Found by an adversarial pass over the release candidate before it was tagged,
and it is the reason the tag waited.
`POST /api/medications/intake` is admitted for delegates because a caregiver
standing next to the patient has to be able to say the tablet went down. It is
an update, though, and the same body can carry two other things. Snoozing
writes `snoozedUntil` on the OWNER's medication row, and the reminder cron
skips that medication for as long as the stamp is in the future. The field
takes any future instant. So somebody with write access could stop another
person's medication reminders for years, and three things made it quiet: the
owner is deliberately not notified of a delegated snooze, the activity feed
renders the whole family as "marked a dose", and the only accurate trace was a
line in the admin audit log.
The second was flipping a dose the owner had already recorded — taken to
skipped and back, which rewrites their compliance history and refunds
inventory. The consent screen promises that changing what is already there
stays with the owner.
Both refuse for a delegate now and stay open to the owner, who is the only
person either was ever for. A refused attempt is filed under the owner and gets
its own sentence in the activity panel rather than borrowing the one for
marking a dose, because folding them together is what made the gap dangerous.
Two more from the same pass:
The consent notice named allergies among the things a delegate may add. That
left the admitted set before this release shipped, and it said so in six
languages on the one screen where somebody decides who may write into their
health record. It also omitted half of what IS admitted. The copy now matches
the frozen list, and the test asserts the withdrawal rather than only the
presence of the rest.
The banner told a WRITE delegate nothing at all — the mode line rendered only
in the read case, guarded by a comment stating that write grants did not exist
yet. This is the release that made them exist. It matters that the banner say
it, because the rule that a refused control is absent rather than disabled
rests on the banner naming the mode globally.
And a third instance of the substitution this release already fixed twice: the
BMI chart divided the owner's weights by the height of whoever was looking.
Inside a shared record it draws nothing now rather than a number belonging to
two different people.
The intake routes call notifyDelegatedIntake with both account ids and
the helper decides. Keeping the comparison here rather than at the call
sites means a route added later cannot notify somebody about a dose they
marked themselves, and a snooze is answered in one place as what it is:
moving a dose, not resolving one.
The medication name is read scoped to the owner, because that name goes
onto the owner's lock screen. Nothing addresses the delegate: the
dispatcher takes a single account id, and under record substitution that
id is the owner. The person who marked the dose learns what they did
from the response they are already awaiting.
The integration test asserts the ledger row before the sender mock, so a
dispatch nobody recorded cannot pass as a dispatch.
The matrix derives each row's label from the event name, so a new event
type gets a row whether or not anybody wrote the strings for it. That is
how SECURITY_ALERT has been rendering its own raw key since v1.23: the
forward i18n coverage test only sees literal call sites, and this key is
assembled from a runtime value.
Move the derivation out of the page so a test can compute the keys the
page computes, assert both resolve in all six bundles, and name the
missing one. Write the sign-in row's strings, and the strings for the
delegated-dose row and its push body.
The owner of a shared record gets an alert when a delegate marks one of
their doses. It is the one exception to the rule that bookkeeping stays
in the activity view: whether the tablets were taken is a fact about the
owner's own body, and they should not have to go looking for it.
Default ON, against the opt-in precedent of PERSONAL_RECORD, because the
event only exists once the owner has invited somebody and given them
write access. The per-channel row in the matrix is the valve for anyone
who finds it noisy, and both positions of that preference are covered.
`apns-collapse-id` is a replace directive: a push carrying an id
supersedes the earlier undelivered push with the same id. Every
medication reminder carried the literal string MEDICATION_REMINDER, so
three medications due at 08:00 arrived on the phone as one notification
naming one of them. The user takes that one and misses the others with
nothing on screen to suggest anything was dropped.
Key the collapse on the per-slot tag the Web Push sender already
coalesces on, falling back to the event type when a payload carries none.
Grouping stays on `threadId`, which is what grouping is for. Discreet
mode keeps the generic id — no record-identifying string on the lock
screen — and a tag over Apple's 64-byte ceiling falls back rather than
costing the send.
The per-phase "already sent today" guard read the Telegram message
ledger, and that table is written only when a Telegram send succeeds. A
user on email, APNs or ntfy never had a row, so the 15-minute tick
re-derived the same overdue slot and sent it again on every pass — around
fifty mails for a single missed dose.
Anchor the guard in `push_attempts` instead, the ledger the daily
briefing and the illness escalation already use. Two properties are
deliberate: the anchor is stamped for every dispatched slot regardless of
what any channel returned, because keying it on a delivered push would
re-open the flood the moment a channel starts failing; and it is stamped
before the dispatch, so a crash costs one phase's notice rather than a
repeat every quarter hour.
The key carries the schedule id and the slot's real HH:mm alongside the
phase and the local date. Both are load-bearing: two legacy
single-window schedules on one medication share the empty first-class
time-of-day, so a coarser key would collapse the evening dose into the
morning one. The tests assert the other direction too — the next phase
still escalates, and the next local day starts clean.
The Telegram ledger keeps its message-tracking duties; it is no longer
the dedup key. Anchor rows carry a sentinel channel and stay out of the
operator's notification health and diagnostic reads, which count
deliveries, not decisions.
`medication.clientManaged` and `measurementReminder.clientManaged` mean
the iOS client schedules the reminder locally, so the server should not
push it. That is what the OpenAPI description, the settings copy and the
iOS contract all say. What the crons did was skip the dispatch entirely,
so a user who flipped the switch also lost Telegram, ntfy, webhook,
email and Web Push — channels they may well read on a desktop while the
phone holding the local schedule sits in a drawer.
Move the gate into the dispatcher's channel loop, where it applies to
APNs and nothing else. The skip is not counted as an attempt, so
`dispatched` keeps meaning "something reached the user", and it stays
countable: the same wide-event keys the crons emitted are emitted here,
plus a `skipped` row in the push-attempt ledger.
The Vorsorge path is the sharper half. It suppressed every channel and
then advanced `nextDueAt`, which dropped the reminder out of the
preventive-care card and out of the daily digest: a reminder that never
reached anyone was recorded as handled. The advance now happens only
where it always belonged, after a dispatch that a channel actually
delivered. When nothing delivers, the reminder stays overdue and visible.
The push endpoint host is user-controlled — the browser supplies it at
subscribe time. `isPublicUrl` guards it at subscribe time and again before each
dial, but it is a literal input-time check and cannot see a DNS rebind between
the accept and the connect; with a short-TTL record the dial reaches the pod's
private network.
`web-push`'s `sendNotification` dials with its own internal `https.request`,
which bypassed `safeFetch` and therefore the connect-time pin entirely. Both
senders now sign and encrypt through `generateRequestDetails` — identical VAPID
handling and payload encryption — and dial through `safeFetch` with
`requirePublicHost`. Non-2xx is re-shaped as the `{ statusCode }` rejection the
existing classification already understands, so the 410/404 expiry reap and the
transient/permanent split are unchanged.
This is the last user-controlled-host egress that was not behind the pinned
dispatcher (issue #217).
Add a once-daily briefing notification that reads the cached daily digest
and never makes a fresh model call. It rides the existing dispatch cascade
(APNs, Telegram, ntfy, webhook, email, web push), so it inherits the
hard-reject classification and the push-attempts ledger unchanged.
- New DAILY_BRIEFING event type, default off: the briefing stays silent
until the user opts a channel in from the notifications matrix, so the
channel choice is explicit and a fresh account never gets an unasked-for
push.
- One decision seam both triggers share: the sleep-arrival morning refresh
fires it the moment the day finalises, and a fixed local-morning fallback
slot covers the case where last night's sleep never synced. Whichever
wins the morning writes the ledger row; the once-per-local-day cap
suppresses the other.
- The body is the digest line — the cached briefing lead when present, else
the deterministic floor — so a keyless self-hoster still gets a
first-class push. Provisional days carry an honest "sleep not in yet"
note.
- Non-clinical and non-urgent by construction, fired only inside a local
morning window: it is a nudge, never the anomaly channel, and never
bypasses Focus. Empty accounts are left alone rather than nudged with a
hollow all-clear.
- Localised across all six locales.
The MEDICATION_INTAKE_SYNC fan-out used to fire one silent APNs per
distinct dose slot, so a bulk intake marking five doses woke every other
device five times. Replace the per-slot dispatch with a per-user
coalescer: the first mutation in a burst dispatches immediately, every
further mutation inside the 2 s window folds into one trailing fan-out.
A bulk request now costs exactly one queue call, and a rapid burst of N
single-dose mutations costs at most two fan-outs. The window state is
in-process; multiple app instances throttle independently, which is
accepted because the iOS reconcile is idempotent and the deployment
model is a single container.
The silent payload shrinks to a pure sync trigger — eventType +
syncedAt. The per-slot medicationId / scheduledFor fields are gone: the
iOS handler runs a full reconcile regardless, a coalesced burst has no
single slot to name, and a push payload travels through Apple's
infrastructure, so it now carries no health data by construction. The
Live Activity end push likewise sends only the ActivityKit envelope.
The trailing flush keeps the origin skip only when every coalesced
mutation came from the same device; a mixed-origin burst wakes everyone.
Each flush still records one push_attempts ledger row, reaps
hard-rejected device tokens, and annotates the wide event with the
recipient count and coalesced-mutation count.
The requirePublicHost egress paths now dispatch through undici's own
fetch, so the existing global fetch stubs in the AI, GeoIP, moodlog,
send, and ntfy suites no longer intercept them. Delegate undici's fetch
back to the installed global stub so each suite keeps asserting its
request shape and canned responses without exercising the network.
On a completed sign-in (password, second-factor, or passkey) from a device or
location not seen before, send one notification through the dispatcher cascade,
honouring the user's per-channel preferences via a new SECURITY_ALERT event
type. Dedupe on a one-way, per-user fingerprint: a SHA-256 over the userId, the
coarse User-Agent family, and a country/ASN-level geo signal, stored in the new
user_known_devices ledger (migration 0203). The first sighting of a fingerprint
alerts and records; later sightings only bump last-seen and stay silent. The
raw User-Agent and IP are never persisted — only the salted hash and a coarse
label. Registration records its device silently so a fresh account does not
alert on its own first session. The whole path is fire-and-forget and never
blocks or fails a sign-in.
A channel whose stored config cannot be decrypted or JSON-parsed is a
structurally-permanent failure, not a transient blip: the ciphertext/key
pairing and the malformed body decode the same way on every retry. Routing
it through the transient path burned the 5-failure backoff budget and ended
on the misleading `give_up_after_5_failures` reason — during a key-rotation
gap every channel hit this and auto-disabled for the wrong reason.
Classify decrypt and parse failures as hard rejects so the channel
auto-disables on first occurrence with the precise reason
(`<type>_config_decrypt_failed` / `<type>_config_parse_failed`). Genuine
transient failures (network, timeout, 5xx) keep the existing backoff path.
Extend the self-cleaning Telegram bot beyond medication. A mood reminder
now carries a 1-5 inline keyboard plus a "Note..." force-reply and a
"Later" action; a tap logs a MoodEntry (source TELEGRAM, server-side,
user resolved from the linked chat, idempotent on a per-tap externalId)
and confirms with a calm callback toast, no chat echo. An optional
free-text note attaches to that entry via the prompt-context correlation.
A measurement reminder carries "Done" (satisfies the cadence) and
"Later" (pushes nextDueAt forward) — completion and snooze only, no value
capture.
Prompts and replies self-clean in ~30 minutes: the window drops from 1h
and moves to a shared helper, the sender schedules an unanswered prompt's
deletion at send time, and the existing 15-min sweep also prunes expired
prompt-context rows. The webhook keeps the shared-secret + timingSafeEqual
+ rate-limit-before-secret posture; every write callback authenticates via
the stored chat binding. The Telegram chat id is added to the egress
redaction denylist.
One-shot prettier pass so the new format:check gate starts from a clean
baseline. Formatting only — no behaviour change. Generator-owned and
vendored artifacts stay untouched via .prettierignore.
The measurement-reminder push already carries the reminder id in its
metadata, but the iOS allowlist stripped it before it reached the device.
Forward the key so a foreground or handled notification can post the
completion directly instead of routing through the capture deep link.
Self-hosters without an Apple Developer account rely on Web Push to the
installed PWA — there is no Live Activity or lock-screen widget to drive.
Make that path carry the dose lifecycle on its own:
- Stable per-slot tag. The medication dose-due reminder now sets a
`med:<id>:<iso-slot>` Web Push tag (medicationDoseTag), so a re-fired
reminder for the same slot coalesces instead of stacking, and a later
clear can target it.
- Clear-on-taken. When a dose is marked taken or skipped, push a
`type:"clear"` notification reusing that slot tag. The service worker
closes the matching pending reminder and refreshes the badge — the PWA
equivalent of ending a Live Activity. A snooze leaves the dose
outstanding and clears nothing. Wired into the single + bulk intake
routes alongside the existing APNs cross-device sync.
- App badge. The dispatcher carries a server-authoritative outstanding-
dose count; the SW reflects it via the Badging API
(setAppBadge/clearAppBadge), feature-detected and a no-op where
unsupported.
- The SW push handler honours requireInteraction for urgent events and
renotify so a re-fired reminder re-alerts.
Dose-due, preventive-care, and low-stock reminders already cascade to
Web Push when no APNs channel exists; tests pin that an APNs-less
instance still delivers.
Add an urgent dimension to dispatched notifications so a self-hosted
instance reaches every configured channel at its strongest level without
assuming APNs is present:
- APNs -> interruption-level time-sensitive + priority 10 (critical
only when APNS_CRITICAL_ENTITLEMENT=true; time-sensitive needs no
Apple approval)
- ntfy -> Priority 5 (max) + warning tag (suppressed in discreet mode)
- Web Push-> Urgency: high send option + requireInteraction
- Webhook -> priority "urgent" in the JSON body
- Telegram-> normal delivery (no urgency tier)
Wire the illness red-flag escalation (sustained low SpO2 / sustained
fever) to emit an urgent SYSTEM_ALERT through the dispatcher, owner-scoped
and module-gated, de-duplicated per episode via the push-attempt ledger.
Normal events are unchanged.
The Coach lived under /insights/coach, rendering inside the Insights
layout's tab strip. It is the product's headline surface, not a metric
sub-page, so it now lives at the top-level /coach in the standard page
chrome without the Insights tab strip. The nav model points at /coach,
every in-app link and the proactive-nudge deep link follow, and a 301
redirect keeps /insights/coach (bookmarks, cached PWA navigation, older
push payloads) resolving.
The email sender built its nodemailer transport without connection,
greeting, or socket timeouts, so nodemailer's defaults applied — up to a
10-minute socket timeout. A mail server that accepts the TCP connection
then stalls (firewalled relay, overloaded Postfix, blackholed smarthost)
could hold a dispatch worker for minutes per notification, while every
other egress in the cascade is bounded. Pin connection/greeting at 10s
and socket at 20s so a stall maps to the transient send outcome the SMTP
classifier already retries, matching the webhook sender's ethos.
Alerts and reminders over email — the one channel every self-hoster
already has. The SMTP transport (host/port/auth/from) is operator-supplied
via SMTP_* env and loaded once per process like the APNs key; the only
per-user value is the recipient address, so SMTP credentials never enter a
user's encrypted channel blob. Absent SMTP env disables the channel
cleanly: the dispatcher skips it and the Settings card hides itself.
Bodies are plain text — HTML is stripped and routine reminders run through
the emoji-strip path; no markdown library is added. SMTP 5xx maps to a
hard reject (permanent — auto-disable); connection/4xx errors are
transient and ride the backoff schedule. Adds nodemailer (the standard,
well-audited mail transport) and its types.
A single WEBHOOK channel covers Gotify, Discord, Slack, Matrix, and Home
Assistant: the user supplies a public URL and an optional shared-secret
header, and the dispatcher POSTs a small plain-text JSON envelope. The
outbound goes through safeFetch with requirePublicHost so the SSRF floor
and the connect-time DNS-rebinding pin apply to the user-supplied host,
exactly as they do for ntfy. Responses classify 404/410/401/403 as hard
rejects (auto-disable) and 5xx/429/timeout as transient (backoff).
Settings card mirrors the ntfy card with a save + test-connection flow;
the header value is never returned by the GET and is preserved across a
save round-trip when left blank.
Add WEBHOOK and EMAIL to the channel set, the dispatcher cascade, the
per-event default policy, the push-attempt ledger union, and the admin
diagnostic config-presence check. Channels are string-typed, so no
migration is needed. The dispatcher routes each new type through its
sender with the same decrypt + parse + classify path the existing
channels use; the cascade orders webhook + email between the self-host
relays and the Web Push fallback.
Add the webhook + email channel config shapes and their settings-form
validation schemas, plus the query-key entries for the two new config
reads.
Introduce the MEDICATION_INTAKE_SYNC event and the server-side dispatcher
that wakes a user's other iOS devices after a dose is logged on one of
them, so a running Live Activity or Home-Screen widget reconciles against
the canonical server state.
The dispatch is APNs-only by construction: it sends a silent
apns-push-type: background push (content-available, no alert) carrying
the affected medicationId + scheduledFor, and never fans out to Telegram,
ntfy, or Web Push, which have no silent-sync transport. A new raw APNs
send helper carries the background and liveactivity push types without
the alert/sound/badge enrichment the alert path injects. Devices APNs
reports as permanently dead are reaped; the originating device is
excluded from the fan-out via its registered device token.
When a device has a stored Live Activity push token, an additional
liveactivity end push goes to the per-Activity topic so the lock-screen
Activity ends immediately rather than waiting for the next background
wake.
Register the MEASUREMENT_REMINDER event type (default on at the channel
layer; the real gate is the per-reminder enabled flag) and add the
notificationPrefs.measurementReminder.clientManaged sub-object plus its
cron-side resolver, mirroring the medication and cycle client-managed
precedent. The event rides the generic APNs category path and stays
non-time-sensitive, so it delivers as a plain banner with no client
change.
- The escalated card row keeps its time anchor: the red badge carries
the matched window or the overdue-slot value beside it, and the
inline row's range stays visible on mobile, where it was the only
clock on the card.
- Take-all-due collects what the cards offer: a dose escalated past its
catch-up band joins the set with exactly the slot the card's own
button would record; as-needed medications never join.
- The table's stock warning follows the user's runway threshold instead
of a leftover four-dose constant; threshold off means no amber, empty
stays red.
- The card itself shows the remaining runway in the warning tone once
it crosses the threshold — only then, the card stays calm above.
- The low-stock push deep-links straight into the supply tab and its
APNs payload keeps url, runwayDays and thresholdDays; supply tab and
threshold setting link to each other; the reminder-thresholds and
notification-prefs endpoints enter the OpenAPI contract.
A daily job projects each tracked medication's remaining supply against
its schedules and notifies through the normal channel cascade when the
runway falls below the user's threshold (default seven days, 1-60 or
off, set beside the reminder thresholds). One notification per
crossing: a stamp on the medication row re-arms on refill or threshold
change, and a failed dispatch leaves it clear for the next day's retry.
The GLP-1 card's fixed four-dose flag stays as the locked iOS read
contract; the engine covers it like any other medication.
Users can now tell the assistant about themselves in an encrypted
free-text profile that feeds the coach and briefing prompts as clearly
fenced context, with the coach allowed one targeted follow-up question
when the picture is incomplete. Operators on closed registrations can
mint expiring invite links with a QR code from the admin area; a valid
invite opens registration exactly as many times as it allows and records
who redeemed it. A nightly pass watches three deterministic signals —
low adherence, blood pressure above its target band, a falling recovery
trend — and sends at most one coach nudge per week through the existing
notification channels, gated behind the same opt-outs as every other
assistant surface.
Add a CYCLE_FERTILE_SOON reminder that fires two days before the
predicted fertile-window start. Twice-gated and default-OFF: it only
surfaces when CycleProfile.goal is TRYING_TO_CONCEIVE and fertileWindowStart
is populated, so fertile language never reaches the avoid / general / peri
goals. Reuses the existing cycle-reminder cron, discreet mode, clientManaged
suppression, idempotency ledger, and push-attempts plumbing verbatim; the
body carries the non-medical, not-a-contraceptive framing.
Reminder pushes (medication-due, mood, briefing, cycle) carried the
colour-coded phase markers the Telegram channel uses, which leaked onto the
iOS lock screen and other push channels as decorative bubbles. Strip emoji
in the APNs, Web Push and ntfy senders for every event except SYSTEM_ALERT,
so routine reminders render as plain text while system / failure alerts keep
their severity glyphs. Telegram is unchanged — emoji stay idiomatic there.
The Web Push subscribe route validated the endpoint with a bare URL check
while the hardened schema (https + isPublicUrl) sat orphaned, so an
authenticated user could register an endpoint pointing at an internal host
and the dispatcher — which dials it through web-push's own fetch, bypassing
safeFetch — would reach it. Route the subscribe input through the hardened
schema and re-check isPublicUrl at egress time in the sender to cover any
row stored before the fix.
Also cap GET /api/cycle/insights with a per-user rate limit; it runs the
365-day FDR phase contrast, the heaviest read in the cycle vertical.
