The documents view already round-trips its filters through the query
string; the measurements and mood lists kept theirs in memory, so any
navigation dropped them. The documents pattern is extracted into a shared
hook (use-url-filter-sync) and both lists adopt it: type/source/day-bounds
and the measurement value range survive reload, back and sharing, defaults
stay out of the URL, invalid facets drop leniently. The insights readings
subpage keeps its locked type in memory — that URL belongs to the insights
route. /measurements no longer strips ?type=, so the deep links the
Vorsorge cards mint now persist as filter state.
- The Coach verbosity picker is disabled while the concise tone forces
brief, with a hint saying so, instead of a live control that silently
does nothing.
- A fresh document upload flashes its new row like the duplicate path
always did; the settle decision is a pure exported helper with a test.
- /about gains its two inbound links: the /privacy footer and a
Settings→About credits row (the GeoLite2 EULA wants the attribution
reachable from the running app).
- The eight settings slugs shadowed by permanent redirects (the seven
layout children + sharing) leave the slug registry: no more
prerendering unreachable pages, no dead component wirings, and the
SectionPlaceholder fallback that could never render is gone. The
redirects, section components, and layout-hub subpages are unchanged.
A content-index attempt that failed (raster failure, a scan with no
text layer, a provider error) recorded its reason only in the server's
observability stream. In the vault, hasContentIndex false looked
identical for a document nobody ever tried and one that failed three
times, and the detail sheet had nothing to say about either.
Apply the summaryState pattern to the index. Two additive columns on
InboundDocument (migration 0333) record when the last attempt finished
and why it produced no index, as a closed reason set: local-empty,
local-unsupported, decrypt-error, raster-failed, provider-error,
pdf-needs-anthropic, empty-transcription. The writer sits at the choke
point the auto job and the corpus backfill share, and in the manual
index route; a success clears the reason. The provider path's specific
failure now survives the local fallback, since "no text" is less
actionable than "the PDF would not render". rasterizePdf names its
failure class so a build that cannot rasterize at all is distinguished
from a PDF that would not render. New guard in both index paths: an
empty provider transcription never becomes a successful empty index.
The detail sheet renders the reason in plain words (muted, six
locales). The DTO and OpenAPI carry the two fields; the disaster
recovery backup and restore carry them verbatim like their summary
siblings.
Refs #776
The index-all trigger returned Boolean(jobId) while the client typed
the field as a number, so the toast read "Indexing true document(s)".
The route now answers with the count of live documents still lacking a
content index, computed from the same live-scoped counts the usage
gauge reads, and an honest 0 when no job was created. OpenAPI schema
updated to the integer shape; the hook's type carries a contract note
so the wire and the toast cannot drift apart again. Route test pins
the number, a component test pins that the toast renders the count and
never the word "true".
Refs #776
The server chain existed end to end (extract stages PENDING facts, per-fact
edit clears the low-confidence gate, confirm is the only write into the
structured stores) but the document detail offered no control over any of
it: it counted observation facts and linked to Labs. A user whose automatic
staging run was skipped had to re-upload the same PDF through Labs > Scan a
report.
- The detail sheet gains a staged-facts block with honest states: pending
facts open an inline review (approve/reject per fact; a low-confidence
fact is checked and saved first via the existing per-fact edit); a
lab-filed, already-read document with nothing staged offers "Extract lab
values" over its stored text; otherwise nothing renders.
- The extract route gains { mode: "stored" }: the same text-structuring
pass the automatic worker runs, over the document's own content index.
No re-upload, no second read of the original; full gauntlet including
the shared document-AI bucket with charge-at-dispatch honesty; 422
notIndexed when no stored text exists.
- A manual "Read with AI" / "Read again" now continues into the same
lab auto-staging the background worker performs; the response reports
labFactsStaged and the toast names the values waiting for review.
- Unchecked values are rejected and the copy says so; an unverified
low-confidence fact receives no decision and stays pending rather than
being silently discarded. Confirmation remains the only write into
Labs, conditions, and medications; approved facts carry the
committedRecordId link back to the source document (existing model, no
schema change).
- The "lab values from this document" link now counts APPROVED facts
only — it claims values that actually exist in Labs.
- Copy in all six locales; OpenAPI regenerated for the new extract mode
and the index response field.
The document-AI actions (read/index, extract, suggest, summarise) shared a
hard-coded 6/hour bucket that lied twice: the 429 said "try again in a few
minutes" while the window is an hour, and several failure paths burned a
slot before any provider was dispatched.
- DOCUMENT_AI_LIMIT_PER_HOUR configures the shared per-user ceiling
(default 6, clamped 1-1000), wired end to end: compose environment
whitelist, .env.production.example, env manifest, operator docs.
- INSIGHTS_RATE_LIMIT_PER_HOUR joins the compose whitelist and manifest;
it was documented but never listed, so an operator value could not reach
the container.
- The 429 mirrors the bucket's reset instant into meta.retryAt (plus the
existing X-RateLimit-Reset header); the client derives "try again in
about N minutes" from it in all six locales, with a plural-aware key.
- A slot is consumed only when a request actually reaches the provider (or
the local index/echo work): malformed bodies, unreadable originals,
refused consent, and exhausted daily budgets refund the charged slot via
a window-scoped refundRateLimit primitive. Consistent across all four
routes sharing the bucket.
The AI section of the document detail sheet collapsed whenever auto-read
was on and nothing was pending review, without checking whether the
document actually has a content index. A document the automatic pass
never reached (uploaded before the preference, or the pass did not
complete) then had NO visible way to be read; turning auto-read off was
the only workaround.
Collapse only once the document is read (or when indexing is unavailable
on the transport, where no action could be offered anyway). An unread
document keeps the Read with AI action, introduced by a short honest
note in all six locales; the suggest and summarise extras stay hidden
because the auto-read promise still covers them. The three states stay
distinct: unread shows the note plus the action, an in-flight read shows
the reading label, a read document collapses the block.

Sweep the spacing rests that sat off the 1/1.5/2/3/4/6/8/10 scale: the
health-score panels and their skeleton, the recommendation ask-Coach
gutter, the onboarding tour footer, the vaccination row, the danger-zone
alert margins, the recovery-codes action wrapper, and the chart-tooltip
tile. Move the last hand-written micro-sizes (coach counters, the
personal-record badge, the score-ring label, the dose-history and
cycle-calendar headers, the mood tag chips) onto the text-2xs token, and
give the documents upload buttons the mobile tap floor their peers set.
A family of settings surfaces rendered a failed read as a bare
text-destructive line with no way to recover: the security sessions,
trusted-devices and activity cards, the coach memory and reminders
sections, the about-me section, and the document link/share pickers. Swap
each to the shared QueryErrorRow primitive with a retry control. The suite
covering the statically renderable members was watched fail first against
the bare lines; the two portaled pickers take the same swap.
- from the record: a document link picker on the capture form, gated on the
documents module (blanked when off, never post-filtered)
- from the upload: a VACCINATION scan offers to file against a dose recorded
within the shared ±7-day window; a single candidate pre-selects, two offer a
picker with nothing pre-selected (WR-8 seen red on the pre-select rule,
reverted by content hash), zero shows nothing, and it never blocks the upload
- the suggestion rule reuses the visit moment's window constant rather than
forking it; it registers with the shared filing module when that lands
- suggestion and link copy in six locales
The suggestion anchored on the document's own date, which the DTO carries
as a date only. The suggest endpoint takes a full instant, so every
document asked the question as `2025-10-14` and got 422 back; the field
then rendered nothing and no document ever offered the visit its date
falls near. Lift the date to noon UTC before asking, the shape the two
lab moments already send.
The e2e that should have caught it never authenticated: the visits spec
carried no storage state, so its `page` redirected to login and its
`request` reads answered 401. It also asserts a global verdict — one
visit in the document's window — so clear the account's visits before
each test, run the file serially, and keep it to one project, since two
projects mutating the shared account would leave two candidates in the
window.
The accessibility mock lagged the same feature: the detail sheet reads
`encounterLinks` off the document and the mock omitted it, so the sheet
threw before it painted. Add the field the server always sends.
A lab panel files itself against the visit it came from, and never has to. The
OCR review and the manual form both offer the suggestion through the one shared
field, anchored on the draw date rather than on now, so backdating a reading
moves the offer with it. The commit writes one link PER RESULT ROW, which is
why the join is m:n: a marker re-run on a different day belongs to its own
visit. Best-effort by design — the readings are the write that matters and must
not be lost to a link that could not be made.
The condition side gets a Visits about this card with an action that pre-links
the episode. From the episode side only, and that is a decision: an episode
spans weeks and a visit is a point, so matching the other way would be wrong
often enough to teach a person to ignore every suggestion in the product.
A due checkup gains a second action beside erledigt. Filing the visit as done
satisfies the checkup through the same forward-only, idempotent primitive the
checkbox uses, so doing both is one satisfaction and no error. The two actions
answer different questions — one says it happened, the other says where.
An appointment now appears where a person looks, and NEITHER surface relaxes
the reminder-table exclusion that keeps it out of the preventive-care lists.
Both read the visit table: a quiet line under the dashboard checkup block for
anything inside two weeks, and one rail item for today or tomorrow. A diff in
either that touches origin is the wrong fix.
A visit write now also refreshes the reminder reads. Closing a checkup moves
its next-due date on three surfaces that would otherwise keep showing it due
until their own poll.
OpenAPI regenerated in the same commit.
Every arm here is the same shape as the one that already exists for
conditions, because the vault's filing model is settled and a second shape
would be the thing to explain rather than the thing to use: encounterIds on
upload and on PATCH with replace-set semantics, linkEncounter / unlinkEncounter
beside the episode pair in the bulk action, and an encounterId list filter.
Both directions ride ONE table. The visit's sheet writes encounter_document_links
from its end and the document reads it from the other, declared as a second
direction in the link service rather than a fourth table. A visit's kind
crosses the wire as the enum constant rather than a rendered name: unlike a
condition label, which is the person's own words, a kind is one of eight closed
values whose human name the reader's own bundle owns.
The suggestion is offered where the document is being looked at, resolved by
the shared rule rather than by this surface: one candidate pre-selects with an
undo, two offer a picker with nothing pre-selected, none offers nothing at all.
It resolves with no AI provider configured, and it renders nothing once the
document already names a visit.
The bulk verb has a caller — the vault's selection bar files a whole selection
against a visit, mirroring the condition verb and hiding itself on an account
with no visits. Nothing here is one-ended.
OpenAPI regenerated in the same commit.
Maximizing the Coach drawer hands the conversation to /coach and carries
?doc= with it, so browser-Back can rebuild the sheet. That is the one
close-shaped transition whose whole point is to LEAVE the selection where
it is, and the close path has always returned early for it.
The dropped-traversal check added beside that close did not know. It saw
a closed sheet and a selection still in the address bar, which is exactly
the state it exists to repair, and it repaired away the return URL a
quarter of a second after the close had deliberately preserved it.
Whether it won that race was a question about how fast the machine
navigated, so it surfaced as one flaky run on the narrow viewport rather
than as a broken feature.
Both sides read one named predicate now, and the check is skipped when
the close kept the selection on purpose. Proved by driving the grace
window to zero, which is the worst case the timer can have: before, the
maximize never even reached /coach, three attempts out of three; after,
it passes there and the whole documents family passes with it. Twenty
consecutive runs of that family on the narrow viewport are clean.
Closing a card-opened document asks the browser to go back, so the entry
the selection pushed is consumed and the reader's next Back reaches the
page before it. A traversal is a request, not a guarantee: one asked for
while another is still settling can be dropped, and nothing raises when
it is. The sheet closes, React stays consistent, and the address bar goes
on naming a document nobody has open, so a reload or a shared link
re-opens the sheet that was just closed.
Pressing Escape straight after the browser's Forward button is exactly
that shape, and it reproduces: ten repeats across both viewports failed
two of twenty with the URL still carrying the id, and none of twenty once
the close verifies itself. The check is guarded by the document id, so a
traversal that landed on a different selection is left alone, and it is
cancelled the moment the sheet re-opens.
The browser test that found it could not say so. It asserted the sheet
was closed through a locator matched on the document TITLE, and the sheet
falls back to an untitled label while its detail read is pending, so the
assertion also passed on an open sheet showing a skeleton, resolved
immediately instead of waiting, and pushed the failure into a URL poll
ten seconds later. It now asserts the dialog role, which is what the
neighbouring deep-link test already does.
The detail sheet loaded the original twice on open — the inline preview
and the download href each fetched and decrypted the whole blob — and
for a non-inline document the old href had cost nothing until clicked.
- one loader, lifted to the sheet, and only for a document that renders
inline; everything else fetches at click time
- make the download a real disabled control that reflects a failed
fetch: href="#" with aria-disabled announces unavailable and still
activates, and it ignored the failure entirely
A browser-issued <img src>, <iframe src> or <a href download> cannot
carry the record-session assertion, so the fence refused all three on
any session that had been inside a shared record — permanently, since
none of them will ever send a header.
- add a structural guard: no client file may name a record-fenced route
in a src=, href= or window.location sink
- fetch the thumbnail, the inline preview and the download through the
app transport and hand the DOM an object URL, revoked on unmount
- make the two API probes in the managed-profiles browser spec assert
the same context the app beside them asserts
Seven findings from the adversarial pass before v1.36.1 was tagged. One
blocking defect shipped in that release; these did not, and they are all places
where somebody looking after another person's health record meets a wall with
no explanation, or where a guard cannot fail.
The document vault was the largest. `/documents` is marked as a shared
destination, so the shell offers it under a switch, and every read behind it
resolved the caller instead of the record: the vault painted a query-error card
and the illness view painted another beside it. That is the same failure the
release describes as fixed on the dashboard. Five GET arms move to the record
resolver now, and only the GET arms: the list, one document, its file, its
thumbnail and the vault's usage payload, which the filter bar is built from.
Uploading, retyping, deleting, linking a document to an illness and every AI
verb stay with the owner, and the two reads that spend decryption CPU key
their quota on the person calling rather than the record, the rule
`medications/compliance` already follows.
Logging a value on a metric you track yourself leaves the admitted set. It was
admitted, named on the consent screen in six languages, and unreachable: the
only form that posts one sits on a page a shared record does not open, and the
component that renders it says so in its own comment while returning null. It
is the third route the rule that withdrew allergies and family history fitted
exactly, and it was applied to two of the three. Reading those values stays.
Two guards could not fail, and both were proved by mutation before and after.
The prefetch guard checked rule three with two regexes: one was satisfied by
the doc comment every page carries, the other missed an aliased import, and
the pair stayed green with the dashboard genuinely prefetching off the
switch-blind session. It reads the AST now, resolves aliases through the
module's own import table, and pins the five prefetching pages by name so a
page that stops being detected is loud. The activity verb map decided
membership on a literal `requireRecordAuth("write")`, so putting the argument
on its own line, which is what the formatter does the moment the call grows,
dropped a route out of the scan and turned a genuinely-red run green. Both
counts are of matches now rather than of files walked.
The rest are single instances of rules applied everywhere else. "Take all due"
was withheld on a note saying it rides the bulk intake route; it does not, and
its own docblock says why not, so a helper with five morning tablets tapped
five times for nothing. Marking a dose skipped is offered wherever marking it
taken is. A form could be opened in the frame before the app knew whose record
was on screen and then stood there once the answer arrived, on the capture
sheet and on the dashboard's quick-entry sheets. Two more copies of the intake
toast were still building their own, one of them with an Undo pointing at a
route no delegate may call. The customise link on the mood page led to a page
that exists to say it is not part of what was shared. And the access-level
radios on the invitation screen are hidden by design with no focus ring on
their label, so keyboard focus moved invisibly across the one screen where
somebody grants write access to their record.
Three new structural guards, each watched fail first: the customise link sits
inside a capability branch on every surface a delegate can reach, every intake
surface confirms through the one shared decision, and the batch sweep is
offered at the level that admits it.
Nine surfaces offered a delegate an action the server declines, and the
switch had never been asked about any of them.
The Today rail's dismiss and the coach check-in's keep / let-go write
through routes that resolve the caller, so both refuse under a switch;
they now ask canManage and are absent at either grant level. A priority
action with no href and no handler is no longer rendered as an inert
button at all — the type carries that rule now.
The Vorsorge dashboard card offered the mark-done that /checkups already
withholds, and the checkups list branch inlined its own ungated copy of
the button the cards branch gates, so which view a browser last chose
decided whether the action appeared. Both bind canManage.
The chart overlay cog persists through a route that resolves the caller
and stores a preference belonging to the person rather than the record.
Gated once in the control so the three chart wrappers cannot drift.
Three query parameters opened sheets past a gated button: ?add=<TYPE> on
measurements, ?new=1 on medications, and ?edit=1 on a medication detail,
the last of which opens a wizard refused at both levels. A deep link is
the same affordance as the control that produces it.
The efficacy retarget dial rewrites a setting the owner chose. The
episode documents card offered a link and an upload the vault gates on
the same endpoint throughout. And the dose ledger built its own success
toast, missing both the Undo suppression and the "saved to" receipt its
two siblings already carry — the one a delegate met on every dose.
The Coach drawer is not mounted inside a shared record, so every button
calling askCoach() opened nothing. The launch provider now publishes no
value there and each entry point's existing null check does the rest;
the documents sheet, the one that rendered without checking, now checks.
Every page a switch can reach now renders the controls the grant covers and
none of the others. Readings, lab results and illness entries keep their add
path at WRITE and lose edit, delete and selection at both levels. Mood,
screeners, cycle days, checkup reminders and the document vault lose every
mutation control at both levels, because no grant admits those verbs and the
routes behind them refuse a switched session outright.
Absent, not disabled, throughout. The banner already says whose record this
is, so no chip repeats it beside each missing button; the one exception is
the measurements empty state, where somebody who may add will look for the
edit control beside the row they just entered, and one muted line says where
the limit is.
Two dead ends went with them: the custom-metric section on the measurements
page linked every row into a route the shell turns a delegate away from, and
the wrench glyphs point at settings pages sharing does not cover.
The Del chip added beside the document card measured 4.06:1 against the card's
background, under the 4.5:1 floor, and axe failed the vault on it. So the
change that made the destructive key visible introduced a barrier of its own.
It uses the full foreground token now. The design rule says muted for meta
text, and a key hint is meta — but it is the one kind of meta that has to be
readable at a glance, so here the contrast rule wins and the reason sits next
to the class list rather than in a commit nobody will read again.
Found by running the e2e suite locally before pushing rather than after.
A focused card deleted on Backspace as well as Delete. The timeline's
roving tabindex gives a card focus the moment someone tabs into the
grid, Backspace is the reflex "go back" key, and a card carries no
delete control at all — so the reflex press removed a medical document
with nothing but a transient undo toast behind it.
Delete alone now deletes. The key resolution moves into a pure helper
in vault-utils so the contract is testable without a render harness,
and the card stops keeping that contract in a code comment: the overlay
button carries aria-keyshortcuts, and a kbd chip appears while the card
holds focus. The label is per-locale (Entf / Suppr / Supr / Canc).
Opening a document said "the summary is being prepared" and kept saying it.
The view had one fallback for a null summary column and used it for every
outcome: a document nobody ever enqueued, a missing vision provider, a spent
budget, a withdrawn consent, an unreadable file, a provider error, and a
summary the outbound screen withheld all rendered as work in progress.
The common case is not the screen. The background job is enqueued at exactly
one place — a fresh upload with the auto-read opt-in already on — and no
backfill reaches anything else. Every document uploaded before the toggle went
on has a null column no job will ever fill, and the view promised all of them a
summary that was never coming.
Record the outcome as `summary_state` (migration 0254, additive, defaults to
NONE which is truthful for every existing row) and branch on it. Only PENDING,
set when a job genuinely exists, may claim generation is running. NONE,
WITHHELD and UNAVAILABLE each say what happened and offer to generate one.
The withhold keeps its rule and loses its dead end. Blocked prose is still
never shown and never stored — only the fact of the refusal is kept, and it is
no longer terminal, so asking again re-runs the gauntlet. That was the original
intent; leaving the column null defeated it, because null read as "generating"
and the guard meant nothing would ever overwrite it.
The on-demand summary now persists what it produces. It is the only repair path
for the documents the job never ran for, and re-deriving the same paragraph on
every open costs a provider call for nothing. Generation is still only ever a
click — nothing runs because a view rendered. Transcription stays session-only.
Also drop the "read by AI" line from the detail footer.
Coach deep-links now carry launch context in the URL instead of dropping it
at the route boundary: /coach reads ?scope= (validated against the closed
CoachScopeSource enum) and ?ask= to seed the drawer's own prefill/scope
props, the Today check-in's "Adjust" action seeds ?ask= with the plan's own
words instead of opening a blank chat, and the workout detail page scopes
its coach button to workouts.
IA: give the document vault a "View N lab values" link on LAB_RESULT-shaped
documents (the auto-stage/OCR-commit linkage existed server-side with no UI
surface); disambiguate the two "Recovery" pages (RECOVERY_SCORE anatomy vs
the composite wearable page) with a distinct label plus a two-way
cross-link; move the opt-in nutrients sync card from Source priority to
Integrations, where every other sync surface lives; rename the user-menu
theme switcher off "Appearance" (collides with the Settings hub) and the
Environment settings section to "Location & weather"; cross-link mood,
mental-wellbeing check-ins, and mood insights, which were three
unconnected islands.
Onboarding: the getting-started checklist's data-source/notifications rows
now gate their fetch on the same checklistRelevant condition the other rows
use, instead of only-while-onboarding — they used to be stuck undone
forever for anyone who finished the wizard with under 5 measurements; the
tour wrap-up no longer promises a replay affordance that was removed;
the wizard's Withings card shows a connected badge once the OAuth
round-trip lands instead of leaving the button looking untouched in the
tab it was opened from; the done screen and Modules hub get a quiet
cross-link; the sleep empty-state CTA reads "Connect a source" instead of
being pinned to one vendor.
All copy changes land across every locale; de.json reviewed for register.
The authenticated shell's actual scroll container is <main
id="main-content"> (AuthShell sets it overflow-y-auto inside a
height-locked layout); document.scrollingElement never moves there, so
the gesture's "only arm at the top of the page" check was permanently
true regardless of how far a user had scrolled a long list. Point it
at the real scroll container instead, with the old document-root check
kept as a fallback for the few body-scrolled surfaces outside the
shell.
Extend pull-to-refresh from 4 pages (labs/checkups/mood/measurements)
to the dashboard, /insights, /medications, and the documents vault —
the sharpest PWA-resume-staleness surfaces, per the audit: a
background-refreshed dose or a cross-device write otherwise sits
stale until a full remount or the staleTime window lapses.
Intake history stayed a 4-5 column table with two full datetime columns
on mobile, forcing sideways scroll of a primary history surface; add
the stacked card-list split below md (the measurement-list pattern) and
lift the row kebab off a hard 32px box onto the size-11/size-9 floor.
The documents bulk-action bar wrapped five labelled buttons into a
2-3 row floating slab at 360px; collapse to icon-only (aria-label
carries the name) below sm, matching the document-detail-sheet
footer's established pattern.
Two coach/medication modals were still on raw Dialog while every
sibling had moved to ResponsiveSheet: the schedule-era editor and the
document attachment picker. Both now share the bottom-sheet-on-mobile
primitive; the era editor's submit button associates to its form via
the native `form` attribute since the footer slot sits outside the
form's DOM subtree (the log-intake-dialog precedent).
Tap-target floor: side-effect entry chips and the coach attachment-pill
remove control were below the 44px floor; the chips get a min-h-11
flex-center, the pill remove gets an invisible hit-slop instead of
growing the pill's visual footprint. Two insights empty-state CTAs
were missing the existing `ctaSize="lg"` treatment already used by
their sibling empty states.
Coach chat bubbles and the document OCR text panel wrapped long unbroken
tokens (URLs, IDs) past their column width on narrow viewports — add
break-words. Drop the /90 alpha on the coach error text so it stays on the
AA-retuned status token.
Today hero's loading skeleton had drifted from the v1.29.1 tightened hero
(gap/items-start/space-y/rail-heading/xl:grid-cols-3), causing a visible
shift on every dashboard load; mirror the loaded classes 1:1.
The wellness-scores strip returned null while its shared batch loaded,
so it popped in late and pushed the page below it down — render the
section heading + tile-shaped skeletons instead, matching the vitals
dashboard's loading pattern.
Priority-card actions (coach check-in keep/let-go) had no pending state,
so a slow round trip could be double-tapped into firing the mutation
twice; disable the action row while in flight. Priority-item dismiss is
now optimistic (removes the card from the rail on tap, rolls back on
error) instead of waiting on invalidate-after-success.
An invalid ?doc=<id> deep-link (a deleted document, a mistyped id) 404s
its detail fetch forever. The fenced send-gate never cleared, so every
message stayed blocked behind a phantom "untitled" attachment banner
that could never resolve — a bad link was a dead end instead of
degrading to a normal chat. Dropping a staged id the moment its detail
fetch settles into an error now clears the doc-scope, drops the banner,
and lets the coach send normally; a toast explains the link is gone.
Also gives the document vault processing feedback: the auto-index (and
optional AI summary) job that starts right after upload is
fire-and-forget server-side, so a freshly uploaded card sat with no
signal while it indexed. The vault list now polls while any recently
uploaded document is still unindexed (bounded to a few minutes so an
old, permanently-unindexed document never keeps it polling), and the
card shows a "Processing…"/"Ready" chip that clears on its own once the
index lands.
When the auto-AI-read opt-in is on, a freshly uploaded document is summarised
once in the background and the encrypted summary is stored and shown on the
document detail view.
- A background step runs after upload alongside the existing content-index and
thumbnail steps. It generates a short (3-4 sentence) plain-language summary
only when the user's documentsAutoAiRead opt-in is on (that flag is also the
standing AI-egress consent), skips a document that already has one, and
no-ops when no vision provider is configured. It reuses the same provider
resolution, egress-consent re-assertion, and AI-budget reserve/reconcile
helpers as the on-demand route; a provider error refunds the reservation and
never fails the upload.
- The summary is stored encrypted (summaryEncrypted Bytes) with a
summaryGeneratedAt timestamp on inbound_documents, registered for key
rotation, and returned decrypted in the document detail response.
- The on-demand transient summary route is unchanged.
Gated by the same consent, budget, and module checks.
"Ask the Coach" on a document used to navigate to /coach?doc=<id> — a full-page
jump away from the vault. It now opens the real, doc-scoped conversation in the
shared side drawer, and the drawer's maximize control expands it to the full
/coach page while preserving scope.
- Thread an optional documentId through the launch context: askCoach gains a
fourth argument, the provider tracks it alongside prefill/scope and clears it
on close, and LayoutCoachMount forwards it to the drawer as initialDocumentId.
The open-state precedence moves into a pure resolveLaunchState helper.
- The vault detail sheet closes itself and calls askCoach(..., doc.id) instead
of routing away, so the drawer owns the surface.
- Maximize preserves continuity: the drawer reads the live conversation id via
a new imperative getter out of the conversation surface (parallel to the
reset registration) and hands off to /coach?c=<id> once a thread exists,
falling back to /coach?doc=<id> for a chat maximized before its first turn.
The send path is untouched: every document turn still routes through the
hardened fenced endpoint keyed on documentId — only where the conversation
renders (drawer vs page) changed. The documents-chat e2e now asserts the
drawer opens in place, doc-scoped, rather than a URL change.
Asking the Coach about a document opened a separate, half-finished chat panel
that never appeared in Coach history. The bespoke drawer is gone; a document
conversation now opens the real Coach view, appears in the Coach list with a
document badge and a "Chatting about: <title>" note, and is read back through
the same history rail as any other thread.
The prompt-injection fence is unchanged and stays server-side. A document turn
is still sent through the hardened fenced endpoint (no tools, no health
snapshot, grounded in the document text only); the coach tool route continues
to fetch with documentId null, so a document-scoped conversation can never be
continued through the tool loop even from a hostile client. Only the surface
moved into the Coach; the boundary did not.
The relaxed conversation-list and detail filters drop only the documentId
predicate — userId scoping is unchanged, so a document thread is still visible
only to its owner. CoachConversationDTO gains documentId + documentTitle
(server-derived, title/filename only); no schema change (documentId exists
since 0230). OpenAPI regenerated.
v1.28.38 removed the purple AI-read glyph from the vault card (it looked
wrong there), but that glyph had been the only visible surface for the
'read by AI' provenance since v1.28.22 — the detail sheet's AI section
shows no status and collapses entirely under auto-read. Add a discreet
muted 'Read by AI' line under the document's uploaded-metadata in the
detail view (data-slot document-detail-ai-read), independent of the AI
section's collapse logic; reinstate documents.ai.statusAiRead in all six
locales. Card glyph stays removed. The e2e that asserted the card marker
now deep-links the document and asserts the detail-view line.
Document vault: card list drops a stray glyph and gives the real filename
its own line (was truncating on the meta row); the detail sheet's close
button aligns correctly — ResponsiveSheet reserved the right gutter for a
close button it wasn't rendering, now conditional on showCloseButton. The
open-in-coach action reads 'Coach fragen'.
Bulk share: a multi-selection in the overview shares several documents in
ONE link (documents-only privacy preserved, owner-scoped, up to 50 per
link; >50 refuses with a toast rather than silently truncating).
Reminders can link to WAIST_CIRCUMFERENCE (additive MeasurementReminderType
enum value, OpenAPI regenerated). The WHO-5 and SCI well-being reminder
labels were absent in all six locales and rendered their raw dotted keys
(the shared 'measurementReminders.types' prefix is what showed as the stray
'Measurement Reminders Types'); added in every language. A new
type-labels-i18n test asserts every dynamic reminder-type key resolves
non-empty in all six bundles — the static coverage guard cannot see the
dynamic key that let this through.
Google Health: the fullSync rollup-defer tracker collected its touched keys via
a single spread push — one argument PER READING, so a multi-year heart-rate
history (six figures of samples) overflowed the call stack and aborted the
metrics collection mid-cycle, exactly on the accounts with the most data. A
plain loop replaces the spread. Reported by a self-hoster; the v1.28.21 sync
diagnostics confirmed every other read on that account healthy in one log line.
Documents: the vault card's read-by-AI tag no longer takes its own line — a
small glyph rides inline on the date-and-size row. The detail view drops the
status bar under the preview entirely; with automatic reading on and nothing to
review the whole block collapses, so the content starts straight with the
document's fields. The provenance contract moves with it: the e2e specs now pin
the card marker instead of the sheet pill, and the unmounted status-pill
component is removed rather than left dead.
The detail sheet's AI block over-removed: the read-with-AI action and the
read-provenance pill are both contract surfaces (the action is the explicit read
path when auto-read is off / for an external provider; the pill is the read-state
indicator). Only the redundant block title + wand were the wasted row. Restore
the action and pill; drop just the title. The action row still collapses when
auto-read is on, as before.
Documents vault card: date + size and an AI-read marker now sit directly under
the title instead of the meta line running off under the thumbnail.
Document detail view: a larger close control replaces the small X and the
redundant top-right shortcut; the inline read-with-AI row under the preview is
gone (the suggest / summarise / show-text actions stay); a button to ask about
the document sits next to share and download.
Mental-wellbeing cards no longer wash the whole tile on hover — only the title
is the open target, matching the medication cards.
Medication API settings render inline instead of behind an expander that never
had anything beside it.
Render a small JPEG preview for each stored document and show it at the
card's leading edge, falling back to the kind icon when none exists.
- Generate previews in the background on upload (new document-thumbnail
pg-boss queue) plus a boot-time corpus backfill for existing documents;
upload never blocks on or fails because of it. Images are downscaled via
@napi-rs/canvas, a PDF's first page is rasterised then downscaled, every
other type is skipped. Generation never throws.
- Store the preview encrypted at rest in a 1:1 DocumentThumbnail side table
(migration 0236) so the list/detail queries that omit the original blob
never drag the preview either. Same AES-256-GCM string codec + fail-closed
decrypt as the content index; wired into the key-rotation walk.
- Serve it through an owner-scoped GET /api/documents/inbound/[id]/thumbnail
(auth + module gate + userId-scoped query + fail-closed decrypt), always
image/jpeg + nosniff + private, no-store; 404 on miss. The card renders an
authed lazy <img>, onError falls back to the icon.
- Add hasThumbnail to the document DTO so the card only fetches a preview
that exists.
Also drop the "search looks inside your documents" caption from the overview
(the index-all prompt stays) and the per-card searchable marker; both were
noise. Remove the now-unused i18n keys across all six locales.
Sharing a single document from the vault minted a full-record share: the
create form always defaulted `resourceTypes` to ["Patient","Observation"]
and never signalled a documents-only intent, so the served /c/<token> view
aggregated the whole doctor report — every vital, lab, medication and
wellness figure — alongside the attached file. An empty section blob was
indistinguishable from "defaults", so the report always resolved to the
full record.
Introduce an explicit documents-only share:
- add a `documentOnly` flag to the create payload; when set the server
forces an empty report scope (all-off sections, no FHIR resource types,
FHIR API off) and requires at least one document, so a document link can
never widen into a record — whatever the body sends.
- gate the clinician-view aggregation on an enabled section: a share with
no section on never calls the report aggregator, so no health metric
leaves the database. The recipient sees only the attached document(s).
- the document detail Share action opens the create form in documents-only
mode: hide the FHIR toggle, the FHIR resource-type checkboxes and the
history-range selector, keep the picker/label/expiry/passphrase, and
always carry the launched document into the created link.
- Settings sharing keeps the full record-share form unchanged.
Extract the create-payload builder as a pure function and cover the scope
contract, the empty-scope data load, and the served-view metric-free
guarantee with tests; refresh the share e2e and regenerate the OpenAPI
contract.
Replace the collapsed bottom-of-card "Chat about this document" outline
button with a neutral, ghost Coach icon pinned to the top-right of the
document detail-sheet header, mirroring the Insights ask-coach affordance.
The icon uses the muted-foreground token only — never the primary/FAB
gradient — and opens a document-scoped chat drawer.
Present the chat in the Coach chat drawer chrome (right-side sheet on
desktop, bottom sheet on phones; same width caps; close-only, no maximize
or conversations rail) with a quiet "Chatting about: <document>" scope
badge. The drawer reuses the existing pure conversation body and the
existing client hooks, so it still posts /api/documents/inbound/[id]/chat
verbatim: no tools, no health snapshot, fenced document, inbound/replay
refusal, outbound screen, numeric grounding, consent + budget + per-user
rate bucket, indexed-only, plain-text render — the route is untouched.
Thread isolation is unchanged (the general Coach filters documentId: null;
the document route always scopes { documentId: id }).
Add a small reusable headerAction slot to ResponsiveSheet for the header
icon. Retire the DocumentChatPanel container and the AI-section chatSlot;
keep the pure DocumentChatConversation body and documentChatErrorKey.

When "read documents automatically with AI" is on, the detail sheet no longer
shows the manual per-document action row (Read / Suggest / Summarise) — reading
already happens on upload, so the buttons only nagged. The section header and
the searchable status pill stay, and content search keeps working. With the
setting off the manual controls render exactly as before.
Drop the "review before saving" heading and its grey sub-caption from the
suggestion panel; the reviewable fields and the apply / dismiss controls stay.
Remove the now-unused documents.assist.reviewTitle / reviewHint keys from every
locale.
The bundled (Turbopack) copy of pdfjs-dist failed its render path in the
standalone image with a bare Error, so a scanned PDF on a non-Anthropic
provider fell back to 'PDF scanning needs a Claude vision provider' — while the
identical un-bundled module rasterizes the same document fine (verified in the
container end to end on the real document). Externalize pdfjs-dist and
@napi-rs/canvas so the server loads the real modules, and hoist pdfjs-dist to
the top level in the image so the runtime import resolves. Also log the render
error message (not just its name) and drop the redundant 'not saved' note from
the document summary panel.
Remove the two per-document AI copy blocks in the document detail sheet:
the external-egress notice and the "read this document" explainer. The
settings-toggle honesty confirm already states the egress trade, so the
per-document repeat is redundant. Drop the now-unused `egressExternal`
prop and the `documents.ai.egressNotice` + `documents.ai.available` keys
across all six locales. Keep the capability `egress` DTO field — the API
still reports it.
On /insights/recovery, the metric block rendered an outer heading plus an
explainer above a tile whose own header repeated the same title. Move the
explainer inside the tile as a `description` beneath its `TileHeader` and
promote the tile title to a real `<h2>` (new `titleAs` affordance), so each
metric shows one heading with the blurb under it and the h1->h2 outline
stays nested. Applies to every metric the block renders.
Add an e2e suite mocking the capability probe, the chat history GET, and the
streaming chat POST so the panel renders (v1.27.31 lesson): the entry opens an
empty safety-noted surface, a turn streams a reply that settles to one assistant
bubble, a non-indexed document shows the calm hint, and no provider leaves no
chat surface. Clear the streamed tail + optimistic bubble once the persisted
twin lands so the reply never double-renders.
Extract DocumentChatConversation (pure) from the chat panel so the open
conversation surface is statically renderable, and pin: composer + always-on
safety note, plain-text/XSS-safe assistant render (raw HTML escaped), the
streaming tail vs thinking placeholder, mapped error keys, and the container's
indexed vs not-indexed gating. Assert the AI section renders the chat slot only
in the provider-available branch.
