Surface actual workout sessions and a per-analyte lab trend in the doctor
report PDF + FHIR bundle (gated on their modules), carry the user's stored
conditions/allergies through to the report and FHIR Condition/AllergyIntolerance,
and round medication adherence to the app's integer convention so the PDF, FHIR
and in-app numbers agree.
The server already computes the clinical glucose panel (TIR / GMI / estimated
A1C / mean / CV%) by the one literature-locked engine the insights panel and
coach consume, but nothing surfaced it in the export. Render it as a dedicated
PDF table after the vitals section and emit it as FHIR Observations
(LOINC-coded where a term exists; CV% under a survey text-only concept). Both
gate on the panel carrying readings, which the aggregator already zeroes when
the glucose module is off — so a disabled module emits no glucose panel. The
spot-reading caveat rides along when the readings are sparse. Adds clinical
glucose-panel keys across all locales and PDF + FHIR tests.
The per-type avg/min/max blended every ingest source, so a metric carried
by two overlapping sources on the same day (WHOOP + Apple Watch resting
heart rate) averaged both readings while the dashboard and rollups already
collapse to the ladder-canonical source per day. Route the report's raw
measurements through the same picker the analytics aggregator uses
(pickCanonicalSourceRows, keyed by metricKeyForType + the user day key +
source-priority ladder) before grouping, so the PDF and FHIR numbers match
the dashboard. SLEEP_DURATION stays untouched — its per-stage rows are
resolved by the per-night reconstruction downstream. Adds a parity test.
The dashboard's recent-workouts and steps tiles painted regardless of
the workouts module state; AND the resolved per-account flag into both
gates (fail-open — only an explicit false hides the tile).
Extend the snapshot module maps to cover the recovery domain: the
cardioRecovery / sixMinuteWalk / stairAscentSpeed / stairDescentSpeed
widgets and their HealthKit measurement types now resolve to the
recovery module, and the per-night breathing-disturbance widget + type
resolve to sleep. A new guard test asserts every toggleable-domain
widget id carries a mapping and every mapped value is a real module
key, so a future device widget cannot leak onto a disabled account.
Gate badge evaluation on the per-user module map: mood badges no longer
evaluate, render, or persist while the mood module is off, and the same
holds for any metric that maps to a toggleable module (sleep). Core and
account-wide badges have no owning module and always pass through.
Move unlock persistence out of the cached builder. The builder now
returns the not-yet-persisted unlocks and the handler runs the
idempotent createMany after the cache read, so a cache hit can no longer
skip the write and a concurrent miss can no longer duplicate it.
Add the parity fields the iOS DTO dropped (category, points, target,
current, isHidden) so the native client groups and renders badges in
lock-step with the web surface.
The cycle gate checked only the per-user toggle and gender, never the
operator server-wide availability layer. With the Cycle module turned off
by the operator, /api/cycle/* kept serving, the coach injected the cycle
block, the Apple-Health importer kept writing cycle rows, and the
cycle-reminder cron kept firing CYCLE_PERIOD pushes — while nav and the
doctor report already honoured the operator switch.
Add isCycleAvailableForUser, which delegates to isModuleEnabled(userId,
"cycle"): the cycle ModuleKey resolves the per-user toggle AND the
operator availability map, so there is one source of truth and no
duplicated operator logic. Route requireCycleEnabled, the coach cycle
block, and the import accumulator through it; switch the cron to the
injectable isModuleEnabled gate, matching the mood and measurement
reminder crons, and add a skippedModuleDisabled counter. Correct the
cron comment that claimed isCycleEnabled was itself the module gate.
isCycleEnabled stays the pure per-user (user-layer) resolver the module
foundation AND-s at layer one.
Tests: assert requireCycleEnabled returns a 403 cycle.disabled envelope
and the cron skips the push when the operator turns the module off.
The per-day compliance series in the medication-compliance insight snapshot
divided every event-day by the schedule count, treating each calendar day as
fully dose-due. A weekly-cadence medication (a Monday-only schedule, a
bi-weekly interval) therefore had its series read off-cadence weekdays as
misses, contradicting the canonical compliance30 value shown in the same
snapshot.
Route the per-day series through the unified dose-history ledger that already
backs compliance7 / compliance30: add dailyComplianceRatesFromLedger, which
groups the ledger's scheduled slot rows by day and scores each day with the
same taken / (taken + missed) tally — on-time-plus-late numerator, skip /
ad-hoc / upcoming exclusions — that tallyLedgerRows applies to the window
rates. Only days the cadence actually expected a dose produce a point, so a
non-due day can no longer be counted as a miss, and the per-day series and
compliance30 are computed from one ledger and cannot disagree.
The insight generator now derives both the canonical compliance results and
the ledger from a single buildMedicationComplianceBundle pass instead of two
calculateCompliance calls plus a hand-rolled per-day map.

Reference-range classification + low/high range rendering was implemented three
times (validations, PDF core, lab list). Extract a single classifier and a
structure-only range formatter that takes a per-surface number formatter, so
each surface keeps its own number formatting while sharing the canonical
classification. Behaviour is unchanged per surface and now covered by one test.
The sleep-debt nudge compared a minutes value against an hours threshold, so
the trigger was effectively dead. Align the comparison to the unit the
sleep-debt value is produced in and pin it with a firing-case test.
The Module hub rendered live switches for the delegated keys cycle and coach,
but the gate resolves those from their own sources (cycle from CycleProfile,
coach from the operator flag + disableCoach), so a toggle wrote an inert entry
to modulePreferencesJson, snapped back, and still fired a success toast. Render
delegated modules as read-only rows that deep-link to their real control, strip
delegated keys from the modules PATCH schema so no inert value is stored, drop
the duplicate cycle control, and surface an operator-disabled state with the
previously orphaned hint string.
The Personal Health Score weighted the 0.20 mood-stability pillar
unconditionally — the fast-path read mood entries regardless of the
account's module state, so a user who turned the mood module off was
still scored (and silently penalised or rewarded) by a pillar hidden
everywhere else in the product. The dashboard already blanks the mood
tile and the coach snapshot unions mood out; the score ring and the
insights card pillar list did not.
Thread a default-on `moodEnabled` flag through the fast-path. When the
caller resolves the mood module as disabled the helper skips the mood
read entirely and leaves the series empty, so `moodStability` returns
null and the existing proportional null-redistribution re-scales the
surviving pillars to sum to 100. The analytics route resolves the
module server-side and threads the flag; the insights card hides the
Mood row via a new SSR-safe `useModuleEnabled` accessor so it never
advertises a pillar the account turned off.
Golden fixtures pin mood-on vs mood-off to different, correctly
re-weighted scores and assert the disabled read is never issued.

Add the resolved `modules` map to ProfileResponse (shared ModuleMap
schema) so the spec matches the alias payload, and add a reusable
module-disabled 403 response documenting `meta.errorCode = module.disabled`
plus `meta.module` (the disabled key). Wire that 403 onto the module-gated
sleep night/rhythm routes, which emit it via requireModuleEnabled.
Regenerate docs/api/openapi.yaml; openapi:check is in sync.
The per-user disableCoach gate moved out of the AI/Assistent settings section
into its own dedicated Coach section in v1.18.0. Update the call-site allowlist
so the grep-based invariant pins the new home instead of the old one.
Add an operator-side toggle matrix to the admin assistant section that
flips each toggleable module on or off server-wide via PATCH
/api/admin/settings/module-availability. Mirrors the assistant-flags
matrix: one neutral toggle per module, optimistic flip with a revert on
failure, and an authMe invalidation so the resolved module map re-gates
the session immediately. New admin.modules.* labels across all six
locales; core domains never appear.
Second layer of the module enable/disable model: an operator can turn a
module off for the whole server, and an operator-disabled module is off
for every account regardless of personal preference — mirroring the
coach master flag above the per-user opt-out.
Storage is a DISABLED allowlist on AppSettings.moduleAvailabilityJson
(absent/empty/true = available, explicit false = disabled), resolved by
getOperatorModuleAvailability(). The gate now resolves every toggleable
module as operatorAvailable(key) AND userEnabled(key); cycle and coach
keep their existing delegation but are still AND-ed with operator
availability. Core domains are never disableable.
GET/PATCH /api/admin/settings/module-availability is cookie-only
(requireAdmin); a strict per-module-key schema rejects core domains and
unknown keys at the boundary.
Surface a dedicated settings section for enabling and disabling the
secondary tracking domains (mood, sleep, glucose, workouts, recovery,
labs, achievements, coach, insights, doctor report, cycle). Each row
carries an icon, label, and one-line description with a switch bound to
the resolved module map on /auth/me; toggling PATCHes /api/auth/me/modules
and invalidates the auth/me query so the navigation, Insights pills, and
dashboard tiles re-gate live. Core domains (weight, blood pressure, pulse,
medications) render as a read-only always-on group. The entry sits right
below Account in the settings navigation.
Generalise the per-destination cycle gate into a requiresModule marker
on each nav destination and read it from the account's resolved module
map (the modules field on GET /api/auth/me). Fold the bespoke cycle
boolean into requiresModule: "cycle" so there is one gating mechanism;
cycle and coach are delegated keys already reflected in the map, so
reading them here is not a re-derivation.
Tag mood, cycle, labs, coach and achievements with their module keys;
core destinations (measurements, medications, vorsorge, insights, the
dashboard) carry none and always render. The sidebar, the mobile More
hub, and the Insights tab strip all filter on the same map.
Insights pills: a metric pill belonging to a toggleable module (mood,
sleep, glucose, workouts) hides when its module is disabled, on top of
the existing data + saved-layout gates; the Recovery pill gates on the
recovery module. Overview and core-metric pills always show. Every gate
fails open — a missing key or an absent map keeps the entry, matching
the gate's default-on contract, so a stale /me payload never blanks the
nav.
Resolve the per-user module map at report-build start and drop a disabled
module's section/resources from the aggregated payload, so the PDF and the
FHIR bundle reflect only the modules the user keeps. The module gate wins
over the per-report section toggle: a module turned off cannot be shared
even if its report toggle is on.
Map report sections / FHIR resource groups to module keys: mood, sleep,
cycle, labs via the existing section toggles; glucose panel + raw glucose
series; recovery + stress wellness scores; workout activity series + strain
score. Core clinical sections (weight, BP, pulse, medications) carry no
module key and stay unconditional. FHIR resources inherit the exclusion
because every builder derives from the gated aggregator output.
Resolve the per-user module map once at snapshot build start and fold any
disabled data-domain module into the same system-side source exclusion the
excludeMetrics flow already drives, so a disabled domain (mood, sleep, glucose,
workouts) never reaches the coach context. The recovery composites are gated
explicitly as well, since their derivedActive flag also reads the sleep signal
and would otherwise survive when only recovery is off. Cycle keeps resolving
through its single-source-of-truth gate. Tests stub resolveModuleMap to the
all-on default so existing fixtures are unaffected.
Gate each per-module reminder cron on the v1.18.0 module foundation so a
disabled module never dispatches its reminder.
- mood-reminder: skip a user whose mood module is off, after the window
gate, with a mood_reminder.skipped_module_disabled annotation.
- measurement-reminder: skip a glucose/sleep reminder when that module is
off and advance nextDueAt past the slot so it does not re-evaluate every
tick; core-vital reminders (weight/BP/pulse/body-comp) and free-text
reminders map to no module and dispatch regardless.
- cycle-reminder: already gates on isCycleEnabled, the single source the
cycle module key delegates to; documented, no behaviour change.
Medication reminders stay ungated (medications is core). The gate is
injectable into each tick so the unit tests pin the disabled-module skip
without the gate's DB reads.
Resolve the per-user module map in the dashboard snapshot builder and
strip every toggleable module's tile + data before the payload leaves
the server, so a disabled module never reaches the client.
- mood / sleep / glucose / achievements / workouts widgets are forced
invisible on both the web layout and the iOS catalogue when their
module is off; the persisted layout is untouched.
- sleep / glucose summary types drop out of the slim slice, so the
derived metricStates and the client data-floor gates also drop them.
- the glucose clinical panel and per-context block are cleared when
glucose is off; the mood block is emptied when mood is off.
- the Daily Briefing falls back to its disabled state when the insights
module is off (alongside the existing operator flag + coach opt-out);
raw vitals stay untouched — insights is the narrative layer.
Core widgets (weight, BP, pulse, body-fat, medications, BP-in-target)
carry no module mapping and always render.
Register the four /api/insights/chat surfaces in the OpenAPI contract:
list (cursor-paginated), streaming send (SSE), detail (messages decrypted
server-side), and delete. Adds CoachConversation / CoachConversationsPage /
CoachConversationDetail / CoachMessage / CoachProvenance / CoachChatRequest
schemas and regenerates docs/api/openapi.yaml. Additive; the endpoints
themselves are unchanged.
The Coach lived under /insights/coach, rendering inside the Insights
layout's tab strip. It is the product's headline surface, not a metric
sub-page, so it now lives at the top-level /coach in the standard page
chrome without the Insights tab strip. The nav model points at /coach,
every in-app link and the proactive-nudge deep link follow, and a 301
redirect keeps /insights/coach (bookmarks, cached PWA navigation, older
push payloads) resolving.
GET /api/auth/me now returns a resolved modules map for every toggleable
module (cycle/coach reflect their delegated state). New PATCH/GET
/api/auth/me/modules read and update the preferences: the body is strict
over the toggleable key set so a core-domain or unknown key is a 422, the
merge is field-by-field (no mass assignment), and userId is narrowed from
the session. Documented in the OpenAPI registry and yaml.
The disable-coach gate-inventory allowlist records the module gate's
intentional coach delegation read.
registry.ts is the single source of truth for the toggleable secondary
domains (cycle, mood, sleep, glucose, workouts, recovery, labs,
achievements, coach, insights, doctorReport) plus the always-on core
domain denylist. Each entry carries a stable key, label/description i18n
keys, a category, and an optional delegatesTo marker.
gate.ts is the one enforcement point: isModuleEnabled / requireModuleEnabled
(403 module.disabled envelope) plus resolveModuleMap for the /api/auth/me
projection. No double source of truth — cycle delegates to isCycleEnabled
and coach delegates to disableCoach AND the operator assistant master
flag; every other module resolves from the disabled allowlist. Default-on,
fail-open per key, per-request memoised reads.
insights gates only the AI-analysis surfaces (briefing, status cards,
correlation narration, score explainer) — never the raw measurement
charts, which are core and render regardless.
With measuredAt now stamped at the segment END, a night that Withings
re-aggregates between syncs shifts its enddate, so the measuredAt-keyed
lookup missed the prior row and the create then collided with the unique
externalId — leaving the night stuck at its stale value. Key the lookup on
the stable externalId and refresh value, measuredAt and stage on update.
Caught by the withings-sleep-sync integration test against real Postgres.
Oura and Polar recovery rows are bucketed on the local day of their raw
measuredAt, on the assumption that both stamp the wake morning. Polar
always anchors at UTC midnight of the wake date and Oura falls back to the
same anchor when no real wake instant is present, so for a user in a far
negative UTC zone that anchor reads as the previous local evening and a
single night can split across two recovery days near the date line. A
correct fix needs an anchor-kind signal threaded through the recovery
read so the date-anchored sources read their wake day in UTC while a real
wake instant stays local — out of scope here. Document the assumption at
the bucketing site; positive-UTC users are unaffected.
The webhook shared secret lives only inside the AES-GCM-encrypted channel
config blob and the GET masks it, so no current path excerpts it into a
wide event. Add headervalue to the sensitive-key denylist as defence in
depth: if a future change ever routes a webhook body through the payload
diagnostic, the secret redacts instead of landing verbatim. The Live
Activity push token is already covered by the token pattern.
The doctor-report PDF computed SLEEP_DURATION min/avg/max over the raw
per-stage rows (a 40-minute DEEP block, a 12-minute AWAKE block), so its
sleep figure was per-stage-row minutes while every other surface — the
dashboard slim slice, /insights/sleep, /api/sleep/night, the iOS feed —
shows the reconstructed per-night asleep total. The sleep-stamping work
rewrites exactly those raw rows, so the report could drift independently.
Route the report sleep value through the night reconstruction engine, the
same way the recovery score routes through the canonical recovery resolver
— one number, every surface. The user's source priority feeds the
reconstruction so the multi-source de-dup matches the dashboard.
The measurement-reminder tick loaded every enabled reminder across all
tenants every 15 minutes and gated due-ness in Node, so the shipped
measurement_reminders_user_id_next_due_at_idx went unused. Add a
nextDueAt bound (not null, lte now plus one tick of slack) to the query
so Postgres scans only rows that could plausibly fire. nextDueAt is
stamped at the notify-hour boundary, so anything due is already past;
non-recurring reminders carry a null nextDueAt and can never fire, which
the in-Node predicate already short-circuits. A reminder satisfied early
re-anchors once it crosses due, before any nudge, so dropping future rows
is safe. The in-Node hour gate stays the authoritative fire decision.
The email sender built its nodemailer transport without connection,
greeting, or socket timeouts, so nodemailer's defaults applied — up to a
10-minute socket timeout. A mail server that accepts the TCP connection
then stalls (firewalled relay, overloaded Postfix, blackholed smarthost)
could hold a dispatch worker for minutes per notification, while every
other egress in the cascade is bounded. Pin connection/greeting at 10s
and socket at 20s so a stall maps to the transient send outcome the SMTP
classifier already retries, matching the webhook sender's ethos.
Polar exposes only per-stage duration totals, so its mapper already
reconstructs a contiguous CORE-DEEP-REM order exactly like WHOOP and flags
each segment reconstructed. The night reader's honesty allow-list only held
WHOOP, so a Polar-won night was served with reconstructed:false — presenting
a synthetic stage order to web and iOS as if it were the device's measured
timing. Add POLAR to the allow-list so the layout is labelled approximate.
Polar's total_interruption_duration carried the night's awake time but was
dropped, so every Polar night read as fully consolidated (awakeMinutes:null,
awakenings:0) with overstated asleep-vs-in-bed efficiency. Lay it as a
leading AWAKE segment so the asleep stages partition the remaining window and
the reader surfaces real awake time.
WHOOP and Polar independently implemented the same contiguous-timeline walk.
Extract a shared reconstructContiguousSleepTimeline helper under src/lib/sleep
and call it from both; each mapper keeps only its field normalisation. Oura's
measured 5-min hypnogram is a different algorithm and is left untouched.
The annual-blood-panel auto-resolve that findLatestLabResultForAnalytes was
meant to feed never got wired into the Vorsorge reminder, so the export sits
with zero callers and the labAnalyteTrend query-key has no consumer (the
sparkline is prop-fed and there is no labs trend endpoint). Remove both so the
knip gate is clean; the Vorsorge lab read-back can come back with the wiring
that needs it.
Surface the webhook + email cards on the notifications settings page, and
add an operator-wide delivery-health view the per-self diagnostic could
not give. A single groupBy(channel, result) over the trailing 24h of
push_attempts lets the operator answer "are anyone's pushes failing?"
across all users without DB shell, and a count of auto-disabled channels
(enabled=false with an automated disabledReason) flags a flapping upstream
that silently disabled a channel after five failures. The aggregate is
anonymous — it counts channel x result, never names a user. Admin-only by
construction (cookie auth, never Bearer); the window is clamped 1..168h.
The panel rides the existing admin Reminders section.
Alerts and reminders over email — the one channel every self-hoster
already has. The SMTP transport (host/port/auth/from) is operator-supplied
via SMTP_* env and loaded once per process like the APNs key; the only
per-user value is the recipient address, so SMTP credentials never enter a
user's encrypted channel blob. Absent SMTP env disables the channel
cleanly: the dispatcher skips it and the Settings card hides itself.
Bodies are plain text — HTML is stripped and routine reminders run through
the emoji-strip path; no markdown library is added. SMTP 5xx maps to a
hard reject (permanent — auto-disable); connection/4xx errors are
transient and ride the backoff schedule. Adds nodemailer (the standard,
well-audited mail transport) and its types.
A single WEBHOOK channel covers Gotify, Discord, Slack, Matrix, and Home
Assistant: the user supplies a public URL and an optional shared-secret
header, and the dispatcher POSTs a small plain-text JSON envelope. The
outbound goes through safeFetch with requirePublicHost so the SSRF floor
and the connect-time DNS-rebinding pin apply to the user-supplied host,
exactly as they do for ntfy. Responses classify 404/410/401/403 as hard
rejects (auto-disable) and 5xx/429/timeout as transient (backoff).
Settings card mirrors the ntfy card with a save + test-connection flow;
the header value is never returned by the GET and is preserved across a
save round-trip when left blank.
Add WEBHOOK and EMAIL to the channel set, the dispatcher cascade, the
per-event default policy, the push-attempt ledger union, and the admin
diagnostic config-presence check. Channels are string-typed, so no
migration is needed. The dispatcher routes each new type through its
sender with the same decrypt + parse + classify path the existing
channels use; the cascade orders webhook + email between the self-host
relays and the Web Push fallback.
Add the webhook + email channel config shapes and their settings-form
validation schemas, plus the query-key entries for the two new config
reads.
