A how-to for subscribing from Google, Apple and Outlook and what the
feed shows; a section on the reproductive-health page saying that once
it is on, the dates also sit with the calendar provider and turning it
off does not reach those copies; a reverse-proxy note with Caddy and
Nginx examples for keeping path credentials out of access logs; and
API_TOKEN_HMAC_KEY rotation named as ending every feed link.
The unreleased changelog entry said the image moved to Wolfi. It now
names the official Node image, why (Raspberry Pi 4 rendering, Node
security releases arriving with the image), that the image carries no
package manager, and that psql is no longer in the app image. The README
and the getting-started prerequisites say the same.
The dependency and build stages stay on node:22-trixie-slim, where pnpm
picks the glibc native packages. The runner moves to wolfi-base, pinned
by its index digest: glibc, about fifty packages, and every one of them
known to the image scan, Node and the PostgreSQL client included.
nodejs-22, tzdata and postgresql-18-client come from apk, so the pgclient
stage and its pin guard go. The builder writes its Node module ABI and
glibc version out and the runner refuses to build on another ABI or an
older glibc. Wolfi's npm is major 12 and runs no install scripts unless
package.json allows them, so the Prisma CLI install names the three
packages that have one, and the build checks that the schema engine file
is there before asking the CLI about it. npm is installed for that step
and removed in the same layer; the check that nothing is left asks a
fresh shell, whose command cache cannot remember the deleted binary.
The image runs the glibc canvas build now, which needs no ARMv8.2
features, so the Pi 4 gets thumbnails and scanned-PDF rendering back
without a code change: the gate only ever closed for musl on arm64. The
gate's comment, the env example and the getting-started prerequisites
say so, and keep NATIVE_CANVAS as the override. The x64 AVX2 gate stays:
the glibc x64 build rendered a PDF page under an emulated core without
AVX2, but it still carries AVX2 code and that one run does not cover
every path.
The reproductive-health page promised that deleting all data or the
account erases at once. Both wait for the grace window now, the nightly
dumps keep a record until they age out, and the old pgdata volume is a
full copy until it is removed; the page says so and how to make deletion
final. The README, the getting-started note on majors, the move guide and
the Unraid and Portainer templates catch up with Postgres 18, the backups
volume and the recovery kit. The demo seed's spread bound leaves room for
the seed's own one-in-150 spread.
The fourth rollback step had been joined onto the paragraph after the
list; it is a list item again, with the way back to the data after it.
The disaster recovery page now says how the server tells admins about
its copies: the recovery readiness card, the recovery kit and the hourly
Backup health notification. The support page put GlitchTip under Admin,
General; the card is under Admin, Integrations, as the deploy runbook
says. The pre-migration dump notes say that without the backups volume
no dump is taken, also where the image's own directory is writable.
The key backup sections become "Save the recovery kit": which keys the kit lists and why, the fingerprint of the token key, and the readiness card with its Backup health notification.
docs/ops/disaster-recovery.md says what is dumped and where, that local
dumps are unencrypted and why, which keys a restore needs and what each
one costs when missing, how to restore onto a new server or back to the
state before a migration, and how the procedure is tested. The deploy
runbook and the getting-started guide now describe the dump the
entrypoint takes before migrating, and the off-host account copy page
points at the dump for rebuilding a server.
Fix two dead anchors (the MCP trust model, the README quick start) and the
statements that no longer match the app: a guardian can connect a managed
profile's services, the environment module is on by default and reaches the
air-quality host too, its readable location columns were dropped in 0409,
settings sections carry their current names, the Withings card lives on
/settings/integrations, the TrueNAS start period matches the image, the
deploy check names HEALTHLOG_PROD_URL, and the docs index describes what
self-hosting/ and integrations/ hold.
The compose file stays on 16. Changing only the image line stops the stack:
the 18 image refuses a data directory from an older major and expects its
volume one level up. The runbook walks the dump and restore into a fresh
volume, keeps the old one as the way back, and is linked from the update
section and the compose pin.
The TrueNAS page now walks through the catalog install field by field, how
to generate the two secrets, where TrueNAS keeps the key and why that is not
a backup, a dump and restore from the postgres container shell, and updates
with forward-only migrations. The listing is marked as pending review, and
the install-via-YAML values stay for the time until it is live.
The deploy notes list what needs a matching truenas/apps pull request in the
same release cycle: a new or stricter required variable, a port or health
check change, a Postgres major or extension, and any other compose-shape
change.
The getting-started requirements now say a host needs 3 GB of RAM, 4 GB
recommended, and drop the advice to lower APP_MEMORY_LIMIT to 1g on a
2 GB host. The compose and .env example comments say the same.
At boot the app reads its cgroup memory limit (v2, then v1) and logs one
line when it is below MEMORY_FLOOR_MB. The support report adds the
container limit and a note below the floor, and Admin > System status
shows "below the supported floor" next to the existing low-heap flag.
The app keeps running either way.
Every raw-SQL clock read against a timestamp column without a zone now
writes NOW() AT TIME ZONE 'UTC', so passkey and login challenges, rate
limits, provider pauses, coach usage stamps, tombstone and measurement
windows no longer pass through the session time zone even where the
per-connection pin cannot apply. The regression-window bounds stay naive
UTC timestamps instead of being turned into timestamptz. A structural
guard keeps bare NOW() / CURRENT_TIMESTAMP out of raw SQL outside a short
list of timestamptz targets. The integration test sets the database
default to Europe/Berlin and drives challenge claims, session expiry, the
native sign-in handoff freshness check and the rate-limit window through
the app's client, plus the challenge claim through an unpinned client.
Self-hosting docs describe the pin for an existing or shared Postgres.
Getting started names the APP_MEMORY_LIMIT default and how to lower it
on a 2 GB host; the scaling guide says that web and worker share one
pool in the bundled single-container setup. The rollup comment names
migration 0372, which carries the hourly-mean statistic.
At a first start there is no recorded key yet, so an operator facing a
refusal they are certain is false has no row to delete. With
ENCRYPTION_KEY_CHECK=warn (default enforce) the check still runs and logs
its full outcome, /api/health names a mismatch as a warning field without
failing, requests are served, and no key is recorded unless every key id
passes.
The variable is on the compose whitelist, in both example files, the env
manifest, the Unraid and Portainer templates and the published-env guard.
The log block, the mismatch page and the self-hosting pages offer it as the
last resort after the three real fixes, and say that it disables a safety
check.
The boot key check recorded whatever key was configured whenever its probe
found nothing to try. It only read string columns, so an install whose
ciphertext sat in Bytes columns (notes, documents, ECG, symptoms) read as
empty; a probe that ran out of time did the same; and rows written by a
process that served under a wrong key could open on the next boot and get
that key recorded as the right one.
The probe now reads every registered column, string and Bytes, in all four
stored layouts, and tries the oldest values first, ordered by the row's
updatedAt (else createdAt, else primary key). A key is recorded only when
the oldest value that opens is preceded by at most one that does not, or
when no column holds a single value under the key id. A time-out, an
unreadable table, oversized values or mixed results are inconclusive: the
process serves, records nothing and checks again at the next start.
When the first insert loses a race, the stored record is read back and must
open under this process's key, else the outcome is a mismatch.
A rotation that walked every column without errors removes the record of
each previous key id that no longer holds any value. The refusal log block
and the self-hosting pages explain how to reset the check for a key id.
A server that issues Secure session cookies cannot sign anyone in on a
plain http:// page: the browser drops the cookie. The login request was
still verified, the MFA ticket minted and then claimed, and the next
attempt answered "Invalid or expired challenge".
The web client now sends an optional clientTransport on the browser
sign-in requests (password login, passkey start, both MFA completions).
When the server issues Secure cookies and the page is plain http on a
non-loopback host, the request is refused with 409
auth.session.insecure_transport before the password is checked, a
ticket is minted or claimed, a code is spent, or a passkey challenge is
issued. A request without the field is never refused. The SSO start
takes client_proto/client_host and sends such a page back to the login
page instead of to the provider.
GET /api/auth/registration-status publishes sessionCookieSecure, and the
login page shows the reason with both fixes and disables the sign-in
controls before anything is sent; on localhost it shows a note only.
SESSION_COOKIE_SECURE keeps its meaning; the header heuristic log line
stays.
Every configured key id now has a canary in encryption_key_canaries,
checked at boot. A key that cannot open its canary, or existing data
written before the canary existed, puts the process in a refusal state:
every API route answers 503 encryption.key_mismatch, /api/health reports
reason encryption_key_mismatch, the pages render one explanation, and
neither the queue producer nor the worker starts. The process stays up
so the reason is visible instead of hiding in a crash loop.
The admin Encryption page gains a "Back up your encryption key" step:
key id and fingerprint (never the key), where the key lives per
platform, a check of a pasted copy that is compared and discarded, and
a confirmation bound to the active key id and fingerprint. The overview
gets a tile and admins get a dashboard banner while it is due.
HEALTHLOG_PLATFORM picks the platform the step opens on. Adds the
TrueNAS self-hosting page and key backup sections in the docs.
Two requests left the server without anything being connected: the
HaveIBeenPwned range query on every password set (registration, change,
admin reset) and the daily GitHub releases request behind the About page
and the admin version tile. Neither could be turned off, which made
"nothing leaves" untrue for an instance that wanted it to be.
PASSWORD_BREACH_CHECK_DISABLED and UPDATE_CHECK_DISABLED (1, true, yes or
on) stop each request. Both stay on by default. The breach check keeps
its fail-open contract: switched off, a password is simply not looked up.
The update route answers status "unknown" with reason "disabled" and
the admin tile says the check is off. The password routes reach the
check only through the switch, and a test holds that import line.
Both are on the compose whitelist, in the env manifest under a new
"Outbound requests" group with IP_GEO_LOOKUP_DISABLED, and documented in
both env examples and the getting-started guide.
- geolite2: quote the real commented compose mount (./geolite2 to
/opt/geolite2, the GEOLITE2_DIR default, so no env var is needed for
the default path)
- getting-started: healthcheck probes /api/health, not /api/version;
the environment module reaches archive-api and geocoding-api
.open-meteo.com, relevant for egress allowlists
- mcp: the module is opt-in and off by default, but no longer the only
such module
- reverse-proxy: the Coach stream is SSE over plain HTTP, not
WebSockets; note that TRUST_PROXY_HOPS is not on the bundled compose
whitelist and must be added by hand
- scaling: /api/health healthcheck; worker liveness is in-memory and
invisible across a web/worker split; pool sizing now goes through
getConnectionBudget with DB_CONNECTION_LIMIT as the compose knob and
a 2-connection pg-boss share
- sso: the native SSO handoff shipped, OIDC_ONLY works with app builds
that carry it
- unraid: geo databases are baked in only when the build had a licence
key, or mounted; replaceable either way
The cookie was the Session row's cuid. cuid is built for collision-
resistant identity across distributed writers, not for unguessability —
it encodes a millisecond timestamp and a process-local counter and never
touches a CSPRNG. A session cookie is a bearer credential and has to be
unguessable, so it now carries 32 random bytes and only the HMAC of that
value is stored, matching how api_tokens and refresh_tokens already hold
their secrets.
Compatibility is passive, and deliberately so. A backfill cannot help:
the cookie already sitting in a browser is the cuid, and nothing we
generate server-side can be pushed into it — backfilling would sign
everyone out at deploy. Upgrading a row in place on first use loses to
the parallel requests a single page load fires: each mints its own
secret, one write wins, and the losers cannot re-emit the winner's
secret because only a hash is stored, so the browser can end up holding
a value the row does not honour. Instead, rows minted before the column
keep a NULL token_hash and stay resolvable by id, and are denied the
sliding-expiry extension so they drain within the 30-day session
lifetime. Every login from this release forward is on a secret at once,
and an id stops authenticating the moment its row holds one.
Note for operators: API_TOKEN_HMAC_KEY now also keys session cookies, so
rotating it signs browser sessions out as well as invalidating tokens.
It is already Core-required, so no deployment gains a new prerequisite.
Registration stays open after the first admin so a household can be
invited, but on an internet-exposed instance that means anyone who can
reach the URL can sign up — and nothing prompted the admin to close it.
Surface a muted hint under the registration toggle whenever it is open
(no behaviour change), and note the reality in getting-started's
first-user section.
The bundled compose constructs DATABASE_URL inside the app service from
POSTGRES_PASSWORD, so the getting-started 'edit CHANGE-ME' step was a
no-op and .env.example's literal POSTGRES_PASSWORD collided with the
quick-start's appended one. Comment both out in .env.example so the
append pattern yields a single key, and rewrite step 2 to state compose
derives the URL. Add a first-user note to close public registration on
an internet-exposed host and to save the password (no-SMTP CLI reset).
Refresh the stale onboarding step-2 source comment.
Reflect the v1.25 surface without touching the changelog or version
numbers.
- README highlights now cover the new clinical signals (respiratory
rate, the opt-in PHQ-9 / GAD-7 mental-wellbeing self-checks with calm
crisis handling, grip strength, pain score, waist / body measurements,
longevity labs), the signal registry that wires each one across charts,
correlations, the coach, FHIR, and the API at once, structured records
(allergies, family history; immunizations noted out of scope), the
opt-in environmental-context module, inbound document extraction
(transcribe, never interpret), and the self-description posture (a
record-keeping and wellness tool, not a medical device).
- ai-providers.md gains a "what the Coach is — and isn't" section: the
non-diagnostic standard and the calm proactive morning check-in
behaviour.
- mcp-capabilities.md notes that the new measurement signals reach the
assistant through the existing metric and labs tools via the registry,
that the mental-wellbeing instruments / records / documents are
deliberately not exposed over MCP, and ties the grounding contract to
the project-wide self-description standard.
- getting-started.md lists the environmental module's Open-Meteo egress
as an opt-in outbound dependency.
One-shot prettier pass so the new format:check gate starts from a clean
baseline. Formatting only — no behaviour change. Generator-owned and
vendored artifacts stay untouched via .prettierignore.
Cover the full channel set self-hosters can run without an Apple account:
Web Push, Telegram, ntfy, the generic webhook, and email over SMTP, plus
the dispatcher cascade and graceful degradation when APNs is absent
(ntfy max priority, Web Push high urgency, webhook urgency flag).
Document the PWA path as the recommended default: install to the Home
Screen, enable Web Push, and get reminders, the taken-clears-the-reminder
behaviour, and the outstanding-dose app badge. Note the inherent limit
that the lock-screen Live Activity needs a native build.
Explain why native iOS push binds to the publisher's Apple team and walk
through running your own signed build under your own bundle ID, team, and
.p8 key, with the APNS_* env vars including APNS_CRITICAL_ENTITLEMENT.
Document the new Generate keys button in notifications.md as the easiest
Web Push setup path, demoting the CLI to an alternative. Note that
clientManaged medication-reminder suppression is an iOS-app-only contract
so a PWA-only operator does not go hunting for a web toggle.
Add the VAPID env-var names and APNS_KEY_B64 to .env.production.example
(the loader and APNs sender read them, but the example omitted them),
both as commented placeholders, with a note that Web Push needs no Apple
account and that these vars are not in the compose whitelist.
Add a Backup and restore section to getting-started.md: a local pg_dump
one-liner and the load-bearing warning that the ENCRYPTION_KEYS /
ENCRYPTION_KEY must be backed up alongside the dump or the encrypted data
is unrecoverable, cross-linking the off-host backup and key-rotation
runbooks. Add troubleshooting entries for the compose env-var whitelist
and pull_policy: always.
openssl rand -base64 24 can emit URI-reserved characters (/, +, =). Since POSTGRES_PASSWORD is interpolated unescaped into DATABASE_URL, such a password breaks the connection URL and blocks boot. Switch every documented example to openssl rand -hex 32 (URL-safe, 256-bit).
Add WHOOP as a synced provider (per-user BYO-keys OAuth2 + HMAC-signed
webhooks), the longitudinal coach (narrative + per-metric trajectory),
and the read-only FHIR R4 REST API plus the scoped, time-limited
clinician share-link record across README, the self-hosting and ops
runbooks, and a new WHOOP integration page. Note the two optional
WHOOP instance env vars, that migrations 0110-0114 are additive, and
the two known limitations (dual-source standard-vital dedup deferred;
coach durable-memory deferred).

It is a build-time NEXT_PUBLIC_* var (inlined into the client bundle),
default ON, disabled only by rebuilding with the var set to false. Note
the build-time nature in .env.production.example and getting-started,
and keep it out of the compose runtime whitelist where it would mislead
operators.
Consolidated hotfix landing the findings from a six-axis audit
covering mobile security, UX + accessibility, code quality + docs,
README + discoverability, web performance, and mobile-deep responsive
behaviour. None of the changes break the public API or the existing
data shape; every fix is additive and SAFE under the v1.4.34.x web
freeze. Full audit reports live in `.planning/round-audit-*.md`.
Security
- Password rotation now wipes API tokens + refresh tokens in the
same `$transaction` as session revocation. A stolen iOS Bearer no
longer survives a user-initiated password change or an admin reset.
- `/.well-known/` proxy bypass tightened to an explicit-allowlist
`Set`. Stale `/api/auth/codex/callback` PUBLIC_PATHS entry dropped.
- HSTS gains `preload`. Withings host removed from the global CSP
and gated to the integration's own routes.
- `getClientIp` warns once per process on `TRUST_PROXY_HOPS`/XFF
mismatch + exposes a tagged helper for tighter universal buckets.
- Service worker rejects off-origin notification-click URLs. Offline
fallback body switched to a locale-neutral icon. Manifest gains
`scope`, `id`, `display_override`.
UX + accessibility
- i18n leaks closed on Dialog/Sheet close-X, ChartSkeleton's loading
announcement, and the notifications-section breadcrumb.
- Focus rings on bottom-nav, the More overflow button, and the
mobile top-bar user menu.
- Tap-target floor: Input / NativeSelect / Select / DateTimeInput
switch to `h-11 sm:h-10`.
- Admin chip-strip auto-scroll-into-view (matches v1.4.33 settings).
- List-page subtitles visible on mobile.
- Insights empty-state CTA vocabulary normalised so the link opens
the right measurement form (was silently failing for
BODY_TEMPERATURE / BLOOD_GLUCOSE / PULSE).
Mobile-deep
- `viewport.themeColor` reacts to light/dark mode.
- TopBar + Coach FAB reserve safe-area-inset-top / -bottom for
notched iPhones.
- Sonner toaster switches to `theme="system"`.
Code quality
- `SESSION_SECRET` dropped — the env var was documented as required
but never read by code. Three secrets, not four. Self-hosters
upgrading can delete the existing line.
- `toJson<T>()` helper consolidates seven Prisma JSON-cast sites.
- `db-compat.ts` gains a header docstring naming it the
schema-bootstrap path.
- Passkey response types adopted from `@simplewebauthn/server`;
`any` + eslint-disable pragmas drop out.
- Zero-TODO CI gate added at `.github/workflows/no-todo-markers.yml`.
Documentation + discoverability
- README rework: How-it-compares matrix, Apple Health + AI Coach
promoted to standalone Key-Features bullets, status block, latest
release + GHCR badges, OpenAPI pointer, three-secret Quick Start.
- openGraph + twitter cards now carry `images` and a
`summary_large_image` Twitter card.
- GitHub repo metadata: description tightened from 290 to 156
chars + keyword-front, `homepageUrl` flipped from a personal
tenant to the demo site, six generic / PII-adjacent topics
swapped for six high-intent ones.
- Five new user-facing docs: `docs/self-hosting/getting-started.md`,
`docs/self-hosting/reverse-proxy.md`,
`docs/integrations/apple-health.md`,
`docs/integrations/withings.md`,
`docs/integrations/ai-providers.md`.
- `docs/README.md` clarifies the docs tree as internal playbooks
vs the user-facing `docs.healthlog.dev` site.
