` against the natural unique index.

A one-shot repair queue re-reads the bounded sleep history once per connection
so no user has to do anything; it stamps `sleepRepairedAt` (migration 0270) only
on a clean run, never touches the sync watermark, and reports the rows it
removed. Nights older than the 365-day backfill horizon keep their old keys and
whatever duplicates they already hold — they can no longer gain new ones.

Alongside: `fitbitGet` now honours a `Retry-After` inside a 120 s cap by waiting
once and retrying once, and fails fast above it so a long hold cannot starve the
rest of the poll cohort or outlive a job's expiration. The measurement upsert
skips a write whose payload is unchanged, which takes the hourly overlap off the
delta feed for every Fitbit resource.

Marc-André Bombeck 1fca6c1d25 fix(mood): key mood rollups by the local day, not the UTC day
The persistent mood-rollup writer bucketed on the UTC truncation of
`mood_logged_at` and ignored the canonical per-row `MoodEntry.date`
label. Every coverage-miss fallback keys on `entry.date`, so the rollup
tier disagreed with the fallback and with the day the user logged the
mood: a late-evening or after-midnight local mood landed on the wrong
calendar day, and the label shifted the moment the boot backfill flipped
a user from the live fallback onto the rollup tier.

Key every bucket off the stored `date` label instead — DAY, WEEK, MONTH
and YEAR — with `bucket_start` set to that label's UTC-midnight instant.
The rollup is now byte-identical to the live `entry.date` fallback and
timezone-independent by construction: DST correctness stays where it
already lives, in `moodDateKey` at write time. No consumer changes, no
schema change; every rollup reader already formats `bucket_start` as a
UTC day label.

Migration 0269 empties `mood_entry_rollups` so the boot-time
full-backfill worker re-mints every user's buckets under the corrected
keying; the worker now deletes-then-refolds so it is self-cleaning
against any row left by an older instance. While the table is empty,
reads fall back to the live `date`-keyed walk, which was already
correct, so there is no window where a wrong day is shown.

Marc-André Bombeck 0b3280c604 chore(release): v1.32.11 — native web-handoff login for self-hosted iOS
Self-hosters can sign in to the iOS app through their own domain, so the
password manager files credentials under the right site and passkeys run in
the correct origin. The app opens the instance's own web login inside an
ASWebAuthenticationSession; after an interactive login the server mints a
single-use, PKCE-locked handoff code on a compiled-in custom scheme, which the
app exchanges for the standard native token bundle. The token pair never rides
a URL — only the opaque code does.

Three endpoints mirror the OIDC native handoff and share its mint/consume core,
now extracted to native-handoff.ts (the OIDC leg keeps working unchanged, as
the default flow):

  - GET /api/auth/native/login validates the PKCE challenge, sets a
                                   short-lived encrypted state cookie, and
                                   redirects to /auth/login?flow=native.
  - GET /api/auth/native/complete validates the web session, enforces the
freshness binding, mints the code, and
                                   redirects to healthlog://login-callback.
  - POST /api/auth/native/token exchanges the code under the cookie-less
native transport gate for the native bundle.

The freshness binding refuses any session older than the flow start, so a
pre-existing browser session can never silently complete a handoff. The start
time is stamped from the database clock (SELECT now()) to stay comparable to
Session.createdAt with no app/DB skew, and createdAt is read through a
dedicated projection so the comparison cannot be silently dropped. A flow
discriminator on the handoff row (migration 0268) makes an OIDC code
non-redeemable at the web-login route and vice versa.

The callback scheme is a compile-time constant with no redirect_uri parameter,
requireAdmin and requireFreshMfa stay cookie-only, and the exchange mints no
wider scope than password login. The state cookie is AES-256-GCM with a fresh
IV. Adds the flow=native login-page banner in all six locales and the three
routes to the OpenAPI contract.

Marc-André Bombeck a2fc6ea243 fix(auth): reconcile step-up elevations with main at v1.32.0
Renumber the elevation migration from 0260 to 0267 — main claimed 0260
for an unrelated table while this branch sat unmerged, and 0266 is now
the highest committed number.

Two loose ends the merge itself did not touch: the schema comment on
StepUpElevation.method still called it "audit/telemetry only, never an
authorisation input" — the exact claim the fresh-factor fix (a prior
commit on this branch) made false, since the method is now precisely
what decides whether an elevation reaches the destructive MFA routes.
Reworded to match. And pruneExpiredStepUpElevations in step-up.ts was
dead code: the actual sweep runs in cleanup-handlers.ts against the
worker Prisma client for reasons that function could never satisfy
against the request-scoped one. Removed rather than left to drift.

Marc-André Bombeck 0991d754e4 fix(auth): make the re-proved factor decide what an elevation reaches
A step-up elevation minted from the account password satisfied the fresh-factor
routes. That is a second-factor bypass: a stolen token plus a known password
could rotate the recovery codes — which take no factor in the body — and then
spend one of them to disable the second factor outright. The web has never
allowed that chain, because a password login does not stamp mfaVerifiedAt.

The method is now an authorisation input. Only totp, webauthn, and passkey
satisfy a fresh-factor gate, which is exactly the set of ceremonies the web
stamps a session for; password reaches what a plain cookie session reaches and
stops. The mint gains TOTP and security-key arms so a TOTP-only account can
still manage its factor natively, and the response says which reach it bought.

The gate also returns a discriminated context. session.id meant a session row on
one arm and a token row on the other, and the disable route passed the token id
to a session-scoped query — excluding nothing, so it deleted every browser
session and revoked the caller's own device login. The transport is now in the
type and the compiler refuses the confusion.

Redemption splits into validate-then-commit so a 429, a 422, or a wrong code no
longer burns a proof against a five-per-fifteen-minute mint ceiling. The claim
is UPDATE ... RETURNING, so the statement that decides is the one that reports.
The mint is transactional behind a partial unique index. Sign-out-everywhere and
both password-reset paths clear elevations. The status read drops to plain
requireAuth — it carries no credential material — and every mutation stays gated.

Marc-André Bombeck 0dba15e902 feat(workouts): add the per-workout insight model
Its own table, cascading from the workout row, because workout deletes are
hard deletes. The paragraph is AES-256-GCM at rest and registered for key
rotation; the input hash and prompt version make a re-post of the same
session a free no-op.

Marc-André Bombeck 5d62236412 feat(arrivals): add the data-arrival event spine
Every ingest path in the product can now emit a typed, salience-classified
event into pg-boss, consumed by one worker that claims a durable per-day
reaction marker and fans out to the reaction surfaces.

The recency classifier runs at the seam, before anything is enqueued, so a
mass backfill — a multi-year Apple Health export, a provider re-sync, a
30-day catch-up — emits nothing at all. Non-salient writes are annotated and
dropped.

The worker makes no provider call by construction: it imports no provider
module, writes only markers, and enqueues nothing that is not budget-gated
downstream. Business refusals return a status rather than throwing, so a
refusal is never retried against a ceiling that will not move until the local
day rolls over.

Seams: sleep (one hook, inherited by all seven transports), workouts (the
batch route, the four provider syncs, the export importer), weight and blood
pressure (both measurement routes), labs (manual, OCR commit, document fact
commit).

The provider workout syncs could not previously tell a created row from an
updated one, so a re-sync of a recent session would have re-fired on every
poll; they now recover the distinction from the row the upsert already
returns.

Reaction lines are AES-256-GCM at rest, following the InsightNarrative
precedent, and the column is registered for key rotation. Markers are pruned
after 14 days by a daily maintenance job.

Marc-André Bombeck e557d9e9e6 feat(auth): add token-bound step-up elevations for second-factor management
Second-factor management has been cookie-only since v1.23. The gate is right:
a Bearer token is a credential whose provenance the server cannot see, and
tearing down a second factor must not be reachable from the weaker of an
interactive login and a value pasted into a script. What was missing was never
Bearer access to those routes — it was a way to say, over a transport with no
session row, that the human holding this device just re-proved a factor.

An elevation is that proof. It is minted only against a re-proved account
password or primary passkey, bound to the exact token that re-proved, single-use
via a conditional UPDATE, five-minute-lived to match the cookie path's step-up
window, and stored hashed. A stolen token mints nothing; a second token cannot
redeem another's elevation; a password change drops every one the account holds.

requireMfaManagementAuth is the one gate the management routes share. On a
cookie session it delegates to the original helpers unchanged, so the web flow
runs the code it always did. requireAdmin stays cookie-only and out of reach.

Marc-André Bombeck 4c3cc84daf feat(medications): carry manufacturer and printed strength on an inventory container
The native pen list renders these two as its headline and subhead. The
server held neither, so a container registered on the web could not be
shown there without fabricating an empty card, and the client kept its
own copy locally with no back-fill possible.

Both columns are additive and nullable (migration 0255). Every existing
row is truthfully NULL — the values were never collected, and a plain
supply row such as a blister pack or a bottle has no use for either.

Strength stays free text rather than a parsed number + unit: pens state
it per dose, per ml, or per cartridge depending on the product, and
forcing a numeric split would lose which of the three was printed.

On PATCH an omitted field leaves the stored value untouched, the same
rule notes already followed, so a client that does not know the fields
cannot blank what another populated; an explicit null still clears. The
web add-container form collects both for a PEN, so the columns do not
ship empty forever.

Marc-André Bombeck a8b0365ca5 fix(auth): carry a generated secret in the session cookie, not the row id
The cookie was the Session row's cuid. cuid is built for collision-
resistant identity across distributed writers, not for unguessability —
it encodes a millisecond timestamp and a process-local counter and never
touches a CSPRNG. A session cookie is a bearer credential and has to be
unguessable, so it now carries 32 random bytes and only the HMAC of that
value is stored, matching how api_tokens and refresh_tokens already hold
their secrets.

Compatibility is passive, and deliberately so. A backfill cannot help:
the cookie already sitting in a browser is the cuid, and nothing we
generate server-side can be pushed into it — backfilling would sign
everyone out at deploy. Upgrading a row in place on first use loses to
the parallel requests a single page load fires: each mints its own
secret, one write wins, and the losers cannot re-emit the winner's
secret because only a hash is stored, so the browser can end up holding
a value the row does not honour. Instead, rows minted before the column
keep a NULL token_hash and stay resolvable by id, and are denied the
sliding-expiry extension so they drain within the 30-day session
lifetime. Every login from this release forward is on a secret at once,
and an id stops authenticating the moment its row holds one.

Note for operators: API_TOKEN_HMAC_KEY now also keys session cookies, so
rotating it signs browser sessions out as well as invalidating tokens.
It is already Core-required, so no deployment gains a new prerequisite.

Marc-André Bombeck d773f460e5 Tell the four document-summary outcomes apart
Opening a document said "the summary is being prepared" and kept saying it.
The view had one fallback for a null summary column and used it for every
outcome: a document nobody ever enqueued, a missing vision provider, a spent
budget, a withdrawn consent, an unreadable file, a provider error, and a
summary the outbound screen withheld all rendered as work in progress.

The common case is not the screen. The background job is enqueued at exactly
one place — a fresh upload with the auto-read opt-in already on — and no
backfill reaches anything else. Every document uploaded before the toggle went
on has a null column no job will ever fill, and the view promised all of them a
summary that was never coming.

Record the outcome as `summary_state` (migration 0254, additive, defaults to
NONE which is truthful for every existing row) and branch on it. Only PENDING,
set when a job genuinely exists, may claim generation is running. NONE,
WITHHELD and UNAVAILABLE each say what happened and offer to generate one.

The withhold keeps its rule and loses its dead end. Blocked prose is still
never shown and never stored — only the fact of the refusal is kept, and it is
no longer terminal, so asking again re-runs the gauntlet. That was the original
intent; leaving the column null defeated it, because null read as "generating"
and the guard meant nothing would ever overwrite it.

The on-demand summary now persists what it produces. It is the only repair path
for the documents the job never ran for, and re-deriving the same paragraph on
every open costs a provider call for nothing. Generation is still only ever a
click — nothing runs because a view rendered. Transcription stays session-only.

Also drop the "read by AI" line from the detail footer.

Marc-André Bombeck e6d9cc229a feat(auth): native OIDC SSO session handoff for the iOS client
The web OIDC flow ends in a session cookie the Bearer-transport native
client cannot consume, so on an OIDC_ONLY instance the iOS app had no
sign-in. Add a native leg: the app opens the login URL with
client=native and its own PKCE S256 challenge inside an in-app browser;
the callback resolves identity exactly as before, then mints a one-time
handoff code and 302s to the fixed compiled-in scheme
healthlog://oidc-callback (or hands back the existing MFA ticket when
the account has a second factor). The app exchanges the code plus its
PKCE verifier at POST /api/auth/oidc/native/token for the standard
native token bundle.

The return address is a compile-time constant — no redirect_uri
parameter and no env allowlist, so the open-redirect / scheme-hijack
class is removed by construction. The native flag and app challenge
travel only inside the AES-256-GCM state blob, so a network attacker or
the IdP cannot flip a web login into a native one. The handoff code is
256-bit CSPRNG, HMAC-hashed at rest, 90-second TTL, single-use via a
guarded consume; a failed PKCE check burns it. A replay of a consumed
code revokes exactly the pair the first exchange issued. The token pair
never rides a URL; only the opaque code or the MFA ticket do. The SSO
login can never satisfy requireFreshMfa step-up — the transport is
Bearer and step-up is cookie-only.

Adds the OidcNativeHandoff model (migration 0252), the token-exchange
route, a daily cleanup sweep, and the OpenAPI entry.

Marc-André Bombeck 4f7420905a fix(strava): normalise raw sport labels to canonical WorkoutSportType
Strava was the second workout integration, after WHOOP, that wrote its
raw vendor sport label ("Run", "Ride", "VirtualRide", ...) straight into
Workout.sportType instead of a canonical bucket. Every Strava row read
as generic, and the read-time picker treats two generic rows as
sport-compatible — a same-source brick session (Strava ride ending
~15:00, Strava run starting 15:03) could cluster into one and silently
drop a leg.

Add mapStravaSportType() (54 documented Strava sport_type values,
mirroring the WHOOP sport-map shape) and apply it at write time in
mapActivity(); the raw label stays in metadata.stravaType for
provenance. Backfill existing Strava rows via a hand-written migration
generated from the same table. Extend the cross-provider sport-map
parity test to cover Strava, and add a picker-level test proving a
back-to-back Ride+Run pair now survives as two rows instead of one.

Marc-André Bombeck 5cff7473c9 feat(documents): persist an auto summary for uploaded documents
When the auto-AI-read opt-in is on, a freshly uploaded document is summarised
once in the background and the encrypted summary is stored and shown on the
document detail view.

- A background step runs after upload alongside the existing content-index and
thumbnail steps. It generates a short (3-4 sentence) plain-language summary
only when the user's documentsAutoAiRead opt-in is on (that flag is also the
standing AI-egress consent), skips a document that already has one, and
no-ops when no vision provider is configured. It reuses the same provider
  resolution, egress-consent re-assertion, and AI-budget reserve/reconcile
helpers as the on-demand route; a provider error refunds the reservation and
  never fails the upload.
- The summary is stored encrypted (summaryEncrypted Bytes) with a
summaryGeneratedAt timestamp on inbound_documents, registered for key
rotation, and returned decrypted in the document detail response.
- The on-demand transient summary route is unchanged.

Gated by the same consent, budget, and module checks.

Marc-André Bombeck da2de015fe feat(nutrients): source-aware day totals; manual water write path
NutrientIntakeDay's composite primary key gains `source` (migration
0249_nutrient_intake_day_source): a manual water entry and the
Apple-synced day total now live as two rows instead of the manual
write being silently clobbered by the next Apple last-writer-wins
sync. Every existing row already defaults to source=APPLE_HEALTH, so
the migration is a pure PK-constraint change with no data rewrite.

- POST /api/nutrients/batch keeps writing the APPLE_HEALTH row,
byte-identical upsert semantics against the wider key.
- GET /api/nutrients sums amounts within (nutrient, day) across
sources before folding into the window summary.
- POST /api/nutrients/water (new): manual water quick-add, add/set
modes against the MANUAL row only, module-gated, rate-limited,

Marc-André Bombeck 270f33edb2 feat(daily): let the Today rail dismiss milestone / ECG / tension cards
The worth-a-look rail mixed actionable items (an overdue dose, a broken
sync) with observational ones (a milestone, a new ECG pointer, a tension
window) that have nothing to act on beyond "seen it" — there was no way
to clear them short of them expiring on their own.

Add server-side dismissal scoped to the three observational kinds only:
a new DismissedPriorityItem table (userId + itemKey, composite-unique),
a POST /api/daily/digest/dismiss route, and a deterministic itemKey the
digest builder stamps per kind (metric + reach day for a milestone, the
recording's own timestamp for an ECG, the local day + part of day for a
tension window) so a dismissal never bleeds into a later day's instance.
The actionable kinds never carry an itemKey, so the dismiss affordance
and the endpoint are both structurally unreachable for them — an
itemKey not namespaced under a dismissible kind 422s before any lookup.

Marc-André Bombeck 86e92f38ca fix(db): backfill non-canonical WHOOP workout sport_type values
Existing WHOOP Workout rows still carry whatever the pre-fix write path
stored — a whoop_sport_<id> placeholder or WHOOP's raw sport_name — so
the sport-map fix only corrects new syncs. This one-shot UPDATE
re-derives every existing WHOOP row's sport_type from the same table
sport-map.ts uses, generated directly from that table so the historical
data and the live mapper can't drift apart. Unmatched values default to
"other", matching the mapper's fallback. Idempotent: the WHERE clause
skips rows already on a canonical value, so a re-run touches zero rows.

Marc-André Bombeck d470d406a7 Merge remote-tracking branch 'origin/main' into feat/documents-coach-bundle 2026-07-17 00:02:51 +02:00
Marc-André Bombeck 502783a3ea feat(coach): attach multiple documents to a conversation
Replace the scalar document_id discriminator on a coach conversation with a
CoachConversationDocument join table and a sticky documentScoped flag, so a
conversation can be grounded on several stored documents at once while the
prompt-injection fence stays intact across the data-model change.

The fence is re-established at both enforcement points, fail-closed:

- Client: resolveCoachSendTarget routes any turn on a fenced conversation (or a
fresh chat with staged attachments) to the new /api/insights/chat/fenced
endpoint; the fenced body is strict and never carries scope, guided-question,
  or prefill hints.
- Server: the tool route fetches with documentScoped:false AND 404s when the
fetched row still carries an attachment (flag/join drift), emitting a
fence_drift audit alarm. Document text now enters an LLM prompt in exactly one
code path — the fenced pipeline — with no tools and no health snapshot; a
module-graph test pins that the tool loop is unreachable from it.

The fenced turn fences each document separately with a server-built, scrubbed
header line (title and filename are untrusted text), runs a per-document egress
consent check on the single picked provider, and grounds numerically over the
live attachment union only. Attachments are capped at five documents and 192 KB
of combined text with honest truncation.

Attach and detach ride dedicated endpoints: attaching validates the document is
owned, live, indexed, and within cap, then flips the conversation fenced (the
one legal, privilege-reducing transition, audit-logged); detaching removes the
document but never clears the flag. The document-sheet chat keeps its public
wire contract and shares the same fenced pipeline over the join.

Migration 0245 adds the join table and the flag, backfills every legacy
single-document chat into a one-attachment fenced conversation, and drops the
scalar column in the same transaction — no dual-truth window.

Marc-André Bombeck b6ded4d1f9 feat(daily): finalise the morning digest when last night's sleep lands
The nightly insight pre-pass runs ~04:30, before last night's sleep has
synced, so the morning digest and health score reference the day without
the current sleep. Close the gap the event-driven way instead of an
on-mount generation.

A day's digest is now provisional until the sleep-dependent output has
been regenerated with last night's sleep, and final after. The lifecycle
is driven by a real marker (User.morningDigestRefreshedOn, the user-local
date the refresh completed), not a heuristic: it flips the instant the
refresh runs, ahead of the snapshot cache that feeds the sleep last-seen
fallback. A day with no sleep device stays honestly provisional and keeps
the sleep-pending note rather than showing a stale final score.

When a completed sleep segment for last night lands from any of the three
sleep transports — Withings sync-sleep, the WHOOP sync, the Apple-Health
measurement batch — a debounced morning-digest-refresh job is enqueued.
"Last night" is judged in the user's profile timezone, so a full-history
re-sync never re-triggers for an old night. The queue singletonKey plus
the marker collapse the many samples of one night, across all sources,
into one refresh per user per local morning; the job re-runs the same
comprehensive pipeline the nightly cron uses (forced, so the content-hash
gate turns a no-change regen into a cheap no-op) and stamps the marker. A
hard generation failure leaves the day provisional and hands recovery to
the existing 45-minute briefing-retry machinery.

Marc-André Bombeck 656e96f72b chore(release): v1.28.46 — server performance on large instances
Deferred perf fixes from the campaign's verified perf-correctness audit,
correctness-first (each with an output-equivalence test):

- comprehensive-generate fan-out: replaced per-type 400d findMany+JS-average
with DB prisma.aggregate(_avg) over the exact half-open windows the JS
reducer used (current 30d, lastMonth (60d,30d], lastYear (395d,365d]), same
rounding + null-on-empty, pLimit(4). SLEEP_DURATION stays on the raw path
(per-night reconstruction dedup). Byte-equality test vs the old algorithm.
- boot-discovery: replaced Prisma in-memory distinct with DB SELECT DISTINCT
in the 4 backfill discovery scans + migration 0243 (7 additive
partial indexes matching each predicate). Integration test asserts identical
  discovered set against real Postgres.
- ChatBubble wrapped in React.memo with a comparator (onRegenerate compared by
presence — the thread rebuilds it each render), so only the streaming bubble
  re-renders per token.
- doctor-report mood read narrowed to {score, tags} (the only fields the
collector uses); the 730d main measurement read left as-is — a blind bound
would corrupt clinical stats (documented deferred follow-up).

Migration 0243 additive-only.

Marc-André Bombeck 82aa66b400 chore(release): v1.28.36 — re-importing after a failed Apple Health import actually retries
The v1.28.33 rollup + staging fixes were correct but never reached the
reporter's case: the upload dedup keys on the content SHA-256 and the
lookup matched a previously FAILED job too, so re-uploading the same
export.zip returned the dead job's id and replayed its stale
failureReason without ever staging or sending again. The dedup now
excludes failed rows (short-circuits only on queued/unpacking/parsing/
upserting/done); the same bytes retry cleanly. A redundant staged upload
is unlinked on a dedup hit instead of leaking in the staging dir.

reconcileOrphanImportJobs no longer unconditionally fails every
non-terminal ImportJob at boot: it now checks pg-boss job state plus a
progress heartbeat (new import_jobs.updated_at, migration 0242), so a
split or multi-worker deployment cannot kill an import running in
another worker. The missing-staging message names the separate-container
cause explicitly, with a matching note in docs/self-hosting/scaling.md.

Marc-André Bombeck 5445fc67ea chore(release): v1.28.34 — nutrient intake lands as quiet context (server side)
Supplement-style intake written to Apple Health by nutrition apps —
vitamins, minerals, water, caffeine — can now sync as daily totals into a
dedicated NutrientIntakeDay store (migration 0241): composite (user, day,
nutrient) identity, re-post replaces, no soft delete, no rollup coupling.
A code-side catalog carries the 26 nutrient codes with canonical units,
plausibility caps, and EFSA dietary reference values (direction-aware;
chromium cites NIH ODS where EFSA declined a DRV).

Deliberately not a nutrition feature: no diary, no calories, no dashboard
surface. POST /api/nutrients/batch is gated behind the new opt-in
nutrients module (default off — with the module off the server refuses
ingest entirely), rate-limited, idempotency-wrapped, with per-entry
unit-mismatch and out-of-range skips. GET /api/nutrients serves a
catalog-ordered summary; the only visible surface is a read-only card
under Settings → Sources. Coach/correlation access is a later slice; the
iOS reading side is coordinated in the companion-app tracker.
2026-07-14 13:10:47 +02:00
Marc-André Bombeck fa326c398a feat(auth): pin OIDC accounts to their (issuer, sub) identity
An OIDC login now resolves the account by the durable (issuer, sub)
pair instead of the email claim. First contact stamps the pair — by
auto-provision or a one-time link against an existing account matched
by verified email — and every later login matches on it alone, so an
IdP-side email change refreshes the display email but can never
re-point the login at a different account. An email that matches an
account already bound to a different identity is rejected and audited
(auth.oidc.link_conflict), never silently re-bound.

The email gate tightens with it: linking and provisioning now require
an explicit email_verified: true — an absent claim is a reject
(oidc_email_unverified) rather than a benefit of the doubt, since an
IdP that does not assert verification cannot anchor the account match.
Provisioned accounts get a lowercased email and the same server-default
timezone resolution the register route uses.

Migration 0240 adds the nullable users.oidc_issuer / users.oidc_sub
columns with a composite unique index.

Marc-André Bombeck c400e2aa88 chore(release): v1.28.23 — sleep disagreement on the dashboard; Apple Health medication groundwork
The sleep source-discrepancy annotation now reaches the surfaces: SleepNight
carries the MAIN session's annotation (naps never cross-compare), the analytics
slim slice / dashboard snapshot / iOS summary thread it additively, and the
dashboard sleep tile renders the shared discreet marker (extracted from the
sleep panel — one implementation, two surfaces). Served totals unchanged,
pinned by tests.

Apple Health medications groundwork (server side of the medication-import
request): IntakeSource gains APPLE_HEALTH; medications accept an external
mirror identity (source-exclusive by policy) and dose imports carry the HK
dose-event UUID — both idempotent per stable key, with the concurrent-create
race resolved to the winner. Migration 0239, additive only. Entries without
the new fields behave byte-identically.

Hardening: doctor-report aggregation + PDF charts compute ranges with loops
instead of spread calls — a report over a sample-dense year cannot overflow
the call stack (the class fixed for the Google Health full sync).
2026-07-11 00:18:23 +02:00
Marc-André Bombeck 1212ed91d5 chore(release): v1.28.21 — sleep history heals itself; plateau context; sync diagnostics
A one-time background pass re-reads each connected account's Google Health
sleep history so nights stored twice before the sleep fix collapse to their
true total (new sleep_repaired_at marker, migration 0238; watermark-safe,
enqueued once at boot per un-repaired connection).

The sleep night read reports when two writers disagree about the same night —
the served total is unchanged, the disagreement rides along as an annotation
(sourceDiscrepancy on the night DTO) so clients can mark the number instead of
silently picking a side.

A GLP-1 medication whose weight has plateaued on the current dose says so next
to the estimated drug level (efficacy tab + insights) — a factual note from the
existing detector, association only, verb-guard covered.

Google Health diagnostics: the sync log now separates a daily-totals read that
returned nothing from one the importer could not parse
(googleHealth.rollup.emptyResponse vs droppedAll), and the connection test's
structure probe reports the raw response envelope key names when a rollup type
parses to zero — a per-account naming drift becomes visible from one report.

Marc-André Bombeck 6d02711e33 chore(release): v1.28.16 — documents-only shares stay documents-only
A share link created to carry only documents now records an explicit,
frozen document_only flag, and the shared view gates on that flag directly
instead of inferring documents-only from "are all report sections off?".
The inference would have re-opened an existing documents-only link the day
a new report section was added — the link's frozen settings never mention
the new section, so it would default on and begin serving that data. The
flag closes that path; the derived check stays only as the fallback for
links created before the column.

Migration 0237 adds the column and freezes it on for every existing link
whose sections are entirely off (checked at any nesting depth), leaving
full-record shares untouched.

Marc-André Bombeck eb102ac50c chore(release): v1.28.15 — document thumbnails; admin-shared AI access
Document vault shows a per-file thumbnail: a server-rendered preview,
downscaled and re-encoded so location and camera metadata are dropped,
served only to the file's owner over an authenticated request. Files that
cannot be previewed keep their type icon. Generation is bounded — a header
sniff refuses an over-large source before decode, so a crafted image cannot
exhaust the preview worker.

Also folds in admin-managed central AI access — an administrator connects
one access on the server and each user opts in or keeps their own provider,
off until an admin turns it on — and trims the redundant documents-search
caption and per-file searchable/read badges.
2026-07-10 01:32:28 +02:00
Marc-André Bombeck ee76672798 feat(documents): preview thumbnails in the vault overview
Render a small JPEG preview for each stored document and show it at the
card's leading edge, falling back to the kind icon when none exists.

- Generate previews in the background on upload (new document-thumbnail
  pg-boss queue) plus a boot-time corpus backfill for existing documents;
upload never blocks on or fails because of it. Images are downscaled via
@napi-rs/canvas, a PDF's first page is rasterised then downscaled, every
other type is skipped. Generation never throws.
- Store the preview encrypted at rest in a 1:1 DocumentThumbnail side table
(migration 0236) so the list/detail queries that omit the original blob
never drag the preview either. Same AES-256-GCM string codec + fail-closed
decrypt as the content index; wired into the key-rotation walk.
- Serve it through an owner-scoped GET /api/documents/inbound/[id]/thumbnail
  (auth + module gate + userId-scoped query + fail-closed decrypt), always
image/jpeg + nosniff + private, no-store; 404 on miss. The card renders an
authed lazy <img>, onError falls back to the icon.
- Add hasThumbnail to the document DTO so the card only fetches a preview
  that exists.

Also drop the "search looks inside your documents" caption from the overview
(the index-all prompt stays) and the per-card searchable marker; both were
noise. Remove the now-unused i18n keys across all six locales.

Marc-André Bombeck d7e616eaa8 feat(ai): add operator-shared central Codex connection with per-user opt-in
Introduce an operator-level ChatGPT-subscription (Codex) credential that the
operator connects once and shares server-wide, plus a per-user switch to route
AI egress through it.

- Schema (migration 0235): three encrypted columns on AppSettings for the
operator's central Codex access/refresh tokens and the ChatGPT-Account-ID
  claim, plus expiry/connected-at/status; a per-user useCentralCodex flag
(default false). Register the new encrypted columns in the rotation registry
  and script.
- Resolution: add the admin-codex chain type; append it to a user's resolved
chain only when they opted in AND the operator connected it. Tokens refresh
server-side against the singleton row (CAS-style), same primitives as the
  per-user Codex path.
- Budget: bill an admin-codex primary against the operator cost cap, not the
user plan — it drains the operator's account.
- Consent: admin-codex is server-managed external egress, gated by the existing
consent machinery (self-snapshot and document surfaces alike). No existing
  consent path is weakened.
- Document order: rank admin-codex with the subscription tier (last), so the
  document class stays local-first.
- UI: admin device-code connect/disconnect card (cookie-only admin routes) with
  honest copy on account pooling, shared rate limits, and training-by-default;
a per-user opt-in switch shown only once the operator has connected it, off by
default, behind a one-time honesty confirm.
- i18n across all six locales; unit tests for the chain type, budget cap,
consent gate, vision capability, and document order; a mocked e2e for both
the admin connect surface and the per-user switch.

The central credential's tokens are encrypted at rest and never returned to a
non-admin API; the opt-in only takes effect once the operator has connected the
shared account.

Marc-André Bombeck 46f8c71ee5 feat(labs-ocr): read PDFs on the configured provider, retire the separate scan provider
Lab-report OCR now rasterizes a PDF to page images for any non-Anthropic
vision provider, exactly as the document vault already does — Anthropic keeps
its native PDF block, every other vision provider (codex/OAuth, OpenAI, local)
reads the rendered pages. The capability probe reports `pdfSupported` whenever
the picked provider reads PDFs natively or the server-side rasterizer is
available, so the "scan a PDF" affordance shows for a codex provider too.

Drop the dedicated document-scan (Lab-OCR) provider: labs OCR always uses the
main configured provider chain. Removes the settings card, the `aiOcr*`
resolution branch, the GET/PATCH handler fields, the OpenAPI schema fields, the
i18n keys across all locales, and the five backing `User` columns (migration
0234). The on-device tesseract fallback is untouched.

Dashboard hero: stack the overdue-dose action directly beneath the verdict
text (tight gap, left-aligned) on every viewport instead of pushing it to the
far container edge on desktop.

Marc-André Bombeck 75eb0637e6 feat(strava): add per-user BYO-OAuth workout source + Garmin indirect-path docs
Add Strava as a per-user BYO-OAuth workout source, mirroring the shipped
WHOOP/Oura/Polar pattern. Strava caps every new app at single-player mode, so
each self-hoster registers their own app and pastes the client id/secret into
Settings (falling back to a shared env app). Strava is a workout source only —
it exposes no daily metrics and no training-depth analytics are derived.

- Schema (migration 0233): per-user encrypted client id/secret + access/refresh
tokens, plaintext athlete id, incremental cursor, and backfill marker on
  `User`; `STRAVA` added to `MeasurementSource`.
- Token model follows Oura's reactive-refresh-on-401 with a compare-and-set on
the stored refresh ciphertext, because Strava rotates its refresh token on
  every refresh.
- Client over safeFetch: authorization-code + refresh, activity list + budget-
aware per-activity detail (for calories/HR), honouring the 200/15min cap; each
activity maps to one `Workout` row. Activity name/description stay in metadata
as data, never surfaced or interpolated.
- Dedup reuses the existing source-agnostic two-layer picker: `STRAVA` slots
into the workout ladder below the device-native wearables and above
MANUAL/IMPORT, so an Apple-Watch + Strava twin collapses to one canonical row.
- Poll-cohort hourly sync (:17) + self-converging boot backfill, wired into the
  integration-sync registrar. Webhook deferred.
- OAuth routes (connect/callback/credentials/disconnect/status/test) with signed
  state, cross-account re-auth rejection, and a surgical `www.strava.com`
connect-src gated to the Strava surfaces.
- Settings card + consolidated status envelope entry; env + compose whitelist;
  i18n across all six locales.
- Garmin: no direct connector is possible (business-partner-only). Add
docs/integrations/garmin.md documenting the Apple Health / Google Health
Connect path and Garmin's withheld proprietary scores, cross-linked from both
platform docs, plus a non-connector info note in the Connections panel.

Regenerate docs/api/openapi.yaml for the new `STRAVA` MeasurementSource value.
