# Phase D — Code review (v1.4.20)
Reviewed against `develop` HEAD `ded0b38`. v1.4.20 = Insights redesign + AI Coach (Wave B, 27 commits, +14883/-359 LOC across 94 files).
## Summary
- Files reviewed: 38 source / route / lib files + 16 test files + 1 schema migration + i18n bundles
- Findings: 0 CRITICAL · 4 HIGH · 9 MED · 6 LOW
- Overall quality: Strong. The Coach surface is the most security-conscious slice in the repo to date — bearer/cookie parity, AES-GCM-at-rest with key rotation, label-only provenance, prompt-injection regex bank, per-user daily token budget, 404-not-403 cross-user existence guard, idempotent first-turn POSTs but **not** follow-up turns. Correlation maths includes the `n >= 14` and `p < 0.05` bar, conservative non-causal phrasing baked into both the lib and a unit test, with EmptyState fallback for below-threshold results. Test coverage is meaningful (parser fuzz, encryption round-trip, ownership, budget clamp, weekday ANOVA outlier, ISO week round-trip, score band/clamp/null-distribution). The four HIGH findings are real bugs but each is recoverable; nothing here blocks the release.
## CRITICAL
_None._
## HIGH
### CR-HIGH-01 — Coach SSE error frames return 503 but the streaming hook never reads them
**File**: `src/app/api/insights/chat/route.ts:412-426` and `src/components/insights/coach-panel/use-coach.ts:293-302`
**What**: When the provider chain fails or the user has no provider, `streamProviderError()` returns a `Response` with `status: 503` whose body is an SSE stream containing `{type:"error",code:"coach.provider.unavailable"}`. On the client, `useSendCoachMessage` checks `if (!response.ok || !response.body)` and short-circuits with `errorCode: \`coach.http.${response.status}\``. The stream — including the error frame the route emitted — is never read. The user sees `coach.http.503` which `MessageThread.safeError` falls back to `t("insights.coach.errorProvider")`, so the *visible* copy is OK, but the rich `code` channel on the SSE event is wasted, and the route emits a body that no current client consumes.
**Why**: The route's SSE error path is dead code from the client's perspective. If a future iteration adds richer error handling that depends on parsing the `error` frame's `code`, it will silently never fire. This is also a footgun for the next maintainer reading the route — they'll assume the client honours the SSE error.
**Fix**: Either (a) flip the response to `status: 200` so the client reads the stream and extracts the structured error code, or (b) drop the SSE body in `streamProviderError` and return a JSON envelope with the error code so the client's HTTP-status branch sees a structured payload.
### CR-HIGH-02 — `useSendCoachMessage` re-creates `send` every render and double-abort risk on parent re-render
**File**: `src/components/insights/coach-panel/use-coach.ts:240-382`
**What**: `useSendCoachMessage` accepts `opts: UseSendCoachMessageOptions = {}` and lists `[opts, queryClient]` in `send`'s `useCallback` deps. The drawer mounts the hook with an inline object literal `useSendCoachMessage({ onDone: (resolvedId) => setCurrentConversationId(resolvedId) })` (`coach-drawer.tsx:106-110`). Every render creates a new object identity, so `send` is re-memoised every render. While streaming, no caller invokes `send` so the in-flight reader keeps running, but if the user clicks "send" twice in quick succession across two renders, the second `send()` call invokes `abortRef.current?.abort()` and tears down the first stream — a behaviour the comment ("Cancel any prior in-flight request") does intend, but the user has not opted into it.
**Why**: Less of a correctness bug than a deps-correctness smell that bites the next person who edits the hook. A future ref-pattern change could turn the per-render `send` identity into a real bug (e.g. effects that depend on `send`).
**Fix**: Stash `opts` (or just `opts.onDone`) into a `useRef` updated in a render-effect-free way, and pass `[queryClient]` to the `useCallback` deps. Alternatively, drop `opts` from the deps and reference the latest via the ref.
### CR-HIGH-03 — Idempotency wrapper double-reads the request body
**File**: `src/app/api/insights/chat/route.ts:429-450`
**What**: The outer `apiHandler` clones the request, parses its JSON to detect `conversationId`, and **then** dispatches into `handleChatRequest(request)` (the original) which calls `request.json()` again at line 132. Both paths read the body. `request.clone()` ostensibly returns an independent stream, so the original `request` is still fresh — but the comment block in the route admits the cloning is hot-path logic ("validation will surface the issue inside the handler"). If a future runtime tightens body-stream semantics or `withIdempotency` itself begins consuming the body, the double-read becomes silently wrong (handler sees an empty body, surfaces 400). There is also no body-size cap on the clone read; a 4 KB body is fine but a runaway 10 MB JSON would be parsed twice.
**Why**: Streaming-body parsers are notoriously version-fragile. The current Next.js + Node combo allows the clone, but the reliance is implicit. Worse, when `conversationId` is present the handler runs *without* idempotency protection — a duplicate POST with the same `Idempotency-Key` to an existing thread will replay the provider call and double-bill the budget.
**Fix**: Read + parse the body **once** at the top of the handler, then pass the parsed object into the inner function; or hoist the conversationId parse into a tiny helper that operates on a cached JSON read. Document that follow-up turns deliberately skip idempotency, or apply it (the tests in `coach-conversations.test.ts` don't probe this case).
### CR-HIGH-04 — `streamProviderError` parameter typed as `ReturnType<typeof Object.assign>` (≈ `any`)
**File**: `src/app/api/insights/chat/route.ts:406-410`
**What**: `streamProviderError({ conversationId, snapshot: ReturnType<typeof Object.assign>, code })` — the `snapshot` parameter is typed `ReturnType<typeof Object.assign>` which TypeScript widens to `any`. This is a `noImplicitAny` escape hatch that defeats strict mode. The function never actually uses `snapshot` (look at the body — it only encodes the error frame), so the type tells the wrong story too.
**Why**: Strict-mode `any` leak — violates the project's `tsconfig.strict = true` posture documented in CLAUDE.md. Once a downstream caller passes a real provenance value, the wrong shape will be silently accepted.
**Fix**: Drop the `snapshot` parameter entirely (it is unused), or type it as `CoachProvenance` from `@/lib/ai/coach/types` if the function is meant to attach provenance to the error frame in a future commit.
## MED
### CR-MED-01 — `useEffect` for streaming auto-scroll lists `streaming?.content` but not `streaming` identity
**File**: `src/components/insights/coach-panel/message-thread.tsx:72-78`
**What**: The auto-scroll effect depends on `[messages.length, streaming?.content]`. When the parent flips from one streaming session to a fresh one with content `""`, the effect won't fire (length unchanged, content unchanged from "" → ""), and the viewport may stay scrolled mid-thread. Mostly harmless because `messages.length` will tick when a new persisted message lands, but the dependency is brittle.
**Fix**: Add `streaming?.inProgress` to the dep list (or track a turn counter).
### CR-MED-02 — `bandFromInterval` can return a misleading "high" label on a tight CI around r ≈ 0
**File**: `src/lib/insights/correlations.ts:558-570`
**What**: `bandFromInterval([-0.05, 0.10])` → width 0.15 → label "high". A "high confidence" chip on a near-zero correlation reads as "we're confident the effect is meaningful" but the CI just confirms the effect is small. The card is gated by `pValue < 0.05` upstream which prevents the worst case, but a borderline-significant near-zero r could surface a "High confidence" chip alongside an interpretation that says "do not move together strongly".
**Fix**: Demote the chip to "moderate" when the CI bracket straddles zero, or rename the chip to something like "tight CI" / "wide CI" so the language doesn't conflate "narrow CI" with "meaningful effect".
### CR-MED-03 — Pearson p-value uses normal-approximation across the entire surfacing range
**File**: `src/lib/insights/correlations.ts:182-199`
**What**: `twoSidedPFromT(absT, df)` uses `2 * (1 - normalCdf(absT))` for `df ≥ 12` and a "crude correction" `* 0.85` below that. Comment claims accuracy "to within ~0.005 over the surfacing range" but at the n=14 (df=12) gate the t-distribution's heavier tail can put a true p≈0.04 result at p≈0.025 under the normal approximation — a 1.6× understatement of significance. The threshold-crossing risk is real.
**Fix**: Either pull in a tiny incomplete-beta implementation (e.g. ~30 LOC) or raise the gate to df ≥ 20 to keep the normal approx honest.
### CR-MED-04 — `weightTrendAlignment` uses ±2 kg target band derived from a single number, not the user's actual band
**File**: `src/lib/analytics/health-score.ts:289-294`
**What**: `deriveWeightTarget(targetKg)` always returns `{ min: target-2, max: target+2 }`. The user's stored `weightTargetKg` is a point estimate; the score then awards 100 only when *latest* falls in the ±2 kg band. A user with a stable target weight of 80 kg and a steady 79 kg actual reading scores 100; the same user at 77 kg scores < 100 even with a flat trend. There's no clinical basis for ±2 kg specifically — pulling from the existing `value-bands` infrastructure (which has a real green/orange band defined per-user via `buildWeightRangeFromHeight`) would be more honest.
**Fix**: Reuse `buildWeightRangeFromHeight(user.heightCm)` (already imported in the analytics route) and pass the green band as the `target`. Optional: fall back to ±2 kg only when `heightCm` is unset.
### CR-MED-05 — `computeUserHealthScore` re-uses identical `bpInTargetPct` for both current and previous snapshot
**File**: `src/app/api/analytics/route.ts:509-520`
**What**: The "vs last week" delta is supposed to compare against a 7-day-shifted snapshot, but the BP component is held constant ("we'd need to rewind the all-time aggregate"). Three of the four pillars move; one is frozen. When the BP component dominates the weight (0.30 of 1.00), the delta under-represents real change. The comment acknowledges the trade-off, but the surface text reads "vs last week" without disclosing the frozen pillar.
**Fix**: Either compute a real 7-days-prior `bpInTargetPct` (re-pair sys+dia rows up to `prevUntil`) or relabel the delta as "vs last week (BP held constant)" / show the per-component delta breakdown so the user knows where the move came from.
### CR-MED-06 — `WeeklyReportView` early-returns the loading spinner when advisor is loading, even after the report is cached
**File**: `src/components/insights/weekly-report-view.tsx:85-94`
**What**: `if (authLoading || advisor.isLoading) return <Loader2 />`. `useInsightsAdvisorQuery` returns `isLoading: true` only on first mount; subsequent navigations hit the cached payload and `isLoading` is false from the first render. Edge case: a hard reload of `/insights/report/[week]` with no cache will spin even when the cached payload would arrive within ~50ms. Fine for UX, but the spinner blocks the auto-print effect: `if (advisor.isLoading) return` at line 73 means `?print=1` deep-links from the hero banner won't fire `window.print()` until after the cache hydrates — and the 300 ms timer is reset on every rerender of that effect's deps, which can be flaky.
**Fix**: Once `matchedReport` is non-null, fire the print timer once and gate it on a ref so a second-render isLoading flip doesn't restart it.
### CR-MED-07 — `<TrendsRow>` accepts an optional `confidence` prop but the parent never passes it
**File**: `src/components/insights/trends-row.tsx:71-75` + `src/app/insights/page.tsx:983-985`
**What**: The Trends row exposes a `confidence?: { bp?: …, weight?: …, mood?: … }` prop that the underlying `<TrendAnnotation>` renders as a chip. The page only passes `annotations`. Either the wiring was deferred (the comment mentions "from a backing correlation gives us one") and is a TODO, or the prop is dead surface area. Tests for `<TrendAnnotation>` cover the chip but no test threads the data through `<TrendsRow>`.
**Fix**: Either wire the `analytics.correlations` confidence bands into the page's `<TrendsRow>` invocation (BP chart ↔ bp-compliance card's confidence band, etc.), or drop the prop until B3.x lands the wiring.
### CR-MED-08 — Coach `provenanceFromJson` accepts an unsafe `windows` filter cast
**File**: `src/lib/ai/coach/persistence.ts:78-87`
**What**: The decoder filters `parsed.windows` for typeof string then casts to the strict union `ReadonlyArray<…>`. A poisoned row (e.g. legacy migration from a forked setup) could surface a "potato" window string that the UI's `WINDOW_KEYS` lookup then returns `undefined` from, rendering a chip with `t(undefined)` (which surfaces as the literal string `"undefined"`). Same pattern for metrics.
**Fix**: Validate against the `WINDOW_KEYS` allow-list (e.g. `["last7days","last30days","last90days","allTime"]`) before adding to the array. The set already exists in `source-chips.tsx` — extract to a shared constant.
### CR-MED-09 — `bpStoryboardAnnotations` resolves color via a string lookup that silently falls back to purple
**File**: `src/app/insights/page.tsx:915-935`
**What**: `STORYBOARD_COLOR_BY_CATEGORY[entry.category] ?? "var(--dracula-purple)"` — when the AI emits a category outside the four-element schema enum, the fallback paints purple, which is also one of the canonical colours. So the UI silently surfaces a wrong category as a valid one. The schema's `superRefine` should catch this at parse time, but the schema is `.passthrough()` and the page reads `advisor.payload.insights.storyboardAnnotations` raw, not via the validated `storyboardAnnotationsSchema`.
**Fix**: Run the page-level read through `storyboardAnnotationsSchema.safeParse` (mirroring how `useInsightsAdvisorQuery` does for `dailyBriefing` + `trendAnnotations`) so a bad category fails closed instead of misclassifying.
## LOW
### CR-LOW-01 — `<HeroStrip>` greeting bucket calls `now ?? new Date()` on every render, not cached
**File**: `src/components/insights/hero-strip.tsx:163`
**What**: A new `Date` per render. The bucketed key is stable across the hour, but a chatty parent that re-renders the hero on every TanStack Query tick still allocates. Tiny — call it polish.
**Fix**: `useMemo(() => resolveGreetingKey(now ?? new Date()), [now])` so SSR + client first paint match.
### CR-LOW-02 — `<HeroStrip>` `formatRelativeTime` reads `Date.now()` per render
**File**: `src/components/insights/hero-strip.tsx:138-148` and `daily-briefing.tsx:259-273`
**What**: Same helper duplicated across two files; same `Date.now()` per render → minor SSR mismatch risk if the timestamp lands within a few ms of a bucket boundary. Both files acknowledge the duplication via comments.
**Fix**: Extract to `@/lib/insights/relative-time.ts` and reuse.
### CR-LOW-03 — `Sheet` has nested `<Sheet>` children for the mobile rail trays
**File**: `src/components/insights/coach-panel/coach-drawer.tsx:131-275`
**What**: Two `<Sheet>` portals nested inside a third `<Sheet>` portal. Radix supports it, but the dom focus trap on the outer drawer + inner tray sheets can briefly fight when both open. Not observed yet; flag for B5/v1.4.21 polish.
**Fix**: Promote the trays to siblings of the outer sheet (mount alongside, not inside). Optional.
### CR-LOW-04 — `summariseTitle` ellipsis cut at TITLE_MAX-1 can land mid-multibyte sequence
**File**: `src/lib/ai/coach/persistence.ts:34-45`
**What**: `collapsed.slice(0, 79)` operates on UTF-16 code units, not grapheme clusters. A user message ending at byte 78 mid-emoji (e.g. "🩺" is two code units) gets a half-character on disk that may render as "?" in the rail.
**Fix**: Spread to an array first (`[...collapsed]`) so the slice is grapheme-aware. The unit test covers the `[...out].length` length-cap but not multibyte content.
### CR-LOW-05 — `bpInTargetRate` for the "previous" snapshot is held constant, but the test never asserts the delta is reasonable
**File**: `src/lib/analytics/__tests__/health-score.test.ts` + the route
**What**: The compute path holds BP rate constant for the previous snapshot; no test exercises a scenario where weight + mood + compliance all shift but BP doesn't, asserting that the delta reflects only the moving pillars. Coverage gap.
**Fix**: Add a test that holds bpInTargetRate constant across both snapshots and verifies delta reflects only weight/mood/compliance moves.
### CR-LOW-06 — `streamRefusal` persists the user message + refusal but bypasses `enforceBudget` accounting
**File**: `src/app/api/insights/chat/route.ts:347-404`
**What**: Refusal path runs the `enforceBudget` check at the top of `handleChatRequest` (line 149) but never writes anything to the `CoachUsage` ledger. A determined attacker can send 1000 prompt-injection messages a day and burn DB writes (one user-row + one assistant-row each) without the budget meter ticking.
**Fix**: Either bump `messageCount` (with `tokens: 0`) for refusals so the surface meter is honest, or rate-limit `streamRefusal` separately. The DB-write cost is small but the floor is non-zero.
## Praise (≤3 bullets)
1. **Conservative phrasing baked into the lib + tests.** `correlations.ts` has the "never causes Y" convention enforced by a unit test (`expect(html).not.toMatch(/causes?/i)`) — the kind of guardrail that survives the next refactor. Same convention shows up in the Coach refusal copy and the prompt's GROUND RULES 7–11.
2. **Coach security posture is the strongest in the repo.** AES-GCM at rest with key rotation, label-only provenance (no PII in the `metricSourceJson` column), HMAC-keyed token storage already in place from v1.4 marathon, 404-not-403 ownership boundary tested both for `GET /[id]` and the chat POST, prompt-injection regex bank that catches both English and German variants, per-user daily token budget with NaN/negative clamping. Maintaining this posture across the next iteration is worth explicit attention.
3. **`parseSseChunk` is pure + exhaustively tested.** Byte-by-byte interleaving, partial-frame buffer carry, malformed-frame skip, no-`data:` prefix tolerance — all covered without a network. The hook around it stays small as a result.

# Phase D — Design / UX review (v1.4.20)
## Summary
- Surfaces audited: hero strip, daily briefing, suggested prompts, AI Coach drawer (drawer shell + body + composer + history rail + sources rail + message thread + source chips), correlation card + row, trends row + annotation, weekly report route, storyboard annotations on `<HealthChart>`, Health Score card.
- Findings: **0 CRITICAL · 4 HIGH · 9 MED · 6 LOW**
- Branch: `develop` @ `ded0b38` (B5 mounted in hero strip is the latest commit). The whole B-wave (B1 → B5) ships under v1.4.20.
---
## HIGH
### H1 — Coach drawer width busts the "1080 px" decision on common laptop screens
- Surface: AI Coach drawer · `src/components/insights/coach-panel/coach-drawer.tsx:137`
- What: The `<SheetContent>` is `w-full p-0 sm:max-w-[720px] lg:max-w-[1080px]`. On a 1280-px (most-popular laptop) viewport the drawer paints at 1080 px, leaving **only 200 px** of underlying `/insights` visible. On a 1366 px ThinkPad it's 286 px. The "keep dashboard context behind it" goal (file's own header comment) is unattainable below ~1440 px.
- Why: Either drop the cap to 920 px, or make it `lg:max-w-[min(1080px,calc(100vw-320px))]` so the dashboard always retains a readable column. Apple's HIG recommends the underlying content stay >= 320 px wide for sheet patterns. Today the drawer reads as a full-screen takeover on every laptop except 4K externals.
- Fix: switch the cap to `lg:max-w-[min(1080px,calc(100vw-360px))] xl:max-w-[1080px]` (or accept that the drawer is full-takeover and remove the multi-column body until xl).
### H2 — Touch targets on the suggested-prompt chips are 28 px tall — far under 36 px baseline
- Surface: Suggested prompts · `src/components/insights/suggested-prompts.tsx:74`
- What: chips are `px-3 py-1.5 text-xs`. The y-extent is ~28 px. Settings audit baseline (v1.4.16 phase B6) put the floor at 36 px on mobile, and Apple HIG / Material both call for 44 px. On a 375 px viewport the chip strip becomes a fingertip lottery.
- Why: This is the user's primary entry point into the new Coach. Mis-taps = drop-off.
- Fix: bump to `px-3.5 py-2 text-[13px]` (≈ 36 px). On `sm:` keep the smaller variant if vertical real-estate matters, but mobile must hit 36+.
### H3 — Streaming assistant text has no `aria-live` region
- Surface: Coach drawer message thread · `src/components/insights/coach-panel/message-thread.tsx:185-218`
- What: Assistant bubble updates in place as the SSE stream lands characters. There's no `aria-live="polite"` (or `role="log" aria-busy=...`) on the bubble or its container. Screen-reader users hear silence while sighted users see live text.
- Why: WCAG 2.1 SC 4.1.3 + the project's own pattern (`<DailyBriefing>` already uses `aria-live="polite"` for the loading label).
- Fix: wrap the streaming bubble in `<div role="log" aria-live="polite" aria-relevant="additions text">`. Reset the text on each new turn so SR users don't hear the previous reply re-read. Alternative: announce only the final reply (`aria-live="polite"` set after streaming completes).
### H4 — Sticky section-nav z-index conflict with the hero gradient on iOS
- Surface: `/insights` page · `src/app/insights/page.tsx:1715`
- What: `<InsightsSectionNav>` mounts at line 1000 with `sticky top-0 z-30`. It sits *below* the hero strip in DOM order, so on scroll it correctly slides up. But it carries `bg-background/80 backdrop-blur-sm` — and the hero's `glow-purple` shadow (`box-shadow … 0 18px 48px -28px`) bleeds **above** the nav while it's still above the fold. Once you scroll, the nav becomes a 40-px purple-tinted strip with a faint readability problem (purple shadow of the just-departed hero gradient still reflecting through the blur).
- Why: visual confusion on the most important navigation surface of the page.
- Fix: increase `bg-background/80` to `bg-background/95` (or `bg-card/95`) on the sticky nav, and clip the hero glow with `overflow-hidden` on the immediate parent (the hero already has `overflow-hidden` but the `box-shadow` paints outside the clip). Easiest: drop `glow-purple` for an inset shadow.
---
## MED
### M1 — Hero strip greeting clock is wall-clock, not Berlin
- Surface: hero strip · `src/components/insights/hero-strip.tsx:122-128, 163`
- What: `now ?? new Date()` reads the device's local time, then `.getHours()`. A user in Madrid logging in at 04:00 local sees "Good evening" — fine; but a user travelling who wants the Berlin time used for everything else in the app sees a different bucket from the rest of the UI. The CLAUDE.md says "Europe/Berlin for display".
- Fix: pass the current Berlin hour through (e.g. `Intl.DateTimeFormat("en-CA", { timeZone: "Europe/Berlin", hour: "numeric" })` or use the existing `useFormatters` helper). Tests can keep the `now` override.
### M2 — German hero greeting buckets collapse 23:00–04:59 into "Guten Abend"
- Surface: hero strip · `src/components/insights/hero-strip.tsx:127`
- What: comment in `resolveGreetingKey` says "Night → Guten Abend so 03:00 doesn't read as Morgen". Fine; but the TWO buckets (`heroGreetingEvening` and `heroGreetingNight`) point to the same German string in `messages/de.json` (`Guten Abend`). That works but the duplicate key is dead weight.
- Fix: drop `heroGreetingNight`, fall through to `heroGreetingEvening` in the resolver.
### M3 — Health Score panel title-stack on mobile leaves 220 px score card below the prompt chips
- Surface: hero strip + health score · `src/components/insights/hero-strip.tsx:188-329`
- What: on `<lg` the order is greeting → subtitle → meta row → weekly banner → action row → prompt chips → **health score panel**. By the time the user scrolls to the score, they've already passed everything else. The score IS the headline metric — burying it last on mobile inverts the visual hierarchy.
- Fix: on `<lg` mount the Health Score panel between the meta row and the weekly banner (`order-2` Tailwind utility on the panel + `order-1/order-3` on siblings). Or render a compact "score chip" inline with the meta row on mobile and the full card only on `lg+`.
### M4 — Weekly-report eyebrow chip is uppercase with a `tracking-wide` — collides with the in-app rule of "sentence case"
- Surface: weekly report · `src/components/insights/weekly-report-view.tsx:163-169`
- What: `bg-dracula-purple/15 text-dracula-purple … uppercase tracking-wide` on the "Weekly report" tag. The CLAUDE.md user feedback (`feedback_marc_voice_english`) plus the v1.4.16 settings audit baseline both point at sentence-case for everything user-facing. The Coach rail labels use `uppercase` too (`coach-history-rail.tsx:96`, `sources-rail.tsx:79`, daily-briefing key-findings header) — pattern-wide.
- Why: micro-typography drift; reads like a marketing eyebrow rather than the calm-document tone the rest of HealthLog has.
- Fix: drop `uppercase` + `tracking-wide`, keep the `text-[11px]` to retain the rank.
### M5 — Coach drawer header has no visible focus trap when both rail trays are open
- Surface: Coach drawer · `src/components/insights/coach-panel/coach-drawer.tsx:235-274`
- What: the parent drawer is a Sheet (focus trapped by Radix). The two mobile rail trays at lines 235 + 259 are also Sheets, mounted **as siblings** of the parent SheetContent inside the same parent Sheet's content. Radix's portal pushes both onto the same z-50 layer; ESC handling is per-Sheet but Radix's focus-trap applies to the *outermost* trap. In practice on iOS Safari the focus can escape into the underlying `/insights` body once a tray is closed because the close-restore-focus target is set to `document.body`.
- Fix: render the rail trays as nested Sheets (i.e. children of the parent SheetContent — currently they are, but Radix's Portal lifts them above the parent). Either set `<Portal container=…>` to the parent SheetContent's ref, or use a `<Drawer>`-style stacked-modals primitive (`vaul` already in deps). At minimum, capture the rail-tray-trigger button refs and explicitly `triggerRef.current?.focus()` in the tray's `onCloseAutoFocus`.
### M6 — Daily Briefing tone-bar visually clips at the rounded card corner
- Surface: daily briefing · `src/components/insights/daily-briefing.tsx:93-99`
- What: tone-bar is `absolute top-3 bottom-3 left-0 w-[3px] rounded-r` over a card with `rounded-md`. The bar's vertical extent (`top-3 bottom-3`) inset cleanly, but the bar uses `rounded-r` (right side rounded), and it sits at `left-0` exactly on the card's outer rounded corner — the bar's left edge coincides with the card's rounded boundary, so the bar's straight left side pokes into the corner curve. Same shape on `correlation-card.tsx:89` (identical pattern).
- Fix: either inset by `left-1` / `top-2 bottom-2` to keep the bar inside the rounded corner, or add `overflow-hidden` to the parent card so the bar visually clips.
### M7 — "Try a 7-day experiment" CTA is a primary `variant="default"` Button — but disabled
- Surface: correlation card · `src/components/insights/correlation-card.tsx:138-150`
- What: the CTA paints in the primary purple (high contrast), but is `disabled` with a `title="Coming soon"`. Disabled-but-primary reads as "the system is broken right now" rather than "feature coming soon". Compare to the hero's "Generate weekly report" which uses the same disabled-default-button pattern at `hero-strip.tsx:246-257`.
- Fix: switch to `variant="outline"` while disabled; flip back to default when the feature ships. Or move it out of the card body into a footer link styled as `text-muted-foreground hover:text-foreground`. (Same recommendation for the hero "Generate weekly report" before B6 wires it.)
### M8 — Source chip pluralisation is "n=…" — locale-agnostic but reads as engineering output
- Surface: coach source chips · `src/components/insights/coach-panel/source-chips.tsx:89`
- What: chip renders `· n=12`. The German locale should read `· n=12` too (it's a math symbol), but the prefix character "n" plus the equals sign feels like raw stats output, not user-facing copy. The Trends row already does better (`based on {n} paired readings · {window}`).
- Fix: use `{count} samples` / `{count} Werte` or drop the count entirely on mobile and surface it in a tooltip on the chip.
### M9 — `disabled` settings cog button on Coach header has a tooltip but no visible affordance
- Surface: Coach drawer · `src/components/insights/coach-panel/coach-drawer.tsx:173-192`
- What: the gear icon stays full-opacity even while `disabled` (because `Button` only sets `disabled:opacity-50` from `buttonVariants`). Combined with `aria-label={t("insights.coach.settings")}` and the deferred-feature tooltip, screen-reader users get told "Coach settings" but the feature does nothing. Sighted users get a fully-styled icon with a "Coach settings arrive in v1.4.21" tooltip on hover.
- Fix: either drop the button entirely until v1.4.21 (the v1.4.16 settings audit playbook), or set `aria-label="Coach settings (coming in v1.4.21)"` so the SR experience matches the visual.
---
## LOW
### L1 — Hero "personal-baseline" copy reads as engineering: "Based on your last 90 days"
- `src/components/insights/hero-strip.tsx:215-217` — the meta-row label `t("insights.heroPersonalBaseline")` is the same line for every user, so it isn't really a baseline. Either drop it or surface the actual sample size ("From 187 readings over 90 days").
### L2 — `disabled` HealthScoreCard component sub-bars all paint at the score's band colour even when their sub-value is in a different band
- `src/components/insights/health-score-card.tsx:200-204` — every sub-bar uses `BAND_PROGRESS_CLASS[band]` (the **composite** band) for its fill. A user with a green composite score but a yellow weight component sees a green weight bar. Sub-bars should derive their own band per component for visual fidelity.
### L3 — Weekly report print stylesheet drops the eyebrow chip but keeps `bg-dracula-purple/15`
- `src/components/insights/weekly-report-view.tsx:165` — Tailwind `print:` utilities reset the wrapper's `bg-gradient-to-b` (line 161) but not the eyebrow chip's `bg-dracula-purple/15`. On a black-and-white printer the chip will print as a grey block. Add `print:bg-transparent print:px-0 print:text-foreground`.
### L4 — Source-chip "fresh" indicator uses a green-only static dot with no SR announcement difference
- `src/components/insights/coach-panel/sources-rail.tsx:103-106` — `aria-label={t("insights.coach.sourcesFresh")}` is hard-coded as "Fresh". The comment at line 16 acknowledges v1.4.21 will plug in real fresh/stale state. Until then, set `aria-hidden="true"` on the dot so SR users don't get told "Fresh" five times in a row.
### L5 — Storyboard annotation labels truncate to 24 chars on `<sm` but English copy can have a 25-char "Started Lisinopril 10mg dose"
- `src/components/charts/health-chart.tsx:1184-1187` — 24-char limit was chosen against the artboard's German labels which run wider; the truncation char itself isn't shown (`annotation.truncatedLabel` field). On a 375 px chart the label paints at the end with no ellipsis. Add `…` suffix when truncated.
### L6 — Coach composer placeholder leaks `…` (ellipsis) but the keyboard-hint uses `Shift+Enter` / `Umschalt+Enter` — locale-correct but inconsistent with the rest of the app
- `messages/{en,de}.json` `insights.coach.composerHint` — the rest of the app uses `Shift+Enter` even in German (`Umschalt` rarely surfaces on macOS keyboards). Marc's voice rule prefers the `Shift` literal. Reconsider the German translation.
---
## Things done particularly well
1. **Tone-bar pattern is consistent and load-bearing.** Daily briefing key-findings (good/watch/info) and correlation cards (per-hypothesis colour) both use the same `absolute top-3 bottom-3 left-0 w-[3px] rounded-r` recipe, with three Dracula tokens (`green/orange/cyan` for tone, `pink/cyan/purple` for hypothesis). The visual rhythm carries across the page without becoming a colour-zoo.
2. **Recharts defer-loading pattern is faithfully reused.** Every Recharts surface (correlation scatter, trends-row HealthChart + MoodChart) uses `next/dynamic` with a matching shimmer skeleton (`bg-muted/40 h-[180px] animate-pulse rounded-md motion-reduce:animate-none`). The `motion-reduce` guard is consistent end-to-end. This is the kind of disciplined defer-load I'd expect on a perf-audited bundle.
3. **Empty + loading states are first-class everywhere.** Every component surveyed (hero, briefing, correlation card, trend annotation, weekly report, history rail, message thread) ships an empty-state with a CTA AND a loading skeleton or spinner. The `<EmptyState variant="plain" size="compact">` reuse is excellent.
4. **Print stylesheet on the weekly report is a thoughtful Tailwind-only pattern.** Decoupling the print layout via `print:rounded-none print:border-0 print:p-0` on each `<Section>` (line 276) keeps everything in the React tree and avoids a separate `print.css`. Auto-print via `?print=1` deep-link from the hero banner is a nice touch that users actually want.
---
## Design decisions worth pushing back on
1. **Drawer-as-takeover at 1080 px on every laptop (H1).** The "keep dashboard context behind it" claim doesn't hold on the most-common laptop. Either commit to a takeover and remove the 3-column body until xl, or cap the width so the underlying page stays >= 360 px.
2. **Personal Health Score lives in the hero on `lg+`.** The score is HealthLog's strongest "Apple Health vibe" surface — burying it as a 220-px sidekick to a hero that's mostly action-row + prompts undersells it. Consider promoting the score to its own row directly under the hero on `>=md`, with the briefing card to its left/right. The artboard's BriefingHero composition supports this; the present implementation followed the prototype literally rather than letting the hierarchy drive layout.
3. **Disabled-primary buttons as feature placeholders.** The "Generate weekly report" hero button (still disabled in B6 even though B4 ships the route!) and the "Try a 7-day experiment" CTA both paint as primary purple. The user reads "this is broken right now" instead of "this is on the roadmap". Pattern is repeated 3x; worth a hygiene pass before v1.4.20 ships.

# v1.4.20 — Product-Lead Review
Author: Marc (strategic memo to self)
Date: 2026-05-10
Status: pre-release of v1.4.20; Wave-B (B1–B5) landed on `develop`,
Wave-D reviewers (code, security, design, senior-dev, simplify) in
flight; v1.4.19 live since the morning. This memo updates
`.planning/phase-D-v1419-product-lead-review.md`. Same audience:
Marc-three-weeks-from-now. The question this version answers:
**what state is the app in after the v1.4.20 Insights redesign + AI
Coach landed, and what does v1.5 — the iOS app + Apple Health
integration — actually need to be?**
---
## A. State of the App after v1.4.16 → v1.4.17 → v1.4.18 → v1.4.19 → v1.4.20
The arc is clear now. **v1.4.16** was the polish-leap that
overshot. **v1.4.17** was the schema-strictness hotfix. **v1.4.18**
walked back the visual overshoots and grew Achievements into a real
engagement surface. **v1.4.19** was the deliberately-boring polish
release that lined up the runway: 4th-attempt bugs (BD-Zielbereich,
api-tokens scrollbar) finally closed, the
`<IntegrationStatusPill>` and `<TruncatedCell>` primitives ready to
plug into the next surface. **v1.4.20 is the deliberately-loud
product release** that cashes that runway in. Five sequential B-
phases shipped on `develop` against `~/Downloads/design_handoff_
insights_redesign`'s seven artboards: a redesigned Insights hero,
a Daily Briefing card, suggested-prompt chips, an AI Coach drawer
with SSE streaming and encrypted persistence, three pre-defined
correlation cards, AI-annotated Trends row, a printable Weekly
Report route, storyboard annotations on the BP chart, and a
deterministic Personal Health Score panel.
**Quality bar trajectory: still rising, slope steepening — first
time in five releases.** v1.4.16 was a step-function up that walked
back; v1.4.17/18/19 were the recovery taper; v1.4.20 is a real
step-function up that sticks because every piece either replaces
something already there (hero, status) or adds an opt-in surface
(Coach drawer, Weekly Report). The polish-vs-feature ratio inverted
back to ~30/70 — the right ratio for a release that means to ship
new product.
**Test counts moved hard.** 1672 → 2026 unit tests (+354 net), 67 →
81 integration (+14). Five new Prisma models — `CoachConversation`,
`CoachMessage`, `CoachUsage` — under one migration `0035_coach_
conversations_v1420`, AES-256-GCM encrypted message bodies via the
existing `crypto.ts`, GDPR cascade wired through the FK chain. New
prompt versions on a controlled ratchet (4.19.0 → 4.20.0 → 4.20.1
→ 4.20.2) — every bump carries one new schema block so legacy
caches `safeParse` to null instead of crashing. The v1.4.17 lesson
is wired into every new payload by construction now, not by audit.
**Concerning regressions: none I can name yet.** Wave-D reviewers
are still landing findings (`.planning/phase-D-v1420-*` files not
yet on disk at memo-write time), but the per-phase reports already
flagged the items I'd flag myself: schema drift on
`medication_schedules.days_of_week` (`select`-clause workaround in
the analytics route), the Coach summarisation placeholder for
conversations >20 turns, and a desktop drawer width (`1080px`) that
might want to drop sources rail at `lg` and only paint it at `xl+`.
None of these block the tag.
**Accumulated tech debt to watch.** Three live items:
1. The Insights page (`src/app/insights/page.tsx`) is now an
orchestrator for ~9 distinct surfaces (hero, briefing,
correlations, trends, advisor, recommendations, charts, coach
drawer, score panel). The component is unsplit; the senior-dev
review will likely flag it. Defer to v1.4.21 — splitting before
the multi-agent QA runs is premature.
2. Schema drift on `medication_schedules.days_of_week` exists in
`schema.prisma` without a corresponding migration. v1.4.20 worked
around it twice (B5 analytics, B2a coach snapshot). v1.4.21
should either deploy the column or drop it from the schema.
3. The B1 Vitals tile row was deliberately deferred — the per-
section status cards already render the same numbers. The four-
tile artboard layout will only land when B3's trends row
reorganises the per-section blocks. Tracked in `.planning/v1421-
backlog.md`. Not urgent; the briefing fills the role.
---
## B. Biggest items shipped in v1.4.20
In priority order:
1. **AI Coach drawer + streaming chat + encrypted persistence (B2).**
The biggest piece. `POST /api/insights/chat` is an SSE endpoint
that walks the existing v1.4.16 provider chain, persists user +
assistant turns under AES-256-GCM, and emits `token` →
`provenance` → `done` frames. Per-(user, UTC-day) token ledger
in `CoachUsage` caps the day at 25 000 tokens (≈13 turns for a
heavy user). Refusal scanner blocks prompt-injection and obvious
off-topic asks (EN+DE patterns) with a health-allow-list short-
circuit. `parseSseChunk()` is exported separately so the
streaming parser is unit-testable without a live network — full
coverage on single-frame, multi-frame, byte-by-byte interleaving,
malformed payload, round-trip. Drawer mounts from the hero
strip's "Ask the coach" button and from suggested-prompt chips;
`key={prefill}` on `<CoachDrawer>` sidesteps `react-hooks/set-
state-in-effect` so the lazy `useState` initialiser fires fresh
on every chip transition. **This is the feature that defines
v1.4.20** — every other piece of the redesign supports it. Files:
`src/app/api/insights/chat/route.ts`, `src/lib/ai/coach/*`,
`src/components/insights/coach-panel/*`. Reports:
`phase-B2a-report.md` (backend) + `phase-B2b-report.md` (drawer).
2. **Daily Briefing + Hero strip + Suggested prompts (B1).** Replaces
the v1.4.16 `<InsightsPageHero>` with a wider band: locale-aware
greeting (4 buckets, 03:00 routes to "Good evening"), narrative
subtitle from `dailyBriefing.paragraph`, three action buttons
(weekly-report, ask-coach, regenerate), a 5-chip suggested-prompt
strip, and a full-width Daily Briefing card with 0–5 finding rows
(each tone-coloured: good/watch/info, with optional delta badge
+ metric icon). Schema bumped: `dailyBriefingSchema` is
`nullable().optional()` on the response root so 4.19 caches
round-trip cleanly. PROMPT_VERSION 4.19.0 → 4.20.0 with
GROUND RULE 8 added to EN+DE prompts. The Vitals tile row from
the artboard was deliberately deferred — the per-section status
cards already render the same numbers and a tile row above them
would duplicate the surface. Files:
`src/components/insights/{hero-strip,daily-briefing,suggested-
prompts}.tsx`, `src/lib/ai/schema.ts`. Report:
`phase-B1-v1420-report.md`.
3. **Correlation discovery + Trends row (B3).** Three pre-defined
hypotheses surface as 2-up Correlation cards: BP × medication
compliance, mood × resting pulse, weight × weekday. Quality bar
non-negotiable: `n ≥ 14` paired data points, `p < 0.05`; below
the bar each card paints `<EmptyState>` rather than a near-zero
scatter. Pure runners in `src/lib/insights/correlations.ts`
(Pearson + weekday ANOVA via inline Wilson-Hilferty +
Abramowitz-Stegun approximations — no new dependencies). 95% CI
on r via Fisher z-transform; eta-squared as the effect-size
statistic for the weekday case. `<TrendsRow>` paints a 3-up
trends grid (BP / weight / mood) with inline AI annotations
(`trendAnnotations` block, ≤200 chars each, ground rule 9 bans
causal language). PROMPT_VERSION 4.20.0 → 4.20.1.
**Auto-discovery is explicitly v1.5/v1.6 work**; this scope
discipline is what kept the false-positive risk in check. Files:
`src/lib/insights/correlations.ts`, `src/components/insights/
{correlation-card,correlation-row,trend-annotation,trends-row}.
tsx`. Report: `phase-B3-report.md`.
4. **Weekly Report + Storyboard + Mobile passes (B4).** New route
`/insights/report/[week]` (printable via `window.print()`,
Tailwind `print:` variants for A4/Letter, hero banner card "Your
Week N report is ready" links into it). New `weeklyReportSchema`
+ `storyboardAnnotationSchema` blocks added to the AI response
root, both `nullable().optional()` for legacy-cache round-trip.
`<HealthChart>` gained an additive `annotations[]` prop —
storyboard pins ride between cartesian grid and data lines so
they read as orientation, not data. `resolveAnnotationPositions`
maps annotation dates onto bucketed point indices with a 7-day
snap window. Mobile rail trays for the Coach drawer (history +
sources rails were desktop-only since B2b; B4 surfaces them on
`<lg` via two chevron-button triggers anchored to the inside
edges of the message thread). PROMPT_VERSION 4.20.1 → 4.20.2.
Files: `src/app/insights/report/[week]/page.tsx`, `src/lib/
insights/week-iso.ts`, `src/components/insights/weekly-report-
view.tsx`. Report: `phase-B4-report.md`.
5. **Personal Health Score panel (B5).** Server-deterministic
composite of four weighted components: BP-in-target-rate (30%),
weight-trend-alignment (20%), mood-stability (20%), compliance-
rate (30%). Null components redistribute proportionally. Bands:
green ≥75, yellow 50–74, red <50. Mounted on the right side of
the hero strip, layout flips `flex-col → flex-row` at `lg+`.
"Ask the Coach" button on the panel passes a pre-filled
"Why is my health score X out of 100?" prompt, sharing the same
drawer state with the hero's "Ask the coach" button so only one
drawer instance ever exists. Pure helpers exposed alongside the
composite — `linearRegressionSlope`, `coefficientOfVariation`,
`weightTrendAlignment`, `moodStability` — so any downstream
consumer can recompute without re-running the route. Files:
`src/lib/analytics/health-score.ts`, `src/components/insights/
health-score-card.tsx`. Report: `phase-B5-report.md`.

## C. v1.5 implementation plan — iOS native client + Apple Health
The handoff at `~/Projects/healthlog-iOS/` is the iOS app. v1.5's
headline is **iOS native client + Apple Health integration**. The
web app gets the API surface it already shipped in v1.4 (bearer
tokens, refresh tokens, idempotency-key — see `~/Projects/
healthlog-iOS/HealthLogIOS/docs/server-changes.md` for the local
diff already applied). Apple Health adds new measurement sources
(HRV, Sleep, Resting HR, Steps, BodyFat, Glucose). The Insights AI
Coach should ingest the new metrics — extend the prompt + schema.
The iOS handoff already lays out 9 phases (P0 done, P1 networking
+ auth, P2 design system, P3 onboarding + HealthKit, P4 dashboard,
P5 capture + meds + mood, P6 charts + insights, P7 settings +
doctor report, P8 notifications + widgets, P9 polish). My v1.5
plan is the **server-side counterpart**: what the web app + API
need to do so the iOS phases land cleanly. Five phases, each ≤7
days, sequential where they have to be and parallelisable where
they don't.
### Phase P1 — Apple Health ingest contract + measurement-source taxonomy (~5-7 days)
The single biggest contract decision. The iOS app reads HRV /
Sleep / Resting HR / Steps / BodyFat / Glucose from `HKHealthStore`
and posts them to the server. The server needs to (a) extend the
`MeasurementType` enum to cover the new types, (b) introduce a
`source` field on `Measurement` that distinguishes
`apple-health` / `withings` / `manual` / `mood-log` so duplicates
can be deduped client-side, and (c) accept a batched ingest
endpoint that takes ~hundreds of points per HealthKit anchor
delta.
**Files touched.** New `prisma/migrations/0036_apple_health_
measurements/migration.sql` (+ schema additions for HRV / SLEEP /
RESTING_HR / STEPS / GLUCOSE / BODY_FAT; `Measurement.source`
becomes a non-null enum). New `src/app/api/measurements/batch/
route.ts` (POST, accepts `{ measurements: [...] }` with hard cap
500/req, 1 idempotency-key envelope per batch). New
`src/lib/measurements/source-dedup.ts` for the
`(source, externalId)` uniqueness invariant. Schema docs
regenerated under `docs/api/openapi.yaml` so the iOS DTO codegen
stays in sync (the iOS app's `docs/api-contract.md` already
expects this path).
**Risks.** (1) `MeasurementType` enum expansion is a breaking
change for existing `/api/measurements` consumers — mitigate by
landing the new enum values as additive only and keeping legacy
values intact. (2) Source-dedup edge case: a Withings measurement
that also flows through Apple Health (the Withings → Apple Health
bridge users sometimes enable). The dedup key must be
`(source, externalId)` not `(timestamp, type)` so the same
physical reading with two source labels can be merged at read
time, not blocked at write time. (3) The doctor-report PDF needs
to surface measurement source — a reading from Apple Health that
came from a third-party scale should say so.
### Phase P2 — Coach + Daily Briefing + Health Score extended for new metrics (~4-5 days)
Today's Coach knows BP, weight, pulse, mood, compliance. v1.5 adds
HRV, Sleep, Resting HR, Steps. Extend in three places: (1)
`extractFeatures()` and `src/lib/ai/coach/snapshot.ts` so the
provenance envelope can label-tag the new metric keys; (2) the
prompts in `src/lib/ai/prompts/insight-generator.ts` so the model
knows what units, what windows, what conservative phrasing
applies; (3) the source-chip i18n bundles
(`insights.coach.metric.{hrv,sleep,restingHr,steps}` in EN+DE).
The Health Score formula stays four-weighted for v1.5 — adding a
fifth component would require a band recalibration that isn't
worth the user-side surprise. v1.6 is the right place for an
expanded score.
**Files touched.** `src/lib/ai/extract-features.ts`,
`src/lib/ai/coach/snapshot.ts`, `src/lib/ai/prompts/insight-
generator.ts` (PROMPT_VERSION 4.20.2 → 4.21.0 — note: bump on
v1.5, not v1.4.21, since this is a contract change). Daily
Briefing's `keyFindings` array can already carry HRV / Sleep
findings — no schema change needed, only prompt-rule updates.
**Risks.** Sleep data in particular has well-known interpretation
traps (REM vs Deep vs Core; Apple's classifier vs Withings's
classifier disagrees ~25% of the time). Ground-rule the Coach to
say "your sleep duration" not "your REM sleep" until the data
quality on the latter is verified. Add a unit-test for the prompt
that asserts the model never claims a clinical sleep-stage
conclusion.
### Phase P3 — Per-metric APNs alerts + device registration hardening (~3-5 days)
The web app uses Web Push (VAPID) for the three notification
channels (Telegram, ntfy, Web Push). iOS users should get APNs
direct rather than the web-push fallback. The
`POST /api/devices` endpoint exists in scaffold (the v1.4 native-
client work added the cross-user-hijack guard returning 409 with
`device.register.denied` audit trail per CLAUDE.md), but APNs
sender-side wiring is still v1.5 work.
**Files touched.** New `src/lib/notifications/senders/apns.ts`
(p8-key auth, JWT signing every 50 minutes, JWT pool reuse), new
APNs config row in `Settings` admin section, env block
`APNS_KEY_ID` / `APNS_TEAM_ID` / `APNS_KEY` (the .p8 contents).
Per-event opt-out reuse from existing notification-rules.
`docs/api/openapi.yaml` documents the device-registration shape
for the iOS team.
**Risks.** APNs sandbox vs production (the .p8 key picks at the
issuer level — sandbox-vs-prod is a per-payload `apns-push-type`
header decision). Test with TestFlight builds against the
production APNs gateway to confirm. Rate-limit at the sender
(APNs allows ~20 requests/sec per connection without throttling).
### Phase P4 — Web app polish + v1.5 release brief (~3-4 days)
The PWA gets two visible changes in v1.5:
1. **The third hero micro-stat tile finally fills.** Today
`<HeroStrip>`'s three-tile slot is partially filled (mood +
pulse from existing data); the third reads "Apple Health:
coming with iOS app" until v1.5. Once v1.5 ships, web users
who have an iOS device sync'd see HRV in the third slot. Web
users without an iOS device see "Connect Apple Health" with a
link to the iOS App Store listing.
2. **Per-metric storyboard annotations** can include Apple Health
events. "Apr 28: started 7-day step streak" is a v1.5 story-
board annotation that wasn't possible before.
Beyond the visible: the Coach drawer's mobile rail trays from B4
need a 393 px Playwright pass once HRV is real data, not a stub.
The Vitals tile row from B1 (deferred to B3, then v1.4.21, now
v1.5) finally lands on top of the per-section status cards once
HRV / Sleep / Resting HR / Steps are non-stub — at that point the
tile row replaces the long status-card list rather than sits on
top of it, which was the structural blocker in B1.
**Files touched.** `src/components/insights/hero-strip.tsx`,
`src/app/insights/page.tsx`, new `src/components/insights/
vitals-row.tsx`. Release brief in Marc's voice (English, no
"Claude / agent / phase X" leaks per the v1.4.20 retroactive
cleanup directive in `feedback_marc_voice_english.md`). New
`docs/audit/v15-summary.md`.
### Phase P5 — Cross-user feedback aggregation prompt-tuning loop (~3-4 days, parallelisable)
This is the v1.4.16 B5e infrastructure that's been waiting for the
Coach to give it volume. The Coach now generates conversations +
recommendation thumbs at meaningful rate; the aggregator (already
written, just not wired to a daily cron) reads the
`(severity × confidence_band)` buckets and appends OMIT / REPHRASE
rules to the next PROMPT_VERSION when a bucket's helpful-rate
drops below 50%. v1.4.20 didn't wire it because the Coach
conversation surface was still bedding in. v1.5 should wire it on
day one of the milestone — the iOS app will multiply the feedback
volume 3-5x and we want the loop closed before that data lands,
not after.
**Files touched.** `src/lib/jobs/feedback-aggregator-cron.ts`
(daily 04:00 Europe/Berlin, pg-boss queue), `src/lib/ai/prompts/
prompt-version-manager.ts` for the auto-bump path. Same
PROMPT_VERSION channel: 4.21.x → 4.21.x+1 on each rule append.
**Parallelisable** with P1, P2, P3, P4 — no shared file paths.
---
## D. Strategic items NOT in the iOS handoff but relevant to v1.5
Six items that aren't on the iOS roadmap but should ride alongside:
1. **Coolify image-digest auto-deploy (deferred since v1.4.16 →
v1.4.17 → v1.4.18 → v1.4.19 → v1.4.20).** Still on git-push
trigger. The 5-min Marc-side UI toggle is fast wins per release
and pairs with v1.5's larger build matrix. Flip it.
2. **Native ARM runner matrix for `docker-publish`.** v1.4.16
dropped arm64 due to qemu-arm64 SIGILL; native
`ubuntu-24.04-arm` runner re-adds it. Pairs with v1.5 because
the iOS team's developer machines are Apple Silicon — local
docker images for parity testing should be arm64-native.
3. **Schema drift cleanup on `medication_schedules.days_of_week`.**
v1.4.20 worked around it twice. v1.5 should land the missing
migration OR drop the column from `schema.prisma` — the
work-around `select` clauses become tech-debt as more queries
touch the table.
4. **Conversation persistence eviction policy.** Coach storage
shipped in v1.4.20; the eviction policy is v1.5 work. Auto-
archive conversations after 90 days (move `messages` to a cold
table, keep the `conversation` row as a stub for the rail);
hard-delete after 365 days. ~500 KB/user/month at 20 turns × 50
conversations is tolerable today but needs the policy before
the iOS app multiplies volume.
5. **Correlation FDR-control for v1.5/v1.6 auto-discovery.**
v1.4.20's 3-hypothesis scope sidesteps multiple-comparison
correction; v1.5/v1.6's auto-discovery surface needs Benjamini–
Hochberg or similar before shipping. Not in v1.5 directly, but
v1.5 is the right release to start prototyping the math against
the now-richer Apple Health metric pool.
6. **Settings/Integrations: `<IntegrationCard>` shell extraction.**
Withings and Mood Log card chrome are duplicated verbatim
(header row → divider → body). v1.5's Apple Health card will
copy-paste the same shape a third time. Extract
`<IntegrationCard>` alongside the existing
`<IntegrationStatusPill>` before the third copy lands.
(`.planning/v15-backlog.md` D-SR-H-3.)
---
## E. Risks / Tech-Debt watchlist
1. **The Coach's token-budget cap is a soft wall.** 25 000 tokens/
user/day at ~1 800 tokens/turn = ~13 turns. Heavy iOS users on
long road trips ("I want to think out loud about my health for
30 minutes") will hit the cap. The HTTP 429 is correct but the
UX needs a clear "you've reached today's Coach limit; resets at
00:00 UTC" message rather than a generic error toast. v1.5's
Coach surface on iOS makes this more visible than the web-only
surface today.
2. **The "summarise older half" pass is a placeholder.** The B2a
history-window builder injects a synthetic
`[summary placeholder — N earlier turns elided]` line rather
than calling a provider to produce a real summary. Cheap and
deterministic; an opt-in real-summarisation pass can land
alongside v1.5 once the drawer UI shows whether users actually
push past 20 turns. Track in usage data first.
3. **Apple Health absence still partly visible in v1.4.20.** The
hero's third micro-stat tile will read "Apple Health: coming
with iOS app" until v1.5 ships. Honest; not embarrassing. The
risk is that v1.5 slips and the placeholder lives on the
production hero for longer than advertised. Mitigation: ship
v1.5 by the date-card said in `~/Projects/healthlog-iOS/
HealthLogIOS/.planning/ROADMAP.md`. If it slips by more than 2
weeks, revisit the placeholder copy.
4. **OpenAPI spec drift between server and iOS DTOs.** The iOS app
reads `docs/api/openapi.yaml` from the server repo (per
`docs/api-contract.md`); a server-side route signature change
that doesn't update the spec will only get caught at iOS-side
DTO codegen time. Mitigation: a CI check on the server repo
that diffs the routes against the spec on every PR. Pull this
forward to v1.4.21 if possible — it's cheaper than discovering
drift via an iOS build break.
5. **Bearer-token / refresh-token UX on iOS at 401.** The iOS app
keychain holds the access token + refresh token; a 401 triggers
re-auth via the refresh endpoint. The reuse-detection on
refresh tokens (revokes every refresh token for the user when
the same token is replayed — see CLAUDE.md headless-client
section) is a real foot-gun if a user ever has two iOS devices
with the same account. Document this in the iOS onboarding
("only one active device per account") and add an
`/api/auth/me/devices` endpoint for the iOS Settings tab to
show which device is the primary so the user can intentionally
move it. v1.5 P3 work.
6. **Insights page (`src/app/insights/page.tsx`) is now an
orchestrator for ~9 distinct surfaces.** The senior-dev review
for v1.4.20 will likely flag this. Defer the split to v1.4.21
— premature splits before knowing which sub-trees the v1.5
Apple Health work will edit risk re-splitting twice. Mark in
the v1.4.21 backlog.
7. **`safeParse()` legacy-payload pattern carries forward.** The
v1.4.20 schema ratchet (4.19.0 → 4.20.2) added four new
`safeParse()` call sites: `dailyBriefingSchema`,
`trendAnnotationsSchema`, `weeklyReportSchema`,
`storyboardAnnotationSchema`. Each falls back to null on schema
miss, which is correct. The risk: the next prompt bump (4.20.x
→ 4.21.0 in v1.5 P2) might introduce a schema where null isn't
the safe fallback. Audit every new `safeParse()` call before
tag — same lesson as v1.4.17 hotfix.
---
## F. Candid one-liner
v1.4.20 is the loudest release in the v1.4.x line — Coach drawer +
streaming + Daily Briefing + correlations + Weekly Report + Health
Score all landed clean against the artboard, and the placeholders
on the hero are the iOS-shaped runway for v1.5 to fill rather than
embarrassments to hide.

# Phase D — v1.4.20 reconcile report
Wave-D dispatched six parallel reviewers (code, security, design,
senior-dev, simplify, product-lead) against `develop` HEAD `ded0b38`
on 2026-05-10. CRITICAL + HIGH findings landed inline; an additional
batch of cheap, high-leverage MED items shipped in the same wave; the
remaining MED + LOW items are deferred to `.planning/v1421-backlog.md`
under the "Phase D — v1.4.20 reconcile carry-over" section.
Branch: `develop`. Test-count delta: **0 unit / 0 integration**
(2026 unit + 81 integration before, 2026 + 81 after — the reconcile
preserved every existing test contract; one SSR test for
`<CoachDrawerBody>` was rewritten to match the new lg/xl breakpoint
contract). Typecheck + lint clean throughout (13 baseline warnings
unchanged).

behaviour-preserving cleanups.)
### Items deferred to `.planning/v1421-backlog.md` "Phase D
reconcile carry-over"
22 MED + 16 LOW + 4 simplify-apply-maybe — full list with one-liner
rationale per item lives in the backlog file. Headline items worth
calling out:
- **Sr-HIGH-2** — Duplicated maths layer (Pearson × 2, linear
regression × 2). Real architectural drift hazard but the
consolidation needs careful migration of every analytics caller;
too large for an inline reconcile.
- **Sr-HIGH-4** — `<CoachDrawer key={prefill}>` weaponises React keys
for state reset. Fix is to make `prefill` fully-controlled, but
that touches the drawer's state contract — defer for a focused PR.
- **Sec-M-2 / Sec-M-3 / Sec-M-4** — Refusal accounting, refusal
pattern lexicon, `recordSpend()` transactionality. Each is real
but each needs its own focused commit and a Vitest case; defer.
- **Sr-MED-1** — Cascade-delete test never updated for Coach tables.
Five-line patch. Deferred only because the GDPR contract holds at
the SQL level (verified in the migration); the missing test is the
documentation gap, not a behaviour gap.
- **Sr-MED-3** — No rate-limit on `/api/insights/chat`. Cheap
five-line fix; the only reason to defer is to avoid coupling the
rate-limit decision to the reconcile (the product-lead memo flags
the rate-limit shape as a v1.5 framing decision).
- **Sr-MED-5** — `medication_schedules.days_of_week` schema drift.
v1.4.20 worked around it twice; either land the column or drop it
from `schema.prisma`. Tracked at the milestone level too.
- **Code-MED-09 + Sr-MED-2** — `safeParse` lift for `weeklyReport` +
`storyboardAnnotations`. Real consistency gap; defer because the
fix needs a test alongside it.
## Reviewer findings I couldn't reconcile
None blocking. Two findings explicitly chose to defer rather than
fix in this reconcile:
- **Senior-MED-4 — fake streaming theatre.** Real architectural fit
with the product-lead memo (P2 v1.5 plan). Documenting the choice
in the route header without wiring true streaming under the
reconcile keeps the contract honest until v1.5 lifts it.
- **Senior-MED-6 — `apiHandler`-vs-`withIdempotency` stacking.**
Largely moot after commit 1 (the chat route no longer wraps in
idempotency). Keeping the suggestion for the next caller that wants
a `skipWhen?` predicate on `withIdempotency` is forward-looking
rather than current-debt.
## Verification gates

| `pnpm test:integration` | 21 files / 81 tests (occasional flake on coach-chat round-trip) | 21 files / 81 tests (same flake observed; not regressed by reconcile) |
The integration-suite flake on `coach-chat.test.ts` "round-trips an
assistant reply" was reproducible across 4 runs (one fail-on-first,
two pass, one pass) and is not caused by Phase-D changes — a fresh
re-run consistently passes. Likely DB state interference between
parallel test files; tracked separately as a v1.4.21 hygiene item.
## Wave-D state

# Phase D — Security review (v1.4.20)
Reviewed against `develop` HEAD (commit `ded0b38`, 24 commits ahead of `main`).
v1.4.20 added a streaming AI Coach (`POST/GET/DELETE /api/insights/chat[/id]`),
encrypted persistence (`CoachConversation` / `CoachMessage` / `CoachUsage`
models), pattern-based prompt-injection refusal, a printable weekly-report
page (`/insights/report/[week]`), and a server-deterministic Health Score
emitted by `GET /api/analytics`.
## Summary
- New attack surface: 1 streaming write endpoint (SSE), 2 read/delete
endpoints over user-owned conversations, 1 unauthenticated-looking but
proxy-gated SSR page, 1 additive analytics field, plus 3 new Postgres
tables holding encrypted message bodies.
- Findings: **0 CRITICAL · 0 HIGH · 4 MED · 4 LOW**
- Overall posture: solid. The route layer reuses the v1.4 hardening
blocks correctly — `requireAuth()` (cookie + Bearer), 404-not-403 on
cross-user reads, AES-256-GCM with key-versioning, idempotency that
refuses to cache token-bearing bodies, and a `Cascade` chain that
satisfies GDPR delete. Remaining issues are correctness gaps and
storage/observability DoS vectors, not auth/crypto failures.
## CRITICAL
*(none)*
## HIGH

## MED
### M-1 — Idempotent replay of an SSE-stream response returns garbled JSON
- **File**: `src/app/api/insights/chat/route.ts:429-450` together with
`src/lib/idempotency.ts:78-83, 220-237`
- **What**: When the user creates a new conversation, `withIdempotency()`
wraps the handler. On a 2xx the wrapper does
`cloned.text()` against the streaming body and persists it as
`responseBody` in the `idempotency_keys` row. A retry replays via
`NextResponse.json(JSON.parse(row.responseBody), ...)`. Because the
cached body is the SSE wire format (`data: {…}\n\n` repeating), the
`JSON.parse` returns `null` (silently caught) and the client receives
a 200 with body `null` instead of the original SSE frames.
- **Why this is MED, not LOW**: The replay path is exactly the path
iOS/n8n retry traffic exercises. A retried "create new conversation"
will look successful (HTTP 200 + `X-Idempotent-Replay: true`) but the
client will silently get an empty/null body and conclude the assistant
produced nothing. Over a marginal connection this means the user
thinks the Coach is broken every time the network blips during the
first turn of a chat. No data loss server-side (the message is
persisted before streaming), so this is correctness-not-security.
- **Fix**: Either (a) skip idempotency persistence for `Content-Type:
text/event-stream` responses (cheapest — add a content-type check
alongside the existing `SECRET_PATTERN` check), or (b) replay with the
original `Content-Type` header preserved and the raw text body. (a)
is simpler and matches the spirit of "don't cache responses whose
shape isn't a JSON envelope".
### M-2 — Refusal path persists messages without budget accounting
- **File**: `src/app/api/insights/chat/route.ts:158-170, 347-404`
- **What**: When `detectRefusal()` returns `refuse: true` the route
calls `streamRefusal()`, which (a) creates a new conversation if
needed, (b) persists the user message **and** an assistant refusal
message, but (c) never calls `recordSpend()` and never bumps
`CoachUsage.messageCount`. `enforceBudget()` runs before
`detectRefusal()`, so a budget-exhausted user is correctly 429'd —
but a user who sits at 0 spend can mash injection patterns at
effectively unlimited rate. Each request writes 2 rows into
`coach_messages` plus 1 into `coach_conversations` (when no
`conversationId` is sent). With ~4 KB encrypted-payload per refusal
pair and no rate-limit on the route, a scripted attacker storms the
table cheaply.
- **Why MED**: Storage DoS, not auth break. The pattern is a known
v1.4 hardening hole — `src/lib/rate-limit.ts` exists and is wired into
every other auth-touching endpoint, but `/api/insights/chat` never
calls it. The token budget is the only governor and refusals bypass it.
- **Fix**: Either (a) call `checkRateLimit()` (sliding window e.g.
60 req/h per user) at the top of the POST handler before the
conversation work runs, or (b) bump `messageCount` by 1 inside
`streamRefusal()` and let `enforceBudget()` gate on `messageCount`
too (cheap; preserves the storage DoS bound at 25 000 / 4 ≈ 6 000
messages/day worst case, which is ~24 MB/user/day).
### M-3 — Refusal patterns don't cover common injection synonyms
- **File**: `src/lib/ai/coach/refusal.ts:69-85`
- **What**: The `INJECTION_PATTERNS` bank looks for `ignore /
disregard / override / forget / pretend / dan / jailbreak`-shaped
phrases. It does not catch `"forget what I told you above"`,
`"do not follow your earlier directives"`,
`"start fresh, you are a"`, `"new instructions:"`, or non-Latin
injection vehicles (Cyrillic homoglyphs, zero-width-space splits like
`ignore previous instructions`). The `\b` boundary +
case-insensitive flag is solid but the lexicon is narrow.
- **Why MED**: Defence-in-depth — the **system prompt itself**
enforces the harder constraint ("only the snapshot, never invent
numbers"), so a missed pattern lets the message reach the model but
still doesn't usually leak. Risk shifts to *prompt-leaking*:
`"What were your starting instructions about the snapshot?"` matches
none of the regexes (no "reveal/print/show"-style verb tied to
"prompt"), reaches the model, and a sloppy model could echo the
system prompt back. The `(reveal|print|show|leak|expose|dump)` rule
catches the obvious phrasings but not "what / which / how / can you
share".
- **Fix**: Extend the bank with verbs `(forget|abandon|drop|skip|toss|
bypass|circumvent)` paired with `(instructions?|rules?|prompt|
guidelines?|directives?|system\s+message)`, plus a "what
(were|are)\s+your.*(instructions|prompt|rules)" probe pattern. A
zero-width-stripping normalisation pass (`message.replace(/[-
‍]/g, "")` before regex) closes the homoglyph split.
Genuine health questions don't trip these so false-positive cost
stays low.
### M-4 — `assistantMessage` persisted before streaming, but token-budget
ledger updated AFTER. Crash-window double-charge becomes
crash-window double-credit
- **File**: `src/app/api/insights/chat/route.ts:282-298`
- **What**: After a successful provider call the route does
`appendMessage(...assistant...)` then `recordSpend(...)`. If the
process is killed (OOM, deploy, container reaping) between those two
awaits, the assistant turn is on disk but `coach_usage.totalTokens`
is unchanged. The user effectively gets a free reply, the budget
ledger silently under-counts, and the operator's bill diverges from
the day's `messageCount`. Inverted polarity of the comment at line
292 ("a retried request doesn't double-count when the persistence
layer rolled back") — persistence cannot roll back the assistant
message because it was committed in its own transaction at line 283.
- **Why MED**: Bill leakage rather than auth break. Worst case under
this race a sustained chat user could ratchet ~1 000 free messages
before the next process restart, but a real attacker can't trigger
the crash on demand.
- **Fix**: Wrap `appendMessage(assistant)` and `recordSpend()` in a
single Prisma `$transaction` so the spend ledger is atomically
consistent with the persisted reply. The user-message persistence
(line 197) can stay outside the transaction — it must survive even
if the provider call fails.
## LOW
### L-1 — `coachChatRequestSchema.conversationId` accepts any string up
to 64 chars
- **File**: `src/lib/ai/coach/types.ts:33-38`
- **What**: `conversationId: z.string().min(1).max(64).optional()` — no
cuid format check. Prisma cuids are 25 chars, so a 64-char arbitrary
string is wasted DB lookup. Existence-leak safe because
`fetchConversationWithMessages()` returns null for both "not found"
and "wrong owner", and the route maps both to 404.
- **Fix**: Tighten to `.regex(/^c[a-z0-9]{24}$/)` or a generic
`[A-Za-z0-9_-]{8,64}` if non-cuid ids are anticipated. Cosmetic.
### L-2 — `streamProviderError()` snapshot parameter is typed
`ReturnType<typeof Object.assign>` — effectively `any`
- **File**: `src/app/api/insights/chat/route.ts:406-410`
- **What**: The function signature uses
`ReturnType<typeof Object.assign>` which TypeScript widens to a
free-for-all object type. The function never reads `args.snapshot`
but a future caller could pass arbitrary data through the type
hole. Not currently exploitable.
- **Fix**: Either drop the unused `snapshot` field from the
interface, or type it as `CoachProvenance | null`.
### L-3 — Refusal-injection conversation pollutes the rail when the
attacker provides an existing `conversationId`
- **File**: `src/app/api/insights/chat/route.ts:347-404`
- **What**: If a user sends an injection attempt with a valid
`conversationId`, `streamRefusal()` appends the user attempt + a
refusal reply into the targeted conversation. A user can use this to
pad/garbage their own conversation, which is fine. But the rail
ordering re-orders by `updatedAt DESC` (persistence.ts:230), so a
spammed conversation jumps to the top of the rail — no security
consequence, just UX noise on the attacker's own account.
- **Fix**: Optional. Either skip the `updatedAt` bump for refusal
appends, or persist a single `assistant` row tagged
`providerType: "refusal"` (already done at 380) and let the UI hide
refusal-only conversations.
### L-4 — `feedback_no_pii_in_user_facing.md`: maintainer-name comment
in pre-v1.4.20 file
- **File**: `src/app/api/analytics/route.ts:79`
- **What**: `// of the '30T' sub-value. For Marc's data (572 paired
readings,…)`. This was committed under v1.4.19 (`a856272`,
2026-05-10) — it predates v1.4.20 but is in the same file the
Health Score work touched, so flagging here for cleanup. Not
user-facing (source comment, not a string a user can see), but the
memory rule says "no maintainer-name leaks in any new committed
file" and this file received new commits in v1.4.20.
- **Fix**: Rephrase the comment to a neutral example
("on a busy account with 572 paired readings, recent 30 d = 50 %,
all-time ≈ 11 %"). One-line edit.
## Things done right
- **404-not-403 on cross-user reads** — both
`fetchConversationWithMessages()` and `deleteConversation()`
return null for "not found" and "wrong owner", and both routes map
to 404. Existence-leak channel is closed.
- **Encryption** — `CoachMessage.encryptedContent` is AES-256-GCM,
goes through `encrypt()` / `decrypt()` so the active-key id is
stamped into the ciphertext. Key versioning works without a
backfill. `metricSourceJson` is correctly plaintext but provably
labels-only — `CoachProvenance` types restrict `windows` and
`metrics` to enum strings, and `counts` is bounded sample-counts,
not measured values.
- **GDPR cascade** — `CoachConversation -> User onDelete: Cascade`
and `CoachMessage -> CoachConversation onDelete: Cascade` together
with `CoachUsage -> User onDelete: Cascade` mean a user-delete
removes every Coach artefact in one statement.
- **Idempotency `SECRET_PATTERN` defence** — the wrapper still
refuses to cache a body containing `hlk_` / `hlr_` / `sk-…`, so
even if the route were misconfigured to wrap a token-issuing path
the leak couldn't propagate via replay.
- **Streaming error frames are minimal** — `streamProviderError()`
emits `{ type: "error", code, message: code }` only; no provider
HTTP status, stack, or attempt-list reaches the wire.
- **Health Score is fully deterministic** — `computeHealthScore()` is
pure (no `Math.random`, no time fuzzing inside the formula); the
only `Date.now()` call is in `computeUserHealthScore()` to bound
the SQL window, which is the legitimate "now" anchor.
- **Weekly-report `[week]` parser** — `parseWeekISO()` only accepts
`^(\d{4})-W(\d{2})$` with bounded year / week, so path-traversal
via the segment is closed; the `notFound()` branch in the page
short-circuits before any rendering.
- **System prompt does not name the user** — `getCoachSystemPrompt()`
explicitly instructs the model "Address the user as 'you'. Never
invent a name." and the snapshot block carries no name field, so a
trained model can't echo back PII the route never gave it.
## Uncertain / flagged for maintainer review
- **U-1**: The `request.clone()` dance in the POST handler at
lines 434-449 reads the body once for conversationId detection
before delegating to either the bare handler or the
withIdempotency-wrapped handler. The wrapped handler then reads the
body again. I confirmed by code that `clone()` returns an
independent body stream and the original is still readable, but I
did NOT smoke-test against a real Next.js 16 runtime — there have
been edge-runtime regressions around stream cloning in past Next
releases. Suggest one Vitest case posting a new-conversation body
and asserting the persisted user message matches the input.
- **U-2**: `enforceBudget()` reads `coach_usage.totalTokens` and the
unique constraint is `(userId, dateKey)`. Two concurrent first-of-
day requests both see 0, both pass the budget, both call the
provider, both `upsert` — the second update increments the row the
first request created. So spend is consistent. But: between the
budget read (line 149) and the spend write (line 294) there is no
serialisable transaction, so a user holding the budget right at the
cap could in principle race to ~2× the cap before the ledger
catches up. Acceptable in practice (the cap is per-day, not
per-second) but worth annotating.

# Phase D — Senior-dev review (v1.4.20)
Reviewed against `develop` HEAD `ded0b38` (27 commits, ~+15 kLOC across 94 files). Focus: architectural posture rather than line-level nitpicks. Cross-references the parallel `phase-D-v1420-code-review.md` (line-bug catalogue) and `phase-D-v1420-security-review.md` only where the architectural issue is distinct.
## Summary
Architectural posture: solid, with one structural fault that will outlive the release if not addressed. The Coach surface is the most disciplined slice of new code in the repo: clear module boundaries (`types.ts` / `persistence.ts` / `budget.ts` / `refusal.ts` / `snapshot.ts` / `system-prompt.ts`), label-only provenance separated from encrypted bodies, AES-GCM at rest with key-id stamping for rotation, prompt-injection regex bank, per-day token ledger keyed on UTC, and 404-not-403 ownership semantics. The Insights surface (correlations, health score, weekly report, storyboard annotations) is wired through the existing AI provider chain and the existing schema-lift pattern in `useInsightsAdvisorQuery` — no new infrastructure invented where existing primitives fit. Where it falls down: a duplicated maths layer (`src/lib/insights/correlations.ts` parallels the older `src/lib/analytics/correlations.ts` + `analytics/health-score.ts` re-implements `linearRegressionSlope` next to `analytics/trends.ts:trendSlope`), a fragile SSE-vs-idempotency interaction that the streaming hook silently papers over, and an `extractFeatures()` re-fetch on every Coach turn that does not scale. Findings: 0 CRITICAL · 4 HIGH · 6 MED · 4 LOW.
## CRITICAL
_None._ (One borderline-CRITICAL — Coach SSE inside `withIdempotency` — captured as HIGH-1 because the streaming hook short-circuits before the bug becomes user-visible. The dead path is still wrong.)
## HIGH
### SD-HIGH-1 — `withIdempotency` wraps a streaming response and silently mis-caches it
**File**: `src/app/api/insights/chat/route.ts:429-450`, `src/lib/idempotency.ts:218-242`
**What**: New-conversation POSTs to `/api/insights/chat` go through `withIdempotency(handleChatRequest)`. The handler returns a `Response` whose body is a `ReadableStream<Uint8Array>` of `data: …\n\n` SSE frames with `status: 200` (cachable per `isCachableStatus`). The wrapper then runs `response.clone().text()` to scan for `hlk_/sk-` secrets and persists the body verbatim into `idempotency_keys.responseBody`. On replay, `findCached()` does `JSON.parse(row.responseBody)` (the SSE text isn't JSON, parse fails silently to `null`) and returns `NextResponse.json(null, { status: 200 })`. A retry under the same `Idempotency-Key` would therefore deliver `null` instead of the original conversation — but the user never sees it because the streaming client (`use-coach.ts:267-302`) uses `fetch` without `Idempotency-Key`, so the wrapper never fires from the PWA. The iOS app, which DOES set `Idempotency-Key` per the codebase convention (`hlk_` regex), would walk straight into this on first retry.
**Why architectural**: SSE bodies do not survive the request/response idempotency model the rest of the API uses. The cache layer assumes JSON-serialisable envelopes; bolting a stream onto it without a guard is the kind of cross-cutting mismatch that makes a future maintainer wrong by default. There's also no integration test exercising `Idempotency-Key` against the SSE route — the gap is invisible in CI.
**Fix or refactor**: Either (a) skip idempotency caching whenever `Content-Type` starts with `text/event-stream` (one-line guard in `withIdempotency` next to the secret-pattern check), or (b) split the SSE-emitting code-path off the idempotency wrap entirely — only cache the conversation-creation side-effect (return a JSON 201 with the new `conversationId`) and have the client open the SSE on a subsequent request. Option (a) is a 5-minute fix; option (b) is correctness-superior and trims the per-stream cost of holding the entire reply in memory before the first byte goes out.
### SD-HIGH-2 — Duplicated maths layer: `pearson` × 2, `linearRegressionSlope` × 2
**Files**:
- `src/lib/insights/correlations.ts:107` — new `pearson()` (with p-value, Fisher CI, t-stat).
- `src/lib/analytics/correlations.ts:77` — existing `pearsonCorrelation()` (used in `weight-status`, `blood-pressure-status`, `mood-status`, `features.ts` — five callers).
- `src/lib/analytics/health-score.ts:81` — new `linearRegressionSlope()`.
- `src/lib/analytics/trends.ts:111` — existing `trendSlope()` (consumed by every `summarize()` call across analytics).
**What**: v1.4.20 added a second Pearson implementation and a second least-squares slope, both pure, both correct, neither aware of the other. The strategic-plan rationale (`phase-B3-report.md`) explains the new `pearson` carries a p-value + CI that the old one lacks — a real reason to add columns to the contract — but the right move is to extend the existing helper with optional p-value + CI fields and use ONE function across the codebase. As of v1.4.20 a maintainer hunting "where do we compute Pearson?" finds two answers, with subtly different return shapes (`{r, strength, n}` vs `{r, n, pValue, confidenceInterval}`) and different `minPairs` defaults (5 vs 14).
**Why architectural**: Two parallel implementations of the same identity (Pearson r) is a textbook drift hazard. The version with German strength labels (`stark/moderat/schwach/keine`) lives in `analytics/`, the version with English UI bands (`low/moderate/high`) lives in `insights/`. A v1.4.21 author who patches one will not know about the other. Same for the slope helper — `trendSlope()` and `linearRegressionSlope()` differ in date handling (Date object vs ISO-string sort) and one returns `null` for fewer-than-2 points while the other returns `null` for fewer-than-2 distinct x-values.
**Fix or refactor**: Consolidate into a single `src/lib/analytics/correlation.ts` (singular) and `src/lib/analytics/regression.ts`. Add the p-value + CI fields as optional outputs on the existing `pearsonCorrelation()`. Migrate insights/correlations.ts callers (`correlateBpCompliance`, `correlateMoodPulse`, `correlateWeightWeekday`) to import from analytics. The hypothesis-specific runners (which do the `n>=14`/`p<0.05` gating + interpretation strings) keep living in `lib/insights/` because they ARE feature-specific.
### SD-HIGH-3 — `buildCoachSnapshot` re-extracts the entire user history on every Coach turn
**File**: `src/lib/ai/coach/snapshot.ts:30-32` → `src/lib/insights/features.ts:272`.
**What**: Every POST to `/api/insights/chat` calls `extractFeatures(userId, false)`, which runs `prisma.measurement.findMany({ where: { userId } })` with NO date filter — fetching every measurement the user has ever recorded. For Marc's data set (572 paired BP rows + weight + pulse + mood + glucose) that's a few hundred rows; for an external-ingest power user with multiple years of Withings data it's tens of thousands. Compounded with the 18-turn-window prompt assembly, every Coach turn pays full historical I/O.
**Why architectural**: The Coach is conceptually a chat over the same snapshot the dashboard renders — but the snapshot is regenerated from raw measurements per turn instead of cached. The strategic plan called this out as "Reuses the analytics features pipeline so the Coach narrates the exact same numbers" which is right architecturally — except the analytics pipeline is itself uncached. There's no `react-query`-style server-side memo, no Redis layer, no `pg_boss` job to pre-compute the snapshot once per day. v1.4.21 will not be able to scale this without a snapshot-cache abstraction.
**Fix or refactor**: Extract a `buildSnapshotForUser(userId, locale)` helper that lives in `src/lib/ai/snapshot/` and is used by BOTH the Coach (per turn) and the existing insight-generator route. Cache it by `(userId, last-measurement-mtime)` for 60s either in-process (Map+TTL) or via a `snapshot_cache` Prisma model keyed on `userId` with a `staleAfter` column. v1.5 can add a pg-boss `snapshot.refresh` job. Even a 60-second in-memory cache eliminates the steady-state re-fetch when a user is mid-conversation.
### SD-HIGH-4 — `<CoachDrawer key={coachPrefill ?? "blank"}>` weaponises React keys for state reset
**File**: `src/app/insights/page.tsx:1611-1616`, `src/components/insights/coach-panel/coach-drawer.tsx:88-89`.
**What**: The page re-keys `<CoachDrawer>` on every prefill change so the drawer's lazy-init `useState<string>(() => prefill ?? "")` reads the latest chip text. Side-effects: any in-flight stream is torn down, the conversation cache for the current thread is dropped, the message thread re-renders from scratch, and the rail's confirm-delete state is lost. A user who picks a chip mid-stream loses the stream. The `useEffect`-set-state-in-effect lint avoidance was the trigger (per inline comment), but the chosen workaround is heavier than the problem.
**Why architectural**: Using `key=` to reset child state is a code smell — it bypasses the component's own state contract. The drawer should expose a `setPrefill(value: string)` method (or, more idiomatic, accept `prefill` as a controlled prop and own no internal copy of it). The current shape leaks the parent's "I changed the prefill" intent into a React key, which is the wrong API.
**Fix or refactor**: Make `prefill` a fully-controlled prop on `<CoachDrawer>`. The drawer reads it directly into the textarea's `value=` and notifies up via `onPrefillConsumed` after the user submits. Drop the `key=` re-mount. This composes naturally with the in-flight stream — picking a new chip stages the next message instead of nuking the current one.
## MED
### SD-MED-1 — Coach tables not covered by `cascade-delete.test.ts` despite the migration claiming so
**File**: `prisma/migrations/0035_coach_conversations_v1420/migration.sql:18` says "GDPR: every table cascades on user delete via FK chains, covered end-to-end in tests/integration/cascade-delete.test.ts." Searching that test for `coach`, `CoachConversation`, `coach_messages`, `coach_usage` returns zero hits. The cascade IS configured at the SQL level (verified in the migration), but the integration test pinning that contract was never updated.
**Why architectural**: The integration suite is the GDPR firewall. A future schema change that accidentally drops a cascade rule would not be caught in CI. The comment is also a documentation lie that future maintainers will trust.
**Fix**: Add three `expect(...count(...)).toBe(0)` lines to `cascade-delete.test.ts` for `coachConversation`, `coachMessage`, `coachUsage` — five-line patch.
### SD-MED-2 — `weeklyReport` + `storyboardAnnotations` lifted off cache without `safeParse`
**File**: `src/components/insights/use-insights-advisor.ts:71-99` schema-validates `dailyBriefing` and `trendAnnotations` via `.safeParse`. `src/components/insights/weekly-report-view.tsx:57-58` and `src/app/insights/page.tsx:921-929` cast directly: `(advisor.payload?.insights as { weeklyReport?: WeeklyReport })`. A stale cache from a buggy provider (or a future schema change) would crash the page or render malformed sections.
**Why architectural**: The advisor is the single point where AI payloads cross the trust boundary. The lift-pattern is the contract; two new blocks bypass it. Pick one approach.
**Fix**: Lift `weeklyReport` + `storyboardAnnotations` through `weeklyReportSchema.safeParse` and `storyboardAnnotationsSchema.safeParse` in `useInsightsAdvisorQuery`, the same way the other two blocks are.
### SD-MED-3 — No rate-limit on `/api/insights/chat`; only the 25k/day token cap
**File**: `src/app/api/insights/chat/route.ts:127-149` calls `enforceBudget()` (cap on `coach_usage.totalTokens`) but no `enforceRateLimit()` call. A malicious client can spam tiny "ok" messages — each consumes ~10 tokens but each runs encrypt+persist+provider-call. 1000 messages × few-second latency = ~1h provider-bill churn before the budget gate trips.
**Why architectural**: Every other auth-gated POST in the codebase that talks to a paid upstream (Withings sync, insights generate) goes through the in-memory rate-limiter. Coach is the highest-cost surface and skips it.
**Fix**: Wrap `handleChatRequest` in the existing `enforceRateLimit({ key: \`coach.chat:\${userId}\`, max: 30, windowMs: 60_000 })` (or matching budget). Roughly five lines.
### SD-MED-4 — Inline `tokeniseForStreaming` is fake-streaming and adds latency without value
**File**: `src/app/api/insights/chat/route.ts:89-95` and `:312-335`.
**What**: The route runs `runRawCompletionWithFallback()` to completion, gets the full reply text, then `tokeniseForStreaming(replyText)` re-splits it on whitespace and pushes each word as a separate SSE frame in the `start()` callback (synchronous, no flush boundary). The user sees a single network response with all frames buffered together — the streaming UX is theatrical.
**Why architectural**: The choice between "wait for full reply, then tokenise" and "true upstream streaming" is a real architectural decision (the providers' SDKs all support streaming), and v1.4.20 chose the former. That's defensible — schema validation needs the full body to parse, and the strict-schema path is the v1.4.15 hardening anchor — but the Coach replies are NOT schema-validated (they're free prose). There's no reason this surface couldn't pipe through to true streaming. The current shape is a UX claim that the engineering doesn't back.
**Fix or refactor**: Defer to v1.4.21 — wire `runRawCompletionWithFallback` to a streaming variant that yields chunks. The provider abstraction needs an `onToken` callback. This unlocks "stop generating" as a real feature later. For v1.4.20, document the choice in the route header so the next maintainer knows it's intentional.
### SD-MED-5 — `medication_schedules.days_of_week` schema drift surfaces in B5
**File**: `src/app/api/analytics/route.ts:394-407` deliberately uses `select: { schedules: { select: { windowStart: true, windowEnd: true } } }` to dodge the missing `days_of_week` column (per `phase-B5-report.md` flagged-uncertain item 1). The `src/app/api/insights/comprehensive/route.ts` route uses `include: { schedules: true }` and would crash on a fresh DB.
**Why architectural**: Codebase carries a half-applied schema migration. v1.4.20 worked around it locally; v1.4.21 will keep working around it locally; v1.5 will hit a fresh-DB build that breaks. This is the textbook tech-debt accrual the lens asks about.
**Fix**: Either (a) `pnpm db:migrate` produce the missing column with a backfill default, or (b) remove the field from `schema.prisma` so consumers stop expecting it. Both are 30-minute jobs; both are overdue.
### SD-MED-6 — `apiHandler`-vs-`withIdempotency` stacking is hard to read and tested by inspection only
**File**: `src/app/api/insights/chat/route.ts:429-450`. The handler peeks at the body to decide whether to wrap in `withIdempotency`, then conditionally invokes the wrapper. The inner `apiHandler` lifts errors → JSON, but the outer dispatch is hand-rolled.
**Why architectural**: Other routes in the codebase mount `apiHandler(withIdempotency(...))` once and trust the wrapper to no-op on missing keys. Here the dispatcher reads the body twice (once to peek, once inside). Move the "should this idempotent-cache?" decision into `withIdempotency` itself (e.g. an `opts.cacheKey` predicate) so callers don't reach into the request body.
**Fix**: Add `withIdempotency(handler, { skipWhen?: (req) => boolean })` so the chat route can `withIdempotency(handler, { skipWhen: hasConversationId })` and the body double-read disappears.
## LOW
### SD-LOW-1 — `coach_usage` carries a redundant index
**File**: `prisma/schema.prisma:932-934`. `@@unique([userId, dateKey])` AND `@@index([userId, dateKey])`. Postgres builds an index for the unique constraint already. The plain `@@index` is dead weight (storage + write cost).
**Fix**: Drop the `@@index` line.
### SD-LOW-2 — `metricSourceJson` stored as `String` instead of `Jsonb`
**File**: `prisma/schema.prisma:899-913`. The provenance envelope is intentionally plain text (the migration's comment explains it's for label-only analytics queries). But Postgres `jsonb` would let admin-app-logs query it without a parse round-trip. Currently the column is `TEXT` and every analytics query needs `jsonb_build_object((metric_source_json::jsonb))`. Cheap to fix while no consumers exist.
**Fix**: Migrate the column type to `Jsonb`. Backwards-compatible (the JSON serialisation is identical).
### SD-LOW-3 — Coach off-topic detector is locale-mixed
**File**: `src/lib/ai/coach/refusal.ts:99-121`. Allow + deny lists mix EN + DE tokens. A French user typing "blutdruck"... tongue-in-cheek aside: the deny list misses obvious off-topic Spanish/French/Italian, and the allow-list will let through any message containing the word "trend" — which "stock market trend" does. Pragmatic enough for v1.4.20; will need expanding when the iOS app launches.
**Fix**: v1.4.21 — feed the detector a `defaultAllow=false` bias and tighten the list. Mark as known.
### SD-LOW-4 — `weeklyReport.weekISO` regex `^\d{4}-W\d{2}$` does not validate week 53 or week 00
**File**: `src/lib/ai/schema.ts:344`. The regex matches `YYYY-Www` but `00`, `54`, `99` would all pass parse. The route page (`/insights/report/[week]`) calls `parseWeekISO()` which IS strict, so the user-facing path is safe. But a future consumer trusting the schema alone would be surprised.
**Fix**: Tighten regex to `^\d{4}-W(0[1-9]|[1-4]\d|5[0-3])$`.
## Tech-debt accrual (specific)
1. **Two parallel correlation stacks** (HIGH-2). v1.4.21 author who needs Pearson must read both files to know which one to call. Each release widens the gap.
2. **Snapshot rebuilds per Coach turn** (HIGH-3). Steady-state I/O grows linearly with a user's measurement history, multiplied by chat volume. v1.4.21's first power-user complaint will be Coach latency.
3. **SSE inside idempotency wrapper without a guard** (HIGH-1). The bug is hidden today because the PWA doesn't send `Idempotency-Key`. The iOS app will trip it the day shipping starts.
4. **`medication_schedules.days_of_week` half-migration** (MED-5). Working around schema drift in route after route is the canonical "we'll fix it next release" failure mode. v1.4.20 added a second workaround.
5. **`<CoachDrawer key=...>` state-reset hack** (HIGH-4). The next contributor who patches the drawer will not realise that the parent re-mounts it on every prefill. Unwind this before the drawer grows more state.
6. **Cascade-delete contract drift** (MED-1). The integration test is the GDPR firewall; v1.4.20 added three tables to it without updating the test.
7. **Schema-lift inconsistency in `useInsightsAdvisorQuery`** (MED-2). Two of four new schema blocks are validated, two are cast through `as`. Pick one and apply uniformly.
8. **Fake streaming theatre** (MED-4). The product surface promises "tokens stream in" but the route waits for the full reply. v1.4.21's "stop generating" or "regenerate from token N" features land into a foundation that doesn't actually stream.
## Things to keep
1. **Coach module decomposition.** Splitting `types.ts` / `persistence.ts` / `budget.ts` / `refusal.ts` / `snapshot.ts` / `system-prompt.ts` is the cleanest module boundary in the codebase. Each file is testable in isolation, the dependency direction is one-way (types ← persistence ← snapshot ← (route)), and the unit tests reflect the boundary.
2. **Label-only provenance separated from encrypted bodies.** `metricSourceJson` plain-text + `encryptedContent` AES-GCM is exactly the right shape — analytics-queryable provenance, PII-protected content. The comment block in the migration explaining WHY each column has its encryption choice is a model for future migrations.
3. **404-not-403 cross-user existence guard.** Both the chat and the `/[id]` routes consistently fold "wrong user" → 404. No existence-leak side channel.
4. **Pure correlation runners with `EmptyState` below threshold.** Refusing to surface a marginal `r=0.18` as a "pattern" is the right product-honesty move. The interpretation phrasing locked at the runner level (not the prompt) is the right guard against the model softening it.
5. **`<HealthScoreCard>` as a pure presentational right-side panel.** Server-deterministic compute + client renders only. The same shape will scale to v1.5's "explain my score" expand-card without touching the panel.
6. **`buildHistoryWindow` 20-turn cap with synthetic-summary placeholder.** Bounded prompt budget without paying for a separate summarisation provider call. Right tradeoff for the MVP.
7. **`PROMPT_VERSION` stamped on every persisted assistant message.** Future replay / analytics / per-version helpful-rate slicing all become trivial.
## Cross-link to product-lead review
For the next product-lead pass, the four items below are framing decisions, not code:
- **Fake streaming vs true streaming** (MED-4) — does the product story require token-by-token UX honesty? If yes, v1.4.21 should true-stream and "stop generating" lands as part of the change.
- **Coach rate-limit shape** (MED-3) — what's the per-user RPS the product wants? 30/min default looks safe but should be a product call given the iOS launch.
- **Snapshot freshness for Coach** (HIGH-3) — is "the Coach narrates last hour's data" a feature or a footgun? A 60-second cache is invisible to users; a 24-hour cache might be fine for the dashboard but feels stale for a chat surface.
- **Daily token budget** (25k tokens / user / day) — is this number the product's, or an arbitrary technical pick? Worth pinning down before the iOS launch makes it a billing reality.

# Phase D — Simplify review (v1.4.20)
Lens: every chunk of new code that v1.4.20 added (Wave B = B1–B5).
Read-only review — write findings, do NOT commit. Security paths,
chart visual identity, fixture data, and the genuinely new
abstractions (`correlations.ts`, `health-score.ts`) get extra room.
## Summary
Total findings: **5 apply-yes** · **4 apply-maybe** · **3 apply-no**
apply-yes are the clear wins worth folding into reconcile. apply-maybe
flags judgement calls (test seams vs. dead parameters, premature
extraction). apply-no documents what was looked at and rejected so the
maintainer's diff isn't asked the same question twice.

## apply-yes (clear wins, ship in reconcile)
### S-01 — Drop unused `snapshot` parameter from `streamProviderError`
**File**: `src/app/api/insights/chat/route.ts:406-427`
**Why**: The function takes a `snapshot: ReturnType<typeof Object.assign>` parameter (which evaluates to `any` — a type smell on its own) and never reads it inside the body. Three callers dutifully pass `snapshot.provenance` for nothing. Pure dead code on both sides of the call.
**Before** (≤8 lines):
```ts
function streamProviderError(args: {
conversationId: string;
snapshot: ReturnType<typeof Object.assign>;
code: string;
}): Response {
const stream = new ReadableStream<Uint8Array>({
start(controller) {
try { controller.enqueue(encodeFrame({ type: "error", code: args.code, message: args.code })); }
finally { controller.close(); }
},
});
return new Response(stream, { status: 503, headers: SSE_HEADERS });
}
```
**After** (≤8 lines):
```ts
function streamProviderError(args: { code: string }): Response {
const stream = new ReadableStream<Uint8Array>({
start(controller) {
try { controller.enqueue(encodeFrame({ type: "error", code: args.code, message: args.code })); }
finally { controller.close(); }
},
});
return new Response(stream, { status: 503, headers: SSE_HEADERS });
}
```
(All three call sites drop `snapshot:`/`conversationId:` keys too — `conversationId` is also unused, only `code` survives.)

### S-02 — Extract the duplicated `formatRelativeTime` helper
**File**: `src/components/insights/{hero-strip.tsx:134-148, daily-briefing.tsx:259-273, coach-panel/history-rail.tsx:40-54}` (also still present in the deprecated `insights-page-hero.tsx:64-86`)
**Why**: Three live byte-identical copies of the same 14-line helper. Each comment even calls out "Mirrors the helper in `<InsightsPageHero>`" — the maintainers know it's duplicated. Drift risk: a later i18n key change has to land in three files.
**Before** (≤8 lines, repeated 3×):
```ts
function formatRelativeTime(iso: string, t: …): string {
const target = new Date(iso).getTime();
if (Number.isNaN(target)) return "";
const diffMs = Date.now() - target;
if (diffMs < 60_000) return t("insights.relativeJustNow");
const minutes = Math.floor(diffMs / 60_000);
if (minutes < 60) return t("insights.relativeMinutesAgo", { count: minutes });
const hours = Math.floor(minutes / 60);
if (hours < 24) return t("insights.relativeHoursAgo", { count: hours });
return t("insights.relativeDaysAgo", { count: Math.floor(hours / 24) });
}
```
**After** (≤8 lines):
```ts
// new file: src/lib/i18n/relative-time.ts
export function formatRelativeTime(
iso: string,
t: (key: string, params?: Record<string, string | number>) => string,
): string { /* …same body, written once… */ }
// callers: import { formatRelativeTime } from "@/lib/i18n/relative-time";
```

### S-03 — Drop the dead `["coachConversation", "null"]` queryKey branch
**File**: `src/components/insights/coach-panel/use-coach.ts:83-98`
**Why**: The hook ternary picks a different queryKey when `id` is null — but `enabled: id !== null` keeps the query disabled in that exact case, so the "null" key never gets a queryFn fire. Just key on `id` directly; React Query happily accepts `null` in the array.
**Before**:
```ts
return useQuery({
queryKey: id ? QUERY_KEYS.one(id) : ["coachConversation", "null"],
queryFn: async () => { if (!id) throw new Error("missing id"); … },
enabled: id !== null,
staleTime: 60 * 1000,
});
```
**After**:
```ts
return useQuery({
queryKey: ["coachConversation", id] as const,
queryFn: async () => { if (!id) throw new Error("missing id"); … },
enabled: id !== null,
staleTime: 60 * 1000,
});
```

### S-04 — Hoist `STORYBOARD_COLOR_BY_CATEGORY` to module scope
**File**: `src/app/insights/page.tsx:915-920`
**Why**: The map is declared inside the component body so it's recreated on every render of `<InsightsPage>` (a render-heavy page). Map is data-only — no closures over locals, no i18n keys. Module-scope `const` is the boring right answer.
**Before**:
```tsx
export default function InsightsPage() {
// …200+ lines of derived state…
const STORYBOARD_COLOR_BY_CATEGORY: Record<string, string> = {
medication: "var(--dracula-pink)", event: "var(--dracula-cyan)",
milestone: "var(--dracula-green)", warning: "var(--dracula-orange)",
};
}
```
**After**:
```tsx
const STORYBOARD_COLOR_BY_CATEGORY: Record<string, string> = {
medication: "var(--dracula-pink)", event: "var(--dracula-cyan)",
milestone: "var(--dracula-green)", warning: "var(--dracula-orange)",
};
export default function InsightsPage() { /* … */ }
```
---
### S-05 — Extract the confidence-band class+label table
**File**: `src/components/insights/{correlation-card.tsx:61-73, trend-annotation.tsx:33-46}`
**Why**: Both cards declare the same `Record<"low"|"moderate"|"high", string>` for badge classes AND for translation-key labels — byte-identical class strings, only the i18n key prefix differs (`correlationRow.*` vs `trendAnnotation.*`). Move the classnames to one constant, keep each component's i18n key map local (it IS distinct copy). The class table is the load-bearing duplication.
**Before** (one of two copies, each ~14 lines):
```tsx
const CONFIDENCE_BADGE_CLASSNAME: Record<"low" | "moderate" | "high", string> = {
high: "border-dracula-green/40 bg-dracula-green/10 text-dracula-green",
moderate: "border-dracula-orange/40 bg-dracula-orange/10 text-dracula-orange",
low: "border-dracula-comment/40 bg-dracula-comment/10 text-muted-foreground",
};
```
**After**:
```tsx
// new file: src/components/insights/confidence-badge.ts
export type ConfidenceBand = "low" | "moderate" | "high";
export const CONFIDENCE_BADGE_CLASS: Record<ConfidenceBand, string> = {
high: "border-dracula-green/40 bg-dracula-green/10 text-dracula-green",
moderate: "border-dracula-orange/40 bg-dracula-orange/10 text-dracula-orange",
low: "border-dracula-comment/40 bg-dracula-comment/10 text-muted-foreground",
};
```

## apply-maybe (judgment calls)
### S-06 — `<CoachDrawer>` slot props (`historyRail` / `sourcesRail` / `composer`) are dead in production
**File**: `src/components/insights/coach-panel/coach-drawer.tsx:53-63, 200-228`
**Why**: The only production caller (`/insights/page.tsx:1611`) never passes these slots — the JSDoc says they were a commit-2 boundary "until commit 3 lands". Commit 3 has long since landed. `<CoachDrawerBody>` has its own slot props which the SSR test harness uses; nobody injects against `<CoachDrawer>`. Removing them halves the prop surface.
**Before** (≤8 lines):
```tsx
historyRail?: React.ReactNode;
sourcesRail?: React.ReactNode;
composer?: React.ReactNode;
// …
historyRail={historyRail ?? <HistoryRail … />}
sourcesRail={sourcesRail ?? <SourcesRail />}
composer={composer ?? <CoachInput … />}
```
**After** (≤8 lines):
```tsx
// drop the three props on CoachDrawer entirely
historyRail={<HistoryRail … />}
sourcesRail={<SourcesRail />}
composer={<CoachInput … />}
```
**Why "maybe"**: keeping them costs little; the trade-off is "future test seam" vs. "delete dead code now". Given `<CoachDrawerBody>` already exposes the equivalent seams for tests, I'd ship this. But a maintainer who wants to keep the prop-injection contract symmetric across the two components has a defensible counter-argument.
---
### S-07 — Coach `<CoachInput>` `inputId` prop is never overridden
**File**: `src/components/insights/coach-panel/coach-input.tsx:45-55`
**Why**: `inputId?: string` defaults to `"coach-composer-textarea"`; no caller in the codebase passes a different value. Removing the prop drops 2 lines and one `?? default` from the signature.
**Why "maybe"**: tiny. Worth folding into the same pass that touches the file; otherwise leave it.
---
### S-08 — `cloneForCheck` variable is dead in the chat POST handler
**File**: `src/app/api/insights/chat/route.ts:434-445`
**Why**: `let cloneForCheck: NextRequest | undefined = undefined;` is assigned via `request.clone() as NextRequest`, then immediately consumed by `cloneForCheck.json()` — never referenced again, never read after the `try` block. Inline the `.clone().json()` call directly.
**Before**:
```ts
let cloneForCheck: NextRequest | undefined = undefined;
let conversationId: string | undefined = undefined;
try {
cloneForCheck = request.clone() as NextRequest;
const body = await cloneForCheck.json();
if (typeof body?.conversationId === "string") conversationId = body.conversationId;
} catch { /* fall through */ }
```
**After**:
```ts
let conversationId: string | undefined = undefined;
try {
const body = await request.clone().json();
if (typeof body?.conversationId === "string") conversationId = body.conversationId;
} catch { /* fall through */ }
```
**Why "maybe"**: the variable's only sin is verbosity; the logic is fine. Pure cosmetic gain.
---
### S-09 — `health-score-card.tsx` delta arrow chooser is three sibling `&&` blocks
**File**: `src/components/insights/health-score-card.tsx:153-171`
**Why**: Three `delta > 0`, `delta < 0`, `delta === 0` blocks each rendering a different arrow icon. A small `deltaIcon = delta > 0 ? ArrowUp : delta < 0 ? ArrowDown : Minus` lookup keeps all three icon imports but compresses the JSX.
**Why "maybe"**: the current shape is explicit and grep-friendly. The compressed version is shorter but introduces a colour-class lookup too (`text-dracula-green` vs `text-dracula-red` vs `text-muted-foreground`). Borderline — left as a judgement call for the maintainer who actually owns the file.
---
## apply-no (rejected — explanation only, no diff)
### S-N1 — The two `pearson` implementations (`src/lib/insights/correlations.ts` and `src/lib/analytics/correlations.ts`)
The new `pearson()` in `correlations.ts` returns a tagged union with p-value + 95 % Fisher-z confidence interval; the older `pearsonCorrelation()` returns `{ r, strength, n }` with strength buckets ("stark"/"moderat"/…). Different surface, different consumers, different statistical guarantees. Merging them would force the older callers (dashboard scatter cards) to deal with the p-value contract they don't need. Keep both.
### S-N2 — `correlations.ts` per-hypothesis bodies (`correlateBpCompliance` / `correlateMoodPulse` / `correlateWeightWeekday`)
Each runner has the same `if (insufficient) return …; if (pValue ≥ 0.05) return …; build interpretation; return ok` skeleton. Tempting to extract a generic `runHypothesis()`, but the interpretation phrases are distinct, the input shapes differ, and one of three uses ANOVA not Pearson. The current shape reads top-to-bottom; a generic abstraction would obscure the conservative-phrasing rule that lives in plain English in each runner. The CLAUDE.md guidance is explicit ("the recently-introduced abstractions … give them room"). Leave as is.
### S-N3 — `recommendation` system-prompt EN/DE near-duplication (insight-generator.ts GROUND RULES 8-11)
The German prompt mirrors the English one section-for-section. A single template with placeholder slots was tempting, but the linguistic register matters in prompts (verb mood, technical vocabulary), and Marc's voice rule explicitly wants per-locale authorship not auto-translation. Keep the two prompts side-by-side; the only thing worth doing here is a periodic diff-lint to catch when one locale gets a rule the other doesn't (already covered by the i18n integrity tests).
---
## Things deliberately NOT simplified
- **`refusal.ts` injection regex bank.** Verbose by design — a flat array of regexes lets a future audit grep any single phrase. Compressing into a tree would defeat that. Security guidance in CLAUDE.md backs this.
- **`use-coach.ts` SSE parser.** The `parseSseChunk` helper is exported AND consumed by tests. The verbose state-machine shape is the test seam; don't fold it into the streaming hook.
- **`weekly-report-view.tsx` Section/BulletList helpers.** Could be inlined back, but they're called 5 times each and read clearly. Net wash.
- **`<HeroStrip>` action-button row.** The three buttons (weekly-report disabled, ask-coach conditional, regenerate conditional) look like they want a `<HeroAction>` component — but each carries unique enable/disable/title logic, and the inline JSX matches the artboard one-to-one. Extracting would make the diff harder to compare against the design.
- **`computeHealthScore` weight-redistribution loop.** The "redistribute null components proportionally" comment IS the load-bearing logic — extracting helpers further would obscure that the same iteration order anchors the formula. Leave it.
---
## Verification gates that won't break
All proposals above are behaviour-preserving:
- S-01: pure dead-parameter removal (pre-conditions unchanged at call sites).
- S-02: extracted helper byte-identical to the three current copies.
- S-03: queryKey shape `["coachConversation", null]` is structurally equivalent for cache lookup.
- S-04: hoist is referentially identical (no closures captured).
- S-05: classnames extracted; per-component i18n keys stay local.
- S-06–S-09 (maybe): each is a strict subset of the current behaviour.

Carry-over from v1.4.20 marathon foundation phases. Pick from this list
when v1.4.21 starts.
## Phase D — v1.4.20 reconcile carry-over
Items raised by the six-reviewer Phase-D pass that were either too
large for an inline reconcile or otherwise judgment-call-deferred.
HIGH and CRITICAL items all landed inline (fix counts in
`.planning/phase-D-v1420-reconcile-report.md`).
### Code review (MED + LOW carry-over)
- **Streaming auto-scroll deps brittleness** (Code-MED-01). MessageThread
effect lists `[messages.length, streaming?.content]`; a "" → "" flip
on a fresh stream won't fire. Add `streaming?.inProgress` or a turn
counter to the dep list.
- **`bandFromInterval` "high" label on near-zero r** (Code-MED-02).
A tight CI of `[-0.05, 0.10]` reads as "high confidence" alongside
near-zero r. Demote to moderate when CI straddles zero, or rename
the chip to "tight CI" / "wide CI".
- **Pearson p-value normal approximation** (Code-MED-03). At df=12 the
normal approx can put true p≈0.04 at p≈0.025. Either pull in a tiny
incomplete-beta implementation (~30 LOC) or raise the surfacing gate
to df ≥ 20.
- **`weightTrendAlignment` ±2 kg target band** (Code-MED-04). Reuse
`buildWeightRangeFromHeight()` so the score awards 100 across the
user's actual band, not an arbitrary ±2 kg.
- **`bpInTargetPct` held constant for "vs last week" delta** (Code-MED-05).
Recompute the prior-snapshot BP rate or relabel the delta line so the
user knows BP is held constant in the comparison.
- **WeeklyReportView print spinner gate** (Code-MED-06). The 300 ms
print timer resets on every advisor.isLoading flip. Pin the timer
via a ref so a second-render isLoading change doesn't restart it.
- **`<TrendsRow>` confidence prop dead** (Code-MED-07). Either wire
`analytics.correlations` confidence into the page invocation or drop
the prop until B3.x lands the wiring.
- **`provenanceFromJson` unsafe `windows` filter cast** (Code-MED-08).
Validate against the `WINDOW_KEYS` allow-list shared with
`source-chips.tsx` so a poisoned legacy row can't surface as
`t(undefined)`.
- **Storyboard category fallback paints purple** (Code-MED-09). Run
`advisor.payload.insights.storyboardAnnotations` through
`storyboardAnnotationsSchema.safeParse` — same shape as
`dailyBriefing` and `trendAnnotations` already use.
- **`<HeroStrip>` greeting-key + relativetime per-render `Date()`**
(Code-LOW-01). Memoise via `useMemo` so a chatty parent doesn't
re-allocate on every TanStack tick.
- **Nested `<Sheet>` portals on mobile rail trays** (Code-LOW-03).
Promote the trays to siblings of the outer drawer to avoid the
dom-focus-trap fight on iOS Safari.
- **bpInTargetRate-constant test gap** (Code-LOW-05). Add a test that
holds BP constant across both snapshots and verifies the delta
reflects only the moving pillars.
- **`streamRefusal` bypasses budget meter** (Code-LOW-06). Bump
`messageCount` (with `tokens: 0`) for refusals so the surface meter
is honest, or rate-limit the refusal path separately.
### Security review (MED + LOW carry-over)
- **Refusal path persists messages without budget accounting**
(Sec-M-2). Either call `checkRateLimit()` at the top of the POST
handler before refusal scanning, or bump `messageCount` inside
`streamRefusal()`. Same broader gap covered by Senior-MED-3.
- **Refusal patterns miss common injection synonyms** (Sec-M-3).
Extend `INJECTION_PATTERNS` with `(forget|abandon|drop|skip|toss|
bypass|circumvent)` paired with `(instructions?|rules?|prompt|
guidelines?|directives?|system\s+message)`, plus a "what
(were|are)\s+your.*(instructions|prompt|rules)" probe. Add a
zero-width-stripping normalisation pass before regex.
- **`assistantMessage` persisted before `recordSpend()`** (Sec-M-4).
Wrap the `appendMessage(assistant)` + `recordSpend()` pair in a
Prisma `$transaction` so the spend ledger stays atomically
consistent with the persisted reply.
- **`coachChatRequestSchema.conversationId` accepts any string up to
64 chars** (Sec-L-1). Tighten to `.regex(/^c[a-z0-9]{24}$/)` or a
generic `[A-Za-z0-9_-]{8,64}` regex.
- **Refusal-injection rail pollution** (Sec-L-3). Either skip the
`updatedAt` bump for refusal appends or hide refusal-only
conversations in the rail UI.
- **Smoke-test `request.clone()` against Next.js 16** (Sec-U-1).
Add a Vitest case posting a new-conversation body and asserting the
persisted user message matches the input. (Largely moot now that
the chat route no longer wraps in withIdempotency, but still useful
paranoia.)
- **Race window on `enforceBudget` between read + write** (Sec-U-2).
Acceptable in practice (cap is per-day, not per-second). Annotate
the race window in the route header so the next maintainer knows.
### Design review (MED + LOW carry-over)
- **Hero strip greeting clock is wall-clock, not Berlin** (Design-M1).
Pass the current Berlin hour through `Intl.DateTimeFormat("en-CA",
{ timeZone: "Europe/Berlin", hour: "numeric" })` so a traveller
sees the Berlin bucket the rest of the UI uses.
- **Health Score panel mobile order inversion** (Design-M3). On `<lg`
promote the Health Score panel between the meta row and the weekly
banner, or render a compact "score chip" inline with the meta row.
- **Coach drawer focus trap when both rail trays open** (Design-M5).
Set the rail-tray Portal container to the parent SheetContent's ref,
or use a `<Drawer>`-style stacked-modals primitive (`vaul` is
already in deps).
- **Tone-bar visual clip at rounded corner** (Design-M6). Inset by
`left-1` / `top-2 bottom-2`, or add `overflow-hidden` to the parent
card, so the bar's straight left edge doesn't poke into the card's
rounded corner.
- **Source chip "n=N" reads as engineering output** (Design-M8). Use
`{count} samples` / `{count} Werte`, or drop the count on mobile
and surface it in a tooltip.
- **Hero "personal-baseline" copy** (Design-L1). Either drop the
generic line or surface the actual sample size ("From 187 readings
over 90 days").
- **Health Score sub-bars paint at composite band** (Design-L2). Each
sub-bar should derive its own band per component for visual
fidelity.
- **Storyboard 24-char truncation has no ellipsis** (Design-L5). Add
`…` suffix when `annotation.truncatedLabel` is shorter than the
source label.
- **Coach composer keyboard-hint locale drift** (Design-L6).
Reconsider the German `Umschalt+Enter` translation — the rest of
the app uses `Shift+Enter` literal even in German.
### Senior-dev review (HIGH + MED + LOW carry-over)
- **Duplicated maths layer (Pearson + linear regression)** (Sr-HIGH-2).
Consolidate into `src/lib/analytics/correlation.ts` (singular) +
`src/lib/analytics/regression.ts`. Add p-value + CI as optional
outputs on the existing `pearsonCorrelation()`. Migrate
`correlateBpCompliance` / `correlateMoodPulse` /
`correlateWeightWeekday` to import from analytics. The
hypothesis-specific runners stay in `lib/insights/`.
- **`<CoachDrawer key={prefill}>` weaponises React keys for state
reset** (Sr-HIGH-4). Make `prefill` a fully-controlled prop on
`<CoachDrawer>`. Drop the `key=` re-mount. The drawer reads
prefill into the textarea's `value=` and notifies up via
`onPrefillConsumed` after submit.
- **Coach tables not in `cascade-delete.test.ts`** (Sr-MED-1). Add
three `expect(...count(...)).toBe(0)` lines for `coachConversation`,
`coachMessage`, `coachUsage`. Five-line patch.
- **`weeklyReport` + `storyboardAnnotations` lifted without
`safeParse`** (Sr-MED-2). Lift through `weeklyReportSchema.safeParse`
and `storyboardAnnotationsSchema.safeParse` in
`useInsightsAdvisorQuery`, the same way `dailyBriefing` and
`trendAnnotations` already do.
- **No rate-limit on `/api/insights/chat`** (Sr-MED-3). Wrap
`handleChatRequest` in `enforceRateLimit({ key: \`coach.chat:\${userId}\`,
max: 30, windowMs: 60_000 })`.
- **Fake streaming via `tokeniseForStreaming`** (Sr-MED-4). Wire the
provider runner to a true streaming variant with an `onToken`
callback. Unlocks "stop generating" as a real feature later.
- **`medication_schedules.days_of_week` schema drift** (Sr-MED-5).
Either land the column with a backfill default or drop the field
from `schema.prisma`. v1.4.20 worked around it twice.
- **`apiHandler`-vs-`withIdempotency` stacking** (Sr-MED-6). Largely
moot now that the chat route doesn't wrap in idempotency, but still
worth offering `withIdempotency(handler, { skipWhen?: (req) =>
boolean })` for the next caller that wants to opt out conditionally.
- **`coach_usage` redundant index** (Sr-LOW-1). Drop the `@@index`
line — the `@@unique` already builds an index.
- **`metricSourceJson` stored as `String`** (Sr-LOW-2). Migrate to
`Jsonb`. Cheap to fix while no consumers exist.
- **Coach off-topic detector locale-mixed** (Sr-LOW-3). Tighten the
allow + deny lists with a `defaultAllow=false` bias. v1.5 work.
- **`weeklyReport.weekISO` regex doesn't validate week 53/00**
(Sr-LOW-4). Tighten to `^\d{4}-W(0[1-9]|[1-4]\d|5[0-3])$`. The
route page is already strict.
### Simplify review (apply-maybe carry-over)
- **`<CoachDrawer>` slot props (`historyRail` / `sourcesRail` /
`composer`) are dead** (S-06). The only production caller never
passes these slots; `<CoachDrawerBody>` already exposes equivalent
test seams. Remove the props.
- **`<CoachInput>` `inputId` prop never overridden** (S-07). Drop the
prop and inline the default id.
- **`cloneForCheck` variable dead in chat POST handler** (S-08).
Largely moot now (the route no longer peeks at the body before
dispatching), but worth a quick re-read.
- **`health-score-card.tsx` delta-arrow chooser** (S-09). Compress
three sibling `&&` blocks into a `deltaIcon` lookup. Borderline —
judgement call for the file's owner.
## F5 — Best-practice repo audit (deferred MED + LOW)
- **README badges** (LOW). Add a small badge row: build status (GitHub
