From 5d80b194ac554def2bef51f2da00a8f548a94b43 Mon Sep 17 00:00:00 2001 From: =?UTF-8?q?Marc-Andr=C3=A9=20Bombeck?= Date: Sat, 26 Sep 2026 12:55:28 +0200 Subject: [PATCH 1/6] feat(mcp): search and fetch cover visits, conditions, documents and vaccinations In its default mode ChatGPT can call only search and fetch, and until now those two covered metric domains, medications and lab analytes. A question about a knee operation or a discharge letter had nothing to land on. search now also returns visit: (procedures included), condition:, document: and vaccination:. Matching runs after the decrypt over the account's own rows, bounded per kind, folded the way the body-sites view folds sites, with German/English body-part and vault synonyms. Documents match on title, file name, kind and whole words of the blind content index; the body is never read for a search. Results rank by matched query words, then score, kind, date and id, so the offset cursor stays stable. Record results are capped at 50. fetch returns each record's key fields and its one-hop links through the link service, every link carrying an id fetch accepts. Free text is fenced; short labels are scrubbed. A document returns metadata and at most a 1500-character excerpt of its indexed text, never the file or its stored summary. A switched-off module's records are neither read nor returned: illness, inboundDocuments and vaccinations for the new kinds, and now medications, labs and the owning module of a discovery signal for the older ones. --- src/lib/mcp/__tests__/record-search.test.ts | 670 ++++++++++ src/lib/mcp/__tests__/search-fetch.test.ts | 7 + src/lib/mcp/__tests__/tools.test.ts | 18 +- src/lib/mcp/record-search.ts | 1340 +++++++++++++++++++ src/lib/mcp/server.ts | 4 +- src/lib/mcp/tools.ts | 189 ++- 6 files changed, 2165 insertions(+), 63 deletions(-) create mode 100644 src/lib/mcp/__tests__/record-search.test.ts create mode 100644 src/lib/mcp/record-search.ts diff --git a/src/lib/mcp/__tests__/record-search.test.ts b/src/lib/mcp/__tests__/record-search.test.ts new file mode 100644 index 000000000..0518685b6 --- /dev/null +++ b/src/lib/mcp/__tests__/record-search.test.ts @@ -0,0 +1,670 @@ +/** + * `search` + `fetch` over the clinical records (v1.39.3, Discussion #1025): + * visits with procedures, conditions, documents, vaccinations. + * + * The Prisma mock is a small in-memory store that honours `userId`, `id`, + * `deletedAt` and `hasSome` in the `where` it is handed, so a read that + * forgot the owner narrowing would hand back the other account's rows and + * the leakage tests below would go red. + */ +import { describe, it, expect, vi, beforeEach } from "vitest"; +import { z } from "zod/v4"; + +process.env.APP_URL = "https://health.example"; + +const disabled = new Set(); + +vi.mock("@/lib/logging/context", () => ({ annotate: vi.fn() })); +vi.mock("@/lib/ai/coach/tools/inventory", () => ({ + buildCoachDataInventory: vi.fn(async () => ({ + entries: [], + window: "90d", + restMode: false, + cycleEnabled: false, + })), +})); +vi.mock("@/lib/ai/coach/tools/executor", () => ({ executeCoachTool: vi.fn() })); +vi.mock("@/lib/modules/gate", () => ({ + isModuleEnabled: vi.fn(async (_u: string, key: string) => !disabled.has(key)), +})); +// Ciphertext in this store is the UTF-8 bytes of the plaintext. +vi.mock("@/lib/ai/coach/bytes-codec", () => ({ + decryptFromBytes: (b: Uint8Array) => Buffer.from(b).toString("utf8"), +})); +vi.mock("@/lib/documents/content-index", async (importOriginal) => { + const real = + await importOriginal(); + return { + ...real, + // A deterministic stand-in for the keyed HMAC: same token, same tag. + hashQueryTokens: (q: string) => real.tokenise(q).map((t) => `h:${t}`), + decryptVerbatimText: (b: Uint8Array) => Buffer.from(b).toString("utf8"), + decryptIndexText: (b: Uint8Array) => Buffer.from(b).toString("utf8"), + }; +}); + +type Row = Record & { id: string; userId: string }; +const store: Record = {}; +const links: Array<{ + userId: string; + sourceKind: string; + sourceId: string; + targetKind: string; + target: { id: string; label: string; date: string | null }; +}> = []; + +function matches(row: Row, where: Record = {}): boolean { + for (const [key, cond] of Object.entries(where)) { + const value = row[key]; + if (key === "document") { + const doc = store.inboundDocument.find((d) => d.id === row.documentId); + if (!doc || !matches(doc, cond as Record)) return false; + continue; + } + if (cond && typeof cond === "object" && !(cond instanceof Date)) { + const c = cond as Record; + if ("in" in c && !(c.in as unknown[]).includes(value)) return false; + if ("hasSome" in c) { + const set = new Set(value as string[]); + if (!(c.hasSome as string[]).some((h) => set.has(h))) return false; + } + continue; + } + if (cond === null ? value != null : value !== cond) return false; + } + return true; +} + +function table(name: string) { + return { + findMany: vi.fn( + async (args: { where?: Record; take?: number } = {}) => + (store[name] ?? Before this + * module those two covered metric domains, medications and lab analytes, so a + * question like "when was my left knee operation" or "what did the discharge + * letter say" had nothing to land on: the visit history lived behind + * `get_visits`, which that mode never calls. + * + * Matching. Visit reasons, outcomes and body sites are ciphertext at rest, and + * so are condition sites and notes, so matching cannot run in SQL. It runs + * here, after the decrypt, over the account's own rows only and bounded per + * kind (`MAX_ROWS_PER_KIND`, the same bound the body-sites view uses). The + * folding is the body-sites service's (`normalizeSearchText`), so "Knie" and + * "knie " are one word here as they are one site there. A document is matched + * on its title, file name and kind, and, when the owner has a content index, + * on whole words of its text through the blind token index the vault search + * uses. The document body itself is never read for a search. + * + * Ranking. Every query word scores the best field it hits (a title, label, + * site, reason or practitioner counts more than a kind word or a side, which + * counts more than a note or an indexed-text hit). Results order by how many + * query words they matched, then by score, then by a fixed kind order, then + * newest first, then by id: the same query over the same record always + * returns the same list, which is what the offset cursor relies on. + * + * Module switches. A record kind whose module the account has off (or the + * operator switched off) is not read at all: no row, no link, no count. + * Visits belong to no module. Links follow the same rule per target kind. + * + * AI capability. These are reads of the person's own data over their own MCP + * connection, like every other MCP read, so no AI capability gates them. The + * one piece of model-written text a document can carry, its stored summary, + * is deliberately not returned: showing stored model text is a capability + * question, and the indexed text is the document's own words. + * + * Free text. Everything a person or a document wrote is fenced in `text` and + * in the long metadata fields; short labels (titles, practitioner names, link + * labels) are scrubbed of forged markers instead, as the existing search + * titles are, because a host renders them in a list. + */ +import type { + EncounterKind, + IllnessLifecycle, + IllnessType, + InboundDocumentKind, + Laterality, +} from "@/generated/prisma/client"; +import { prisma } from "@/lib/db"; +import { decryptFromBytes } from "@/lib/ai/coach/bytes-codec"; +import { fenceUserText, scrubFenceMarkers } from "@/lib/ai/coach/data-fence"; +import { + decryptIndexText, + decryptVerbatimText, + hashQueryTokens, + tokenise, +} from "@/lib/documents/content-index"; +import { expandQueryTokens } from "@/lib/documents/search-synonyms"; +import { normalizeSearchText } from "@/lib/encounters/procedures"; +import { encounterKindLabel } from "@/lib/encounters/kind-label"; +import { getServerTranslator } from "@/lib/i18n/server-translator"; +import { defaultLocale, locales, type Locale } from "@/lib/i18n/config"; +import { listTargets } from "@/lib/links"; +import type { LinkedTarget } from "@/lib/links/link-service"; +import { isModuleEnabled, type ModuleKey } from "@/lib/modules/gate"; +import { DEFAULT_TIMEZONE, userDayKey } from "@/lib/tz/format"; +import { resolveCatalogEntry } from "@/lib/vaccinations/vaccine-catalog"; + +// ── Bounds ─────────────────────────────────────────────────────────── + +/** Rows one search decrypts and matches, per record kind. */ +export const MAX_ROWS_PER_KIND = 500; +/** Query words that take part in matching; the rest are ignored. */ +export const MAX_QUERY_WORDS = 8; +/** Record results one search can return in total, across the four kinds. */ +export const MAX_RECORD_RESULTS = 50; +/** Newest records per kind listed when the query is empty. */ +export const EMPTY_QUERY_PER_KIND = 10; +/** Characters of indexed document text one `fetch` returns. */ +export const MAX_DOCUMENT_EXCERPT_CHARS = 1500; +/** Characters of a condition or dose note one `fetch` returns. */ +export const MAX_NOTE_CHARS = 1000; +/** One-hop links one `fetch` returns. */ +export const MAX_LINKS = 25; + +// ── Kinds and their modules ────────────────────────────────────────── + +export type RecordKind = "visit" | "condition" | "document" | "vaccination"; + +export const RECORD_KINDS: readonly RecordKind[] = [ + "visit", + "condition", + "document", + "vaccination", +]; + +/** + * The module that owns each record kind. Both reads use an explicit column list without the note, as the Coach illness snapshot and the doctor report already do. A structural guard fails when any MCP read of either model names noteEncrypted or has no explicit select. - A document uploaded with AI reading deferred is not searched by its text and returns no excerpt (metadata.reason "ai_read_deferred"); its title, kind and links stay findable. - Documents older than the newest 500 are found by title and file name through a bounded SQL lookup on those plaintext columns. - Query words shorter than four letters match whole words or word starts only, so "arm" no longer finds "Darm". - A deleted practitioner is no longer named or matched, in search, fetch, get_visits and the appointments in get_preventive_care. The empty-query listing shows visits that happened, not booked ones. - get_labs, get_medication_schedule, get_medication_compliance and the lab and medication resources (read, list, completion) answer { present: false, reason: "module_disabled" } when their module is off, without reading. get_visits withholds condition labels when illness is off and says so in conditionsReason. - The consent screen and the tool descriptions name what a connection can read, the indexed text of stored documents included. --- .../api/mcp/oauth/__tests__/authorize.test.ts | 10 + src/app/api/mcp/oauth/authorize/route.ts | 5 + .../mcp/__tests__/module-gate-sweep.test.ts | 186 +++++++++++++++ .../__tests__/no-note-columns-guard.test.ts | 134 +++++++++++ src/lib/mcp/__tests__/record-search.test.ts | 153 +++++++++++- src/lib/mcp/__tests__/tools.test.ts | 12 +- src/lib/mcp/record-search.ts | 219 ++++++++++++++---- src/lib/mcp/resources.ts | 24 +- src/lib/mcp/tools.ts | 168 ++++++++++---- 9 files changed, 810 insertions(+), 101 deletions(-) create mode 100644 src/lib/mcp/__tests__/module-gate-sweep.test.ts create mode 100644 src/lib/mcp/__tests__/no-note-columns-guard.test.ts diff --git a/src/app/api/mcp/oauth/__tests__/authorize.test.ts b/src/app/api/mcp/oauth/__tests__/authorize.test.ts index f302a20c3..68142b8c1 100644 --- a/src/app/api/mcp/oauth/__tests__/authorize.test.ts +++ b/src/app/api/mcp/oauth/__tests__/authorize.test.ts @@ -106,6 +106,16 @@ describe("GET /authorize — consent gate", () => { expect(body).not.toMatch(/hlac_/); }); + it("names what the connection can read, document text included", async () => { + signedIn(); + const body = await (await GET(getReq(authorizeUrl()) as never)).text(); + // v1.39.3: search and fetch reach visits, conditions and the indexed + // text of stored documents, so the consent says so before Allow. + expect(body).toMatch(/visits/); + expect(body).toMatch(/text of your stored documents/); + expect(body).toMatch(/held back from AI reading/); + }); + it("prompts sign-in (with a return link) when there is no session", async () => { const res = await GET(getReq(authorizeUrl()) as never); expect(res.status).toBe(200); diff --git a/src/app/api/mcp/oauth/authorize/route.ts b/src/app/api/mcp/oauth/authorize/route.ts index 3f83189b0..783940496 100644 --- a/src/app/api/mcp/oauth/authorize/route.ts +++ b/src/app/api/mcp/oauth/authorize/route.ts @@ -375,6 +375,10 @@ async function authorizeGet(request: NextRequest): Promise { const accessSummary = writeGranted ? `
